Hi Zubair, First of all i think you are saying something wrong "No Surname in passport" , which is not true please check it again there must a surname instead given name that might be empty. Its depends on situation, If you are student then it really does matter. Apk ID and education documents p nam same hona must h. No Space between two name like Johnmery with space John Mery both have completely different sense ok. I think future m preshanina bachne k ly yes please make the name same both on educational and Identity documents Thanks

You can follow your passport Details, as its already mentioned in Visa form with the reference of (**) Fields 1-3 shall be filled in the accordance with the data in the travel document. However in the Responses Document Italy embassy have not submitted their response but they have given you option in Visa form. Simply leave the Spaces and submit the form InshAhllah you will get visa

Hi, As far as i know it means that they haven't any specific requirements to mention the name , You may use your original names as it as mentioned in your passport I hope there won't be any problem. In normal practice you can leave that place blanked.

@@mohit5664 Here is a good news for you, Portugal High commissions has mentioned in the document that due to the obligation of insert at least one name and one surname, the procedure is to repeat the name in both fields. Further They also have mentioned that most of such cases are dealt when there is Pakistani Passport . So use MOHIT in both names (SURNAME AND NAME) , Application won't be rejected . Goodluck
