Dr Ozello's Sports Medicine Report: Oblique Muscle Strain
Oblique muscle strains are a common sports injury that usually result from a sudden eccentric contraction of the muscle while its in a stretched position.
Oblique muscle strains are linked to sports that require trunk rotation including baseball, cricket, ice hockey, golf, tennis, handball and hurdles.
The oblique muscles play an important role in pitching and hitting. Proper abdominal muscle activation during throwing and swinging is crucial for generating optimal ball velocity and bat speed.
The incidence of oblique muscle strains in MLB has risen over the past 20 years according to a study from 2010.
Injuries opposite to dominant arm or batting side are more common.
Internal and External Oblique Anatomy: Fibers run perpendicular to one another and function to control trunk rotation and lateral flexion.
Symptoms of an oblique muscle strain include pain, discomfort, tenderness and muscle tightness. The symptoms are aggravated by trunk lateral flexion, trunk rotation, aggressive stretching and deep inspiration.

I have had so much pain on my left abdominal muscles. Went to my internist and I got a pain pill, but it doesn’t help. Had a CT scan which was unremarkable. What you are saying in this video makes sense.

Thank you, Dr Ozello. I am 64, and I think I may have strained an oblique muscle a week ago after I picked up a large stone vessel by the spout with one hand and carried it across the room to place it somewhere else. For the first few days I was in agony when getting into bed, and especially when getting out of bed, and it hurt when I put my socks on. Now it hurts less, but I still feel some pain in the morning.

Thank you Doctor. I knew it. I now have a malpractice & misdiagnoses as Concentra stated my injuries were lumbar when they are not. I specifically told them my left side & waist are strained & sprained as well as have my left bottom rib to latissimus dorsi injured You just stated all I am & have been feeling. Thank you

BJJ practitioner here. I initially pulled an intercostal muscle (exact spot was about 2" down my right nipple), rested for a while, got back to training (too soon?) and eventually re-aggravated the issue few weeks later. Long stop for about 3 months (because of Corona the gym was closed during that time) and when I got back to training after I few classes I started developing a constant yet growing discomfort/pain in the rib area, which eventually radiated all the way back to the latissimus dorsi muscles. Now I'm at a point where I can rest 2 0r 3 weeks and every time I get back to training I start feeling very soon this discomfort in the side area of my rib cage, which led me to believe that it may be an issue with the external abdominal oblique. Really frustrating stuff because it's hard to exactly pinpoint the pain. It usually starts on the rib area but it then extends to the side, kind of in the middle of my mid section, where these muscles attach. Stretching doesn't really help much, sometimes it actually make it worse. In bjj we do a lot of "guard playing" and movement with the torso and when the opponent puts pressure on you (side control position) it really hurts.. What kind of exercise would help?

If I were you I would see a medical professional for an evaluation to rule out a rib fracture or dislocation and a muscle tear. If imaging is negative, I would see a Doctor of Chiropractic to get my ribs adjusted to correct any rib subluxations (Rib out of place). I would get muscle work with mild intensity to the intercostals & obliques and surrounding areas to combat any muscle strains. I would start to perform mild stretching for the tight muscles at this point.

Recovery time depends on severity. I would saw on average it take two to three months.

I sneezed today while in a bent position and it felt like my rib popped and punctured me internally. It was a violent sneeze and I instantly went down to the ground in pain. It’s hard to pinpoint an exact location but I have pain in my lower side and it wraps around to part of my back. Im 95% sure it’s an oblique strain and I’m so bummed that one stupid sneeze will now cost me weeks of rehabilitation.

If I were you I would see a Doctor of Chiropractic to get my spine and ribcage adjusted. A rib subluxation (Rib out of place) can cause pain throughout the entire rib.

Hey Dr Ozello I strained my oblique muscles over three months ago and still struggling. I’ve favored my obliques a lot in the past due to 6 ileostomy surgery recoveries (one placement with five retractions and a mesh placed inside.) The injury is to the right of the stoma, underneath the pouch. I injured it pulling morning glory roots underground at my farm. I still experience swelling and pain and I went to a physical therapist over a month ago that pushed me too hard and put me back over a month. I’ve also accidentally aggravated it a couple of times on accident which put me back even further. I’m seeing another physical therapist now and this one seems better. I’m making sure I’m resting, icing three times a day, using a tens 7,000 unit that temporarily helps with pain, doing a little light walking (not too much), doing trans versus abdominis bracing exercise/clamshell from therapy, using CBD cream. Is there anything else I could do to help it heal faster? This has been a living nightmare and I feel it will never end. Thank you for your time.

Thanks for watching my video and for the detailed information. I'm sorry to hear about your injury. If I were you I would continue with my current recovery routine. It sounds pretty thorough. I would be very mindful to perform all exercises & stretches through a pain free range of motion.

I had one more question. Can any small amount of alcohol worsen my injury? The reason why I ask is a couple of weeks ago I had one drink the night before and the next morning I had worse swelling and some pain came back. Is there a correlation? I’ve been avoiding it now in case that was the culprit.

