You could update the notes or a newly created field with the original title, configured to append new titles. If you update the record after the initial action, I would think you can maintain a history that way.

In the test mode, whether trigger conditions meet or not, the log and approval creations all execute. Right? Even though, the 'Chuck Tomasi' record title does not start with 'Sr', the log statement & approval creation completed.

You are correct. Test runs all actions. Approval records are created, update record actions update, etc. There's no roll back feature like there is with ATF.
