Reason I think Purpose (1) is Bray is because he may have first found out that he needed hurt people to turn them into their true self, one opponent that came to mind was Finn Balor cause he defeated him and Finn disappeared then came back as a heel in NXT and despite a brief face run he still turned heel and joined Judgement Day, Seth Rollins was also a face at the time he fought Fiend then later became The Monday Night Messiah, Kevin Owens had a heel run after Fiend as well, so I think this is Bray talking about needing to hurt people in order to heal them. And if we go chronological this could be the first time he hurt Finn maybe? Not saying these are targets, but that is something I always associated “Hurt/Heal” with when Bray started the Fiend persona.