Alcohol usually impairs wound healing, likely by reducing the inflammatory response. Therefore If I were you I would avoid it for now.

Thank you, I believe I injured my external oblique trying to dive right in to using an ab roller vigorously. Confusing the matter though I got bloodwork done around the same time and my cholesterol / liver came back with high results. I was also freaking myself out thinking that the pain may be from my appendix. Both of those ruled out this seems much more likely (aside from needing to better my diet..) but reading some of the possible liver or appendix reasons for pain, a muscle injury in this region seems much more the culprit. Kind of bummed since snowboarding season is right around the corner, but hopefully a visit to a PT or chiro can get me fixed up before too long, plus- reading what else it could be with some of the organs near that region of pain relieved if it is “just this” Thank you for the info.

Hi Dr Symptoms: ive had lower left abdominal pain a day after doing kickboxing workouts. Diagnosis: My GP did a physical examination and said it was outer abdominal mucsle pain, i was releived that it wasnt a ventral hernia (Ive read about these online) but 4 days later the pain was still here... now im getting worried. so i investigate further... Further self research: Then thank God... i started thinking back... i was doing a lot of front kicks (body height) in my workout which involves: quickly lifting the left knee to stomach high, then thrusting the bottom of the foot forward and THEN retracting the foot just as quickly. So i thought the knee lift, and then retraction of kick, both times maybe it triggers the Psoas muscle a lot? Healing: Today is the 6th day with pain, so i finally decided to try some left psoas stretches. Lie down on back, let the back have a bit of a natural arch, knees pointing to the roof, feet flat on floor, and breathing in and out deeply, letting the abdomen pressure massage the psoas. Then put my left leg out without touching floor, and SLOWLY pull the leg up with knees bent, then slowly let down whilst straightening leg (do not touch floor), im assuming this would give a lovely psoas stretch and then let it drop and relax for a min and repeat. Symptoms HIGHLY reduced: reduced pain in lower left abdomen: when coughing, sneezing and pushing in the bathroom Final Diagnosis: So IF... my symptoms keep getting better from now with these daily stretches... i think ive successfully self diagnosed a psoas strain right? :D

Every started with a sudden contraction that left my IO tense for a couple of weeks (playing baseball). A week later I came back and with a strong swing while batting I felt like a rubber band cracking, painful line hell; I had to stop for 3 weeks. I went back and the same happened. 3 more weeks and still feels sore every time I try to swing. Now I can throw but the mild pain keeps me in fear hurt myself again. My performance went down, I'm not the same. I will take longer brake and follow your advices Dr. Ozello.

If I were you I would see a medical professional to get examination, imaging, diagnosis & treatment program. All exercises should be performed through a pain free range of motion. If an exercises elicits or intensifies symptoms, skip that exercise & use a viable substitute.

This described me to a T. Thank you, Doctor, for such an informative video. I believe I have internal oblique strain that has been misdiagnosed as everything else under the sun since 6 months ago. I have trigger points all down my abdomen (the diagonal path you describe), particularly on my right side. I have intercostal inflammation and also trigger points underneath my ribs and above my hips. NO idea what I've done, possibly some lifting at work, but I never felt an immediate injury occur. The pain has just progressed over time and is making my life a living hell. No doctor seems able to properly diagnose me. I am now seeing a private physiotherapist who has been helpful in giving me gentle exercises to try. But it is dismaying to wonder how much more damage has been done in the time spent being misdiagnosed.

Hi Doctor, I am a frequent gym goer. I was performing a deadlift and after finishing my set I felt a small bulge or cramp underneath my rib cage on the right side. I then rested a bit and attempted to clean up the weights I had, but then the same feeling came again so I laid down to rest even longer (2 mins). Got up felt fine and finished the rest of my workout with no problems. Next day, I sat down for majority of the day not doing much physical activity. As I was sitting down I was leaning with most of my body weight on the right side. On that side in the lower external oblique (on my stomach near my groin/hip) I experienced a dull ache with a burning sensation. The area was red, but the experience has gone away and cleared up. I felt lmited moving that area for about a day, but now I feel soreness that seems to fluctuate between my lower back/side or my groin/inner hip area. I never hear too much about burning sensations when researching this so I'm curious as to if this is an oblique strain. Also would it benefit me to follow the rehab process?

hey doctor. how are ya. please I hope you really help me with my injury. story short, I am an intermediate bodybuilder and i had an oblique strain after doing Russian twist with weights two years ago and i had stopped lifting weights since then. now I am on comeback to build my body again but unfortunately I still feel some symptoms of the strain during the day like tightness and discomfort feeling around that area especially when I go to work and start typing on my computer this tightness gets a little intense and it's really bothering. and thank god there is no pain at all even when I'm doing my weight training exercise only this tightness which sometimes pops out of nowhere . please tell me if there is certain starches or exercises which help me recovering completely. I know I'm late and I didn't seek help early and only that because I don't have doctors or experts in sport injuries around my area. sorry for this long story but really i Need some help. Thankx

It is extremely difficult to recommend the exact stretches that you need but three general stretches that may help are: Fitball Oblique stretch, Pull-up Bar hanging Traction & a standing stretch.

