Completing the Acknowledgment of Parentage Form

Unmarried parents, or parents who conceived a child using assisted reproduction, have the option of signing a form called the Acknowledgment of Parentage (AOP) - available from local hospitals, local county child support offices, local birth registrars and online at childsupport.ny.gov - to establish parentage for their child. Signing an AOP is a simple, no-cost process; has the same legal force and effect as a court order; and establishes the duty of both parents to provide support for the child. This video informs parents of the benefits of establishing parentage and provides information about the rights and consequences tied to signing an AOP.

I’m at the birth certificate office in Manhattan and they are saying they accept NO CROSS OUTS on any forms and that I will be rejected. I crossed out the baby’s fathers first name, initialed, and rewrote it very cleanly. They rejected it with a cross out!
