I believe the Explanation of a Nexxus(medical opinion) letter delivered by this Physicians Assistant that was attached to a Special Forces unit was Excellent! She explained the quantitative and qualitative elements as as they relate to a DBQ(disability benefits questionaire) vs the Nexxus letter... The composition of the Nexxus letter is very thorough in delivery...the medical expert first giving a quick summary of who they are and what medical records or other evidentary info concerning the veteran that they have reviewed...followed by their professional medical opinion... For each condition...**On a side note, I have a former special forces medic(retired) friend that told me about the carpal tunnel surgeries they performed out in the field, we were stationed at old BAMC at Ft Sam Houston, Tx before he became a team member**

I am meeting with my Primary Care Dr. at the VA New Orleans and informed her about Directive 1134 thanks to you. I am 55 years old. What advice can you give when I meet with her. I am 40% Service Connected. I am trying to use current diagnosis scoliosis, radiacul l/r, degenerative disc, anxiety, sleep apnea moderate/severe, insomnia (PTSD appointment upcomg to diagnosis) While in service my bunkie dropped a grenade in bunker and it went off and I feel down a mountain. My Primary Care has agreed to help me next week so I am researching all of the possible primary and secondary diagnosis.

Thank you for the information you provide which brings value and help to veterans. I have been seeing a DC Chiropractor for many years due to Low Back Pain, which stems from service. Very little MD exposure for treatment post service. Can my Chiro complete a DBQ or must it be an MD? Army Treatment Records show vast amounts of complaints for LBP. Thank you.

I have a low back condition, L4 &L5 degenerative disc disease as well as arthritis. My primary care doctor told me that my condition could have very well been caused by my in service duty. However, she refuses to write a nexus letter saying she can’t because it’s a conflict of interest due to her being a VA paid doctor. She also said no one does nexus letters anymore more. None of my issues are in my in service medical records.

I hear that! I HATE doctors, particularly psychiatrists. "Did you ever think of killing anyone?" WTF did we rehearse for years and years? Now this paperwork is driving me nuts. "Were you REALLY there?" How about.. are you REALLY a doctor? Or just went to the University of Muckabunga in Bongaland, then get a job with the VA. Did you ever do this- on a human being?

Nexus letters can be helpful for secondary and primary claims depending on the specifics of the case. As far as your second question is concerned it really just depends on the specifics. Nexus letters are never required they can just be helpful in some circumstances

If the service connection has been established (presumptive) will I be given a C&P exam? Is a DBQ written after the service connection?

Service connection is not automatic for presumptive matters. For example if you smoked 3 packs of cigarettes per day and were in Iraq for a week, most medical professionals would attribute your COPD to smoking rather than a weeks worth of exposure. Also many people believe they have a specific medical condition that aligns with the presumptive list but they actually do not and this is discovered during the exam. A medical opinion and DBQ must be completed in order for the condition to be evaluated and rated appropriately. You will want to follow up with a legal professional for additional clarification on this.

As far as stroke is concerned it really just depends on your risk factors and the particulars of your case etc. for example if you also have hypertension or another condition considered presumptive to agent orange exposure that could add to it as well if that makes sense.

Morning, I have shoulder pain still to this day from an injury I received in the military years ago. I didn't go to a doctor but I have been seeing a chiropractor on a regular basis for treatment. do I have to see an MD or will involvement from my chiropractor suffice enough to file a claim for my shoulder pain? I take Tylenol pm to dull the pain at night.

Hi sir that is a great question. I cannot speak for the VA as I do not work for their organization. If I had to guess it is likely because they want their own examiner to provide an opinion or because they just automatically order exams without reviewing the content of the submission very well.

The VSROs or Veteran Service Rating Officers or DROs Decision Review Officers are not actually reviewing the content of the evidence submitted. They are only looking for those medical opinions and taking action on the results of those medical advisory opinions in the "NEXUS" letters. this is likely the biggest reason for denials.... The submitted evidence is not being properly reviewed.

Hello, Dr. I filed a claim for my back and neck. I won my neck claim by getting 60%. I need to do a Supplemental claim. I have more evidence that was not included in the last nexus letter. During my last 8 months in the Army, I had a varicocele hernia. My left testicle was swollen badly I had to do PT and everyday activities, and they did nothing. It put a strain on my back and had me walking not right, and I was discharged honorably like that, I have several complaints, and after service 6 months later, I had an operation for it at the VA. Is there a link for that kind of problem? Thank you.

So far ive done two supplemental claims and a higher level review. I would get an increase here and there and also they found a duty to assist error during the higher level review .But ill get items on the decision notice where I am missing certain information when ive faxed packets of medical records, dbq's. But somehow ive always get something saying about how "the medical evidence youve provided doesnt show the severity of your condition, etc etc...." which is holding me back from getting the extra percentage.... like seriously??? paralysis in the left side of my face stated on the dbq and they wannna know how severe it is? VA loves this game of tag for the last 4 years... im thinking about using your services but am discouraged by the last 4 years.

I need one for Ischemia Heart Disease, Cornonary Heart Disease, AAA, Aortic Aneurysm (of way over 5CM and Lift threating ) Aortic Valve replacement. These operations were done by Experts in their field at Mijuer Hospital Grand Rapids, Mich. May 2023 , had heart attack, while at hospital, they did CT Scan , found out I also had an AAA that so big it could burst at any thim. sent by ambulance 300 miles to Grand rapids, where they put stent in heart than I seen dr, who was going to do the AAA. I asked the doctor: how long does it take it to get this big, Dr. stated 30 years at the very least, more in the 40-50 year range. because it doesn`t grow very fast.Had that surgery on June 26th and Had the Aortic Valve replaced this Feb. 1st.. I did really good for being 79.