Hi, Doctor. I'm hoping you're still replying to comments/ questions on this video: Are _strains_ and _spasms_ the same thing, or rather, are spasms the result of strains? I've been experiencing _spasms_ on both sides of my trunk, in the area that seems to border the serratus and oblique muscles. These spams are mostly triggered when I'm at the gym, but some instances have occurred when simply sitting still: 1) Engage in _pulling_ actions, e.g. lat pull-downs on the machine, as well as when pulling in a horizontal angle, even with little to no weight, as when I'm warming up with rubber resistance bands. Oddly, no spasms occur when I'm on the Concept 2 rower. 2) When performing sit-ups/ crunches. The latest episode occurred after I had performed sit-ups focusing on the negative end of the movement. Not long after I reached the floor to rest, both sides started going into spams, eventually cramping up...... I could not even roll to one either side to attempt to get off of the floor. .... It took about 20 minutes of heavy breathing/ breath-holding to wait out the pain before I was able to roll and get up. Thanks for your time.

Thanks for the question. Strains & spasms are not the same thing. Spasm can occur for many reasons & a muscle strain is one of them. If I were you I would try changing my routine & eliminating any exercises that elicit or intensify symptoms. Here are links to several exercises I would start with.

Can I ask a question, I did not train the oblique muscle from the beginning, and when I started practicing the oblique board exercise, I had strange symptoms, which are abdominal swelling without pain in the oblique and bloating, poor digestion and gases that have not subsided so far, despite leaving the oblique exercise, did I weigh because of this exercise. Can it be cured

When this injury occurs is it always on the opposite side or can it be the same side of use. I’m right handed, after boxing and kicking the bag with my right leg I felt like my right oblique felt like a sore muscle. Not debilitating just achy/ sore. I didn’t really stretch before starting and threw a lot of powerful punches and kicks from my right side. Thank you!

Hi Derek. An oblique strain is more common on the opposite side but can occur on either side.

Hey is it possible that a prolonged stomach bloating (4-5 days) can cause oblique strains? Because right now im currently experience a painful muscle strain on my right side external oblique, its even influencing my leg and my knees feel like somebody keeps pinching them. And I am sure that before I started having sensations of strain on my right oblique I was suffering with some sort of an ulcer or a gastritis that causes my stomach to bloat everytime I eat. It is very alarming knowing how weak my back muscles really are relative to my size and weight and not to mention in my family I am the only one plagued with frequent stomach issues.

I'm currently suffering from oblique muscle strain on both sides caused by overuse of the overhead dumbbell extension. Had too much weight on and not the right position. Thanks for this video. If I was in the US I would come to you for treatment. Here in Switzerland this injury is new to doctors and my chiropractor and GP are baffled by the situation. So I think I will tell my chiropractor to refer me to a physiotherapist or sport injury specialist.

Here are a few guidelines to follow when rehabbing your obliques strains. Always perform stretches and strengthening exercises through a symptom-free range of motion. Move into a stretch slowly, hold a mild comfortable stretch and move out of the stretch slowly. Execute strengthening exercises slowly and focus on the eccentric phases of motion. thanks for the video...I'm 75 and was very active...golf 5 times a week, etc...spinal stenosis has stopped me in my tracks...to relieve some of the pain I had a spinal cord simulator implanted...all was well until 2 months ago when I had surgery to implant a newer battery/computer...it is implanted at the top of my butt on the right side...after the wound healed when I began walking I developed what I think is an oblique strain...all the symptoms...my question is...can this be a result of the surgeon making "room" for the batter/computer ? he does not thinks so...pain management doctor told me not to stop walking just don't go for an extended time...but when I do it aggravates it...anyway I would love your thoughts and input...thanks

Good question. If I were you I would continue walking and would consider other types of low impact exercises such a bike riding. The oblique strain is very interesting. As a general guideline, if one area is not functioning properly another area has to take a great work load. The increased work leads to injury.

Thanks a ton for this information. I strained my right oblique last year while trying to strengthen my core (to unload my hurting back). Now it just keeps coming back every time I work out. The pain is not that bad and no damage can be seen by ultrasound. Can you please give me some example exercises I can use to slowly build up the strength?

It tends to hurt after I’ve massaged it for a bit, but my theory is that a massage will rip apart weaker parts of the muscle, while keeping the stronger ones.

Dr Donald A Ozello DC thank you! Mind me asking another non-related question? I’ve suffered from lower back pain for 1.5 years now. I’m only 26 yo, quite fit and active. I’ve seen many doctors and chiropractors, non of which can find any wrong with me. I’ve done an MRI-scan, which showed nothing bad. My back seems to take over too much work load and it’s always stiff. Also my knee-jerk reflex doesn’t seem to work - that might be related to my pain, I don’t know. The doctors say I’m fit, so they can’t figure out why my back is so stiff. I know this is quite the long shot to ask over KZbin, but I’m getting desperate. Thanks a lot!

Hey Doc Can the obliques or abs muscles get a strain from hyperventilating due to severe trauma and emotional stress? Ever since the altercation my right oblique or ab is so tight and not at all like the left side. During the hyperventilating I felt a very hard severe pulling inside next to my hip. I have very strong abs usually but ever since I can’t really use my core anymore without heavy pain on the right side. I do suffer from ehlers danlos so I’m super overmobile. I can’t really workout like that atm so I am super worried. How can I fix this ? I’ve been resting and stretching for 3 weeks already hoping I’d get better but it won’t

Hello Dr. Ozello, can you please help me? I felt a zap (a long tweak) in my obliques from right to left about 2 years ago while I was doing a standing-over head press. I didn't feel anything when it happened-no redness, no swelling. Since then, I've been feeling pricky sensations in my right and left obliques. It feels like being poked with pins and needles, and many times there is a random dull cramping sensation in the obliques and also burning sensations. I consulted many physiotherapists and orthopaedic doctors, but nobody understood what was going on with me. They gave me physical therapy exercises, which did not help. I did a CT scan and an ultrasound sonography, but none of these tests found anything abnormal with my abdominal muscles. I haven't been in the gym for two years because of this. I don't know what to do or where to go.

You're welcome. Thanks for the great question. Sports Hernia is a nickname for Athletic Pubalgia. This condition usually takes a long time to heal. If I were you I would see a Doctor of Chiropractic whose specializes in Sports Injuries for evaluation and treatment. I would request an MRI to confirm diagnosis. Surgery is an option for cases which do not respond to conservative care. I can not give an percentage.

Great advice. should I do stretching without warmup? should I do stretching every day? Do you advise me to perform upward-facing dog as stretch for oblique muscle? is it good for me to do planks every day? how much time do I need to get fully recovered?

Hi Asem Moqbel. Great questions. I recommend that you perform light & slow dynamic motions before stretching to prepare the muscles. The best time to do a stretching routine is after your workout or run. You can do light stretching at work. Take a short break every fifteen to twenty minutes by standing & doing a mild stretch. You can also perform light & slow dynamic movements. You can stretch everyday but I recommend you vary the stretches & the order you perform them. Up Dog is a great stretch, it will target the obliques indirectly & should be beneficial. Planks are a great exercise but should not be performed everyday. Give your body adequate rest between workouts so your muscles can adapt & grow. It is almost impossible to tell how long a full recovery will take. Each case is individual. Be diligent & consistent with your recovery work to ensure a complete recovery & reduce the possibility of a recurrence.

I hurt my left oblique muscle while using rotary torso gym equipment for training abs. My question is should I stop doing workouts all together and let the muscle rest? Should I stop abs and back workouts or all workouts? I have been using heat pads every day and it has been a week with no improvement. What else can I do?

If I were you I would focus on rehab of the injured area, I would follow these basic principles. Never perform an exercise or stretch that elicits or intensifies symptoms. Always work through a symptom free range of motion. Use slow and controlled motions.

Thank you for the information. Do you think it can happen due to a dog pulling? My dog is about 75 lbs and jolts out when he sees another animal. I had a ct scan done today with and the dr. ruled out appendicitis. She thinks it’s a muscle strain. All I can think of is my dog pulling me. 🤷🏽‍♀️ thanks!

Very useful information, thankyou. I feel I have done this exact injury while doing a big number of burpees ect. This has been sore for around six weeks now but I've continued to train unbeknownst that it might actually be an oblique strain as I've had issues with my back at the same height as the oblique. An oblique strain can occur from "doing too much too soon" and inadequate rest & recovery between workouts. If I were you I would concentrate on stretching and other recovery techniques to rehab the injury. Yeah these are really easy to get, I literally got one like like an hour of boxing with a heavy bag. I do a lot of hooks and I put all of my strength into them. That and the costochondritis I also got probably means I'm not working out anytime soon.

This is interesting , I have a pain in my left side and it can radiate from just in front of my left hip to just in back just above the illiac crest. I am a right handed table tennis player and twist to the left hundreds if not thousands of times. Can this cause this? I am also noticing that it can cause my stomach to feel slightly "off" and even sometimes into my groin. . I had a pelvic CT scan which didnt see anything crazy going on. It comes and goes. Can I ice it? Thank you Dr.

Repetitive motions of twisting & bending can cause an oblique strain. If I were you I would apply ice & see a Doctor of Chiropractic to get a proper diagnosis & treatment. MRI & Diagnostic Ultrasound may help conform the diagnosis.

Most often the oblique strain occurs on the non-dominate side, For example a left handed baseball batter will usually get an oblique strain on the right side. Oblique strains can occur through repetitive motions of twisting in the same direction.

I recently picked up golfing. Love the sport. I felt my right side start hurting one day and kept playing. Anyways, now my upper side from under my armpit all the way around to my upper ab area feels like I'm being stabbed when I lay down or raise up from a seated position. I don't have a lot of options here but who would I see? A regular doctor, a chiropractor? Or see if the pain goes away soon? It's been 10 days not much change if any. Thank you.