I just want to know what exactly do i say to the doctor to write a nexus letter? I said to mine "can you write me a letter with your medical opinion regarding my military service and headache?" And I received a 3 sentence letter with a bunch of lab results stapled to it stating in plain language, that it "may be related" to military service. So I'm requesting to switch doctor as my provider isn't familiar with the VA obviously. They can ask for opinions on your case at their discretion based upon VA policy, so if they needed a second opinion for clarification for some reason they can do that as well if it aligns with their policies and federal law etc,

That is not necessarily the case sir. The issue is that it is a federal offense for anyone to give claims filling advice unless they are accredited to do so by the VA's office of general council. That is why I recommended that you seek assistance from someone accredited to advise you on claims filling. @@jimi0710

Do you work with Lawyers on a L.O.P “letter of Protection” my lawyer going to need help with my case, i can’t win everything on appeal. The only thing I have won is Tinnitus 10%, we lost back pain, knee pain, ptsd, which I had a stim wave put in for my knee pain, Basic training accident, went through basic 4 times, kept getting injured and hospitalized. Since service I had 20 spine surgeries, all joints that were injured in there, I spent 3 weeks in the hospital, i had shin splints in there. They made me do a mental valuation , I had a profile not to March over a 1/2 mile for my knee, so my D.I made me platoon leader, causing more damage. They recycled me 4 times ! Fort Knox is the worst on backs and knees under 50lb back pack, already damaged. I rather do this right, then lose in the end?

I have no private insurance. None of my VA providers will give me a nexus letter tying my ptsd to an already acknowledged motor vehicle accident that caused head wounds and TBI. Where can I go? Helpless

When your doctor evaluate you and give his opinion after looking at your medical records should serve as a Nexus letter...wording is important, he must state he saw your service records and his opinion is " more likely than not" is related to time in service

Why is it necessary to have another C&P exam after you file a supplemental Appeal with new and relevant evidence in the form of a strong and well written nexus letter using all VA recognized language , is this what's called developing to deny ? Can the VA just render a decision on the Supplemental claim based on the new evidence without another C&P exam. I filed my supplemental a month ago , I'm just waiting on the next step .

Yes I contacted The Vet Resource Group for my nexus letter , I'm sorry I didn't choose you but they had a really high review satisfaction rate. They did an excellent job on my nexus letter , connecting my ankle to my bilateral , osteoarthritis service connected knees.

Hello Leah, what have you found with Airborne personnel after years of no problem. Then the pain and issues begin. Neck and Back pain then hands tingling, legs in pain. This is happening to me now and since 2008. I got out of the US Army in 1970. Yes, I was active 82nd Airborne.

Interesting. My chiropractor says that DC are not considered "doctors" in the VA system. So if I understand you correctly madam, he can right an opinion letter as supporting evidence to a Nexus letter. I will speak with him on this matter.

Can a compensation rating be given without a nexus or DBQ to a claim obviously associated with a secondary condition, or will it be denied out of hand without one? Ex: one has Bilateral Neuropathy already service connected, a V.A. psychologist examination renders an opinion and diagnoses a mental health issue as Adjustment disorder (SCT 17226007) based solely on the veterans claim of impairment?

So DBQ's were created out of convenience but prior to their ratings were decided often in the manner you describe, simply by evidence of record. They still CAN be decided this way however I would bet almost anything that they would not be. That would require some high level thinking on a very large scale and most often C&P's are simply triggered in an automated way. Does that make sense?

Adj Disorders are usually acute. But I have seen them evolve untreated fulminating into 100% PTSD. So MDD Anxiety would be syllogistic. Agree with Leah as to rater would have to do some high level thinking. I have seen that happen just once in a score of years. Vet wrote a letter complaing about something else unclaimed. Threw it in just for "good measure." Rater gratuitously gave him 30%. We both were flabbergasted. Moral ? Don't give up. Keep watching Leah B. videoes. :- ))

I do not combine multiple issues in my letters. Some people may do that but I do not as the letters I write are focused on the condition at hand

Good afternoon is there anyway that I can get your assistance in getting a Nexus Letter/Report I was diagnosed with Severe Sleep Apnea in Jan 2023 from the VA with blockage and the VA denied my claim because they said it’s not service related but they did give me 30% for sinusitis I am trying to refile for a secondary condition

Many vets find it almost impossible to get nexus letters because doctors do not want to stick their necks out on a limb. Especially when it comes to a V.A. nexus letter, because they do not want to get involved with a mind boggling V.A. healthcare system on steroids. Many doctors do not have the time to write a nexus letter. That is the problem I am having.

It is unfortunate because I think doctors overreact to the request. It really is not that big of a deal and they should support their patients if they believe it is related to service. I do understand the time issue

@@prestigeveteranmedical The V.A is a ramshackle hodge podge system difficult to navigate for old veterans like myself. The system is more complicated than a Rubik's cube. I have cochlear Meniere's disease. The Mayo Clinic told me only the best ENT's and otologists can understand and successfully treat the disease, yet the V.A assigned me an ordinary P.A. in the C & P exam on my secondary claim. She could not even spell the disease. She told me she had heard of it though, although she was completely unfamiliar with it. I am sure I will get a fair review.