Great question. If I were you I would see a Doctor of Chiropractic for evaluation and treatment. I would not wait to see if the pain goes away, because symptoms may lessen but most likely the source will remain. Therefore it may recur.

Is there low recovery rate for this injury? I may have strained mine yesterday doing rows. Going with the weight I would do on an olympic bar to one-handed variant, I twisted at the top while holding the valsalva and felt the small burn around my lowest rib. I didn't feel anything the rest of the day, but I feel the inflammation in the area constantly this morning, and tenderness if i move wrong, twist, cough or laugh. Does this sound like an oblique issue?

It is possible that you strained an oblique. If I were you I would see a medical professional who specializes in sports injuries to get an evaluation & proper diagnosis. This will help set you in the correct recovery path.

What if the tear is down near the pubic bone? Tore the external oblique and rectus abdominis. So hard to feel any strengthening in that area?

i think i tore my oblique by riding my horse without stretching beforehand.. the pain happens when i lope or trot on my horse and it feels like a sharp, red hot needle shooting through my left abdomen right above my hip. Does this sound like a tear?

Great video and thank you . I really believe I injured my right oblique last week overdoing a core exercise . I went to the ER they found a tiny stone in my right kidney and assumed that was the trouble since it’s on the same side . I went to a urologist and he felt it was to small to cause that type of pain/ swelling and spasms that where a curing . So at this point I am 100% it’s a badly sprained oblique , how do I allow this thing to heal without living on 800mg Motrin every 8 hours ? I am almost 60 and was trying to get in better shape but did way to much obliviously .😔 As soar as I am would coming in to get therapy help? . It’s tough to walk upright to be honest until the pill kicks in . Thank you Eric

I’ve been suffering with this for years (age 15 to now 32) & just found out what it actually is. I honestly thought it due to my bulging disc. It impacts my daily life. So much pain and I’m sick of it 😮‍💨 How do I start to heal this at home?

If I were you I would start performing slow movements through a symptom free range of motion. Then gradually progress to static stretches through a symptom free range of motion. I would perform isometric contractions of the core muscles to build strength and endurance.

Hi Dr., I've done my urinalysis and ultrasound and there was no problem in my urinary tract and organs, meaning it's all clean. I feel a muscle strain in my lower abdomen and oblique area, is it normal to have a feeling of frequent urination due to the pulled muscles on that area?

Well I’ve been in pain since late January and my doctor wants me to heal on my own and I don’t see her until May. Before I was in pain to the point where I felt like I should go to the ER. Now I’m still in pain but nothing like before. I feel like It’s taking me forever to heal. I can’t rest because I have to lift somewhat heavy stuff at work, but I’ve been wearing a waist belt. I’ve been doing massages and using the heating pad as well but still in pain.

Recovery from an oblique muscle strains usually take a long time. If I were would continue massage and heat and I would see a Doctor of Chiropractic for evaluation and treatment.

@@drdozellodc thank you sir. I’m no longer having intense pain, but my left abdominal and lower back gets sore when I push or lift heavy items. Still haven’t been able to workout as well. My X-ray and ultrasound results didn’t show anything bad though

I have had side pain (tender just below ribs on back) for months now and have been doctors several times and still unsure of what's wrong. I'm a dog walker and have multiple dogs on each arm and sometimes they pull, as its my job I'm unable to take time off to rest. What can you suggest please?

If I were you I would give the overstretched area a few days to recovery by avoiding stretching for at least three to four days. When stretching always move into the stretch slowly, hold a mild comfortable stretch and move out of the stretch slowly.

Hello Ws Ford. Thanks for the question. The recovery time of an oblique strain depends on the severity. Oblique strains may take longer to recover than other muscle strains. Be patient, stay positive & be consistent with your recovery work.

Hi Dr Ozello. I was shoveling snow three days ago and then went grocery shopping. While loading the cart with 3-32 pack of bottled water I felt a pop or like a bulging feeling in my right side. The pain was immediate. I've tried icing it, heating pad and resting with little relief. Then I had to shovel again yesterday and now I can barely move without a spasm of pain in my right side. Sometimes it feels dull and radiates to my mid back or abdomen. I don't have any other symptoms and I recall this type of pain back in the Fall after kayaking but not nearly as severe.Do you think I reinjured myself?

Shoveling snow is an intense activity which most likely fatigued the muscles. When lifting the cases of water bottles the fatigued muscles were injured or re-injured. It sounds like you had a recurrence of the injury you had in the fall from kayaking.

Greetings from Reno and thank you for this video and the others referenced here. I pulled/strained my right oblique playing golf last fall. I laid off for several weeks, felt better and resumed playing every other day without pain. Now, 6 months later, I am having the same pain in the same muscle. My question is, in addition to starting to slowly stretch again, and then strengthen, do I need to wait until I am completely pain-free before returning to swinging a club? Obviously, now that this has happened twice, I am worried about recurrences, but I also do not want to lose my entire summer golf season!

Excellent question. If I were you I would, once symptoms have lessened I would ease back into my golf game. Starting with a hitting a small bucket of balls, taking several rest days then repeating. I would slowly increase the amount of swings in small gradual calculated increments. Also, I would look into my swing since a slight modification of technique may help.

Hi J Smith. Great question. If I were you I would start by performing a mild comfortable stretch to reduce muscle tension. If pain persists or intensifies I would seek professional treatment for evaluation, diagnosis & treatment. When you return to training I would add variety to the workout by targeting the spine, glutes & posterior shoulders.

If I were you I would schedule to see a Doctor of Chiropractic for evaluation, diagnosis and treatment. I would also perform mild stretching exercises through a symptom free range of motion. Move into the stretch slowly and hold a mild, comfortable stretch.

Hello Dr, I think I have strained my oblige muscle many months ago and it has only become worse. My glutes and hamstrings are now affected and I am unable to sit down or have a proper sleep as a result. I’ve tried to train my obliges however I’ve only made it worse now I’m stuck I don’t know how bad I’ve stained it but seems to be aching all the time. It’s affecting my day to day life aswell as not being able to work. It has ruined my life and I don’t know where to go from here. Can you advice or if possible to have a video consultation with you.

Hello Mana 69. Thank you for watching the video & for the question. The oblique muscles are involved in a multitude of motions & functions. Symptoms of an oblique strain can be provoked by running, laughing, bending & twisting. If I were you I would see a medical professional & request an MRI to determine the correct diagnosis & extent of injury. This will help direct you in the correct path of recovery. If an exercise elicits or intensifies symptom forego that exercise & find a viable substitute. Exercises should be performed slowly in a controlled manner through a pain free range of motion, even if that motion is severely limited. Be patient in your rehab.

Good morning Dr. Ozello. Last Thursday I reached far over my kitchen sink to get something in the corner of my counter and pulled my oblique muscle bad. I could tell right away, but it didn't start hurting really bad until the next morning, which was about twenty-four hours later. It hurts when I breathe it hurts when I talk it hurts when I move. I started taking ibuprofen which helped a little, but did not ice it or put heat on it. Is it too late to put ice on it now? Should I go in to see a doctor? It is still very painful. I'm wondering if I tore it or if I just trained it.

If I were you I would see a medical professional for evaluation just to be on the safe side. I would apply ice to the injured area for 15 minutes and make sure that I used a towel between the ice pack & skin.

Thank you for the informative video. My most upper left oblique is very sore, tight, it’s uncomfortable to take full, deep breaths, and I can even see some swelling. It concerned me at first because I don’t recall an exact moment when I may have strained it. But, I’m constantly lifting a 27lb 1-year old up off the floor all day, so I’m wondering if that may have done it. Is it normal to see swelling? It worries me at first but then I thought, “Surely muscles must become inflamed when they’re injured...” ?? I’m right handed, and the injury is on my left. So that also aligns with what you’ve explained. Thanks for any feedback. I really appreciate it.

Good evening doctor I'm currently a student of orthapedics and i have a question for you Recently i have been coming across conflicting information and controversy in regards to lwb pn and poor function in the thoracic region. In my own research I noticed in alot of strength and conditioning sports the valsalva maneuver is utalised to increase abdominal pressure and protect the spine; however alot of tradesmen and clients with lower thoracic dysfunction are not aware of this maneuver. In the past I have worked as a labourer in various position and i have never been instructed to "brace the core" when undergoing heavy lifting. I have recieved conflicting feedback on this idea or topic. I was wondering if you have any feedback, comments or references Thank you in advance

Hi James. Excellent and intelligent question. I agree that there is conflicting information. I don't know of any research that I can immediately sent you links. Most of the strength and conditioning coaches and fitness trainers I know instruct the lifter to brace the core. Personally, I have used this technique and it seems to work well. I always instruct my patients that if any exercise, stretch, position, movement or technique elicits or intensifies symptoms then they should stop immediately and find a viable substitute.

Hello, I am hoping you can help guide me, it is hard to find a doctor who deals with this or even diagnoses it. I have tried. To make a long story as short as possible I was allowed to be severely distended for 3 months because of a hernia. I looked 9 months pregnant. I distended on January 6th and eventually began to wear an abdominal binder mid-February. My abdominal muscles failed on March 29th. I finally had my hernia repaired on May 15th but no one deals with my abdominal muscle issues. My PCP does not know how to image them and he has no clue. I just started with a Physical Therapist who says my fascia is all glued together but does not know how to image to see how much damage was done on both sides. I had worked myself up to being able to be upright for short periods of time and then rest. I was an unproductive upright person as I was unable to do anything. I hope that makes sense. I became impatient and when the horses went through an electric fence on August 13th, I so wanted to fix things and took the weedeater out to the pasture and tried to find areas of grass that would be grounding out the fence. My husband warned me that this was the one thing that could injure me the most and yet I was pig headed and lasted for less than 45 minutes of intermittent using the weedeater. It landed me on the couch for more than 6+ days and then when I was finally able to start getting up, my right hip rotated and moved up when I went for my first appointment with the PT and she was assessing me. I know it is from the weak/injured/non-existent abdominal muscles and the pain on both sides is in the external oblique direction of hands in pockets. I am extremely discouraged. Trying to keep my hip in place, doing exercises to strengthen and put the hip back into alignment, and she is working on keeping my entire body from being tight in places. It is like we are chasing things, but I am not getting better. I see a GI doctor at the U of W on Thursday and they told me that he does abdominal muscles (I am very doubtful), It is a 3 hour drive for us and travel in the car is hard on me. My husband has to take the day off from work and then the whole day is wasted. I would like to know what kind of imaging to ask for if this doctor does not do abdominal muscles. My PCP said it is hard to image the abdomen because of respiration but no one has offered ultrasound or anything else. In all this time, the surgeon who did the hernia surgery did not know what to do with me, so made this appointment for me and I have waited 3 more months. My PCP does not know anything and in my small town, I don't know that they would know how to do proper imaging or have anyone able to read it. At the U of W, they should have someone. If the GI doctor does not know what he is doing, I want to ask him for specific imaging and then an appointment with someone who does...very quickly as everything I have learned is that this is not an injury that should be allowed to linger. If the PCP had sent me for hernia surgery in February, I would not be dealing with this. I feel like I am an anomoly in that most oblique injuries occur from sports, not from the abdomen being severely distended for three months before muscle failure. I don't really understand what occurs in muscle failure, I just know how it disabled me and that I am a nonproductive part of society that is in pain and I know that it is not nerve pain and will not do a Tapp-block or take nerve pain meds as that is not the problem. The inablity to move due to muscle weakness, ride in the car, bend over, having the weed eater cause such severe pain and disabilty after 5 months of rest, no lifting, no carrying, I cannot do anything and it fatigues me to be upright even in sitting. I hope you can point me in the right direction as coming to Nevada is out of the question. Making it to Seattle is difficult enough. Imaging the abdominal muscles is difficult but If I were you I would request an MRI & Ultrasound. These tests will help determine the location & the exact of your injuries. Your injury is long standing so most likely your recovery will be long & slow. It sounds like your PT is on the correct path. Your rehab routine is dependent on your imaging results. If I were you I would start with very mild isometric contractions of the abdominal muscles for two seconds while in a supine position. One to four reps every other day.This is a good starting point & you can progress very gradually.

Hello doctor I have been doing physical therapy for a muscle strain and I was able to finally get back to running some and on one run I had sharp pains in that spot and through the next few days to now it has gotten much worse. I can lean over and feel it and it is tender and feels like a knot. Is that part of the strain that I might of injured that part. I also was wondering can that pain radiate to the hip and groin? I lay in bed and can feel the muscle aching dull pain too...

Most likely, you over did it & re-aggravated the strain. If I were you I would ease back into my workout by increasing intensity in very small calculated increments.

I am 17 and a while ago at work i bent over super quick to pick something up from a low area and suddenly i heard something pop by my lower back and as soon as i went to go back up i had the biggest pain on my lower left side of my back, it was super quick and went away and didn’t think anything of it then the next day it hurt to get out of bed, bend over, and when i go back up the stairs

Thanks for the great info. My injury occurred 5 days ago while golfing. Got worse the first couple days, but pain (typically when I take a few breathe) has slowly gone away each day. When do you recommend it is safe begin core training or playing golf again?

When my range of motion is full and symptom free I would begin isometric strengthening exercises and practicing my golf swing.

Hello doc, I was doing round house kicks in Muay Thai and after a while I got sharp pain in my side around the lower rib cage. That night it was terribly painful to move or stretch. The next few day it’s been a lot better but still hurts of I turn a certain way. Do you have a video on what to do to recover from this?

so my son just got a trampoline and has been learning flips, he says he has a sharp pain under his left rib when he jumps, it may be his obligues, any advice?

Hello Doctor, I am writing you from India. It seems I strained my Obliques while doing a Russian Oblique twist during an exercise session. Its been 3 weeks now and my pain is not fully gone. it is in my right side just above my thigh area. I am sometimes able to do slight stretching, but sometimes that aggravates the pain. I was planning to start swimming. can I do swimming now or should I wait till I am sure it is fully healed

Doctor.. I'm located in Mexico and I have seen multiple doctors. chiropractors.. you name it.. I have pain in the right side of my neck, shoulder, arm combined with numbness. If i'm sitting down my body shifts to the right side... kinda like it collapses and I have moderate pain which can be big sometimes. now why is this relevant with this video? well one thing I have felt during all this years is that when stretching, my oblique muscle has very short range motion and it doesn't progress as i stretch and it's very tender. since my major pain is around my ribcage and above the hip.. I started ti think that it might be an issue of oblique strain... hopefully you can take the time to respond as I have been with this pain for over 5 years and i'm really tired of not having a normal life :(

Hello Jose. Thanks for watching & thanks for the question. It sounds like you have several conditions. If I were you I would get an MRI of my Cervical spine to determine the source of the neck, shoulder & arm numbness. I would also get imaging done to determine if a torn oblique muscle is present.

Hey Doc. Baseball pitcher here. Unsure if my pain is an oblique strain, another doctor said it was lumbar joint restriction. It’s on my throwing arm side, but hurts my outer core and middle back. Does it sound like an oblique strain?

Hi Max. You can have both. Lumbar Joint Restriction is a Chiropractic diagnosis meaning that one or more vertebra in your lumbar spine are subluxated. Oblique Muscle Strain means that you injured your oblique muscle.

Maybe I have a left oblique strain that feels like it's right on top of my hip cradle and can also affect my quad and also the outside of my knee. I used knee compression because it felt like the pain was then going below my knee. Is this bad?

When an area is injured or not functioning properly another area has to take a greater work load. The greater work load can lead to injury. If I were you I would see a medical professional for evaluation, treatment & rehab recommendations.

Hello sir, I am 12 and I tried to run some distance for the first time in sometime and the oblique strain, I think, has happened. It has hurted for about five days and I would love an advice from a doctor. Is it strange if sometimes, breathing deeply in causes pain. This has been a very useful video to help convince me that I have an oblique injury... and not a sarcoma ! What worries me is the time it's taking to heal... it's been 12 weeks and whilst not crippling it nonetheless has stopped me from having an active summer.No cycling, hiking, swimming or weight training. I'm left handed and have a manual job testing foundations. This entails drilling into steel and concrete and just prior to the injury I had to drill 36 holes in a short space of time. Felt ok at the time but within a few days the soreness set in on my right side. I'm in my mid 50s and need to drive a thousand miles per week to visit sites so that's not helping either ...moan, moan, moan. Most web sites state 6 weeks should be long enough for a recovery … not in my case. Anyone else ?

@@freestylebully4849 I do seem to have recovered ...but it took the best part of a year ...and to be honest, I haven't really put it to the test. (been lazy) I'll try to up my fitness regime in the coming weeks/months ... but now, apparently, I've got tennis elbow . ( I'm falling apart !)

If I were you I would get advanced imaging to confirm the diagnosis of an oblique muscle strain. If the diagnosis is an oblique muscle strain I would see a Doctor of Chiropractic & a Massage therapist. I would perform static stretching for the oblique muscle. Moving into the stretch slowly & holding a mild comfortable stretch.

Greetings Dr. Ozello. Maybe you can help me. I had a core injury recently which prevented me from working the core for a few months (happened while doing leg presses on a machine that was too upright. I still don't know what exactly was injured, but it was extremely painful and debilitating.) I've recently been able to resume core exercise, however about a week ago I was working my obliques, and I felt something like a strain, right where the right external (or internal) oblique meets the ribs. It is a dull pain that exacerbates if I try to stretch the right side. I stopped core work to allow for recovery, and the pain has gone away. Then last night while sleeping I went to turn and felt a prickly pain along the lower ribs (sometimes on both sides of the ribs when I sleep face up and stretch my rib cage). It feels akin to tearing a piece of paper very slowly. I take it my core fascia is weak after all those months of inactivity and it tears slightly every time I over stretch it, but I would like to hear your thoughts. To summarize, 1)Are oblique injuries at the rib joint common? 2) Are fascia tears (what I THINK those prickly pains along the rib are) common during stretching and recovery and something I just need to go through on my road to recovery? Thank you so very much for your time. It has been about 5 really frustrating months

Hello Tony. Oblique strains can cause pain along the entire length of the muscle, near the ribs, where the muscles attach to the ribs & where they originate. When you stretch always move into the stretch slowly, hold a mild comfortable stretch & move through a pain free range of motion.

I don't know about chiropractors in my area. They like to jump on you, ram you against a wall. I'm injured and I don't need rough treatment right now or ever.

If I were you I would see a Chiropractor who uses a non-force technique or Activator technique. I would contact the office and ask what type of technique they utilize.

If I were you I would see a Doctor of Chiropractic for evaluation & treatment. I would start my rehab with slow range of motion exercises, progress in small calculated increments to stretching, then strengthening exercises, followed by throwing specific exercises.

Hey doc I’m pretty sure I have this injury right now, I did it while lifting weights and playing basketball right now pain is kind of severe hurts to breath and even speak sometimes, any movement I do gives me sharp pain I have even been taking pain medication because of how bad it feels which I usually don’t do, I’m pretty sure it’s an internal oblique strain and I have been feeling this pain for 5-6 days now,my question is when should I start stretching it out and how do I go about handling this issue also when do you think this pain will at least start to go down?

If I were you I would see a Doctor of Chiropractic for evaluation and treatment. I would request soft tissue imaging to rule out a muscle tear. If no tear is present I would start very gentle stretching exercises. Holding a mild comfortable stretch and moving through a pain free range of motion. Pain intensity and duration depends on severity of injury.
