Topical Minoxidil has been used for many decades to promote hair growth. Due to the potential side effects of high dose oral Minoxidil for hypertension, the oral formation was not used for alopecia. However, low dose oral Minoxidil seems to reduce the side effect profile of the mediation without any decrease in its effectiveness of promoting hair growth. This video looks at the various hair loss conditions where low dose oral Minoxidil has been used, the dosage needed, the potential side effects and a dosage strategy for its use.

Thanks for not dumbing this down and actually helping those of us who need to know the pluses and minuses. My doc gave 2.5 2xd and I’m in another bp med. I’ll take your advice over her prescription any day. Don’t twant to die in my sleep.

With long term oral medications for a chronic conditions like hair loss, the usual protocol is to start low and work up according to the response. Generally, in the Uk we start at less than 1mg for women and 2.5 mg for men and then increase every 8 weeks or so dependant on the response. The dose should be tailored according to the individual persons response.

Hi Dr Diwa, what about oral minoxidil's effect on collagen? Dr Hasson ( a reputable Hair Surgeon) has gone on record saying that HT patients who have been using minoxidil have degraded scalp skin.

Interesting point, thanks for brining it up. I have not read any reports of problems with collagen when oral minoxidil is used. I could not find any reference on this topic on a PubMed search either. Perhaps it is a possible effect of topical minoxidil? There is no mechanism I can think of wherein oral minoxidil can affect collagen. With topical agents, some of the other constituents in the solution or foam can irritate the skin and therefore have an effect of the local collagen.

I tried topical minoxidil for 3 months and developed topical dermatitis. I attempted to apply to my arm and chest to test if I would develop skin irritation and did happened a redness and bumps. Can I attempt oral or I need and allergic test to this medication? Thank you for your great video! Peter

It appears that you are allergic to one of the constituents of the topical minoxidil. It is very rare to have an allergy to minoxidil, so its likely to be one of the preservatives. Its certainly worth considering oral Minoxidil. Maybe take a test dose of 1mg and if you experience any allergic effects, then avoid it. I suspect it is unlikely to cause problems though as oral allergy is quite different to contact allergy.

Thank you so much for this video.🙏🏻 I (F, 41) was just prescribed Minoxidil (Loniten) 2.5mg (1/2 a tablet every night) and Teva-Spironolactone 100mg (1 tablet every night). I have female pattern hair loss and according to my dermatologist this could be genetic. When I asked my dermatologist if there could be any side effects after starting this treatment she said no. However, I just read the information sheets that were given to me by the pharmacist and the first thing they say is that Minoxidil can cause serious heart problems and worsen chest pain amongst other potential issues. Reading this made me feel very uncomfortable to say the least. Knowing that I'll have to take both of these medications for the rest of my life I was wondering if there could be any long-term side effects, or any other issues? My dermatologist said that we'll do a blood test every 6 months to monitor the kidneys and potassium levels. I was also wondering if it's allowed to drink some alcohol every once in a while while on both medications. I usually don't drink much (maybe 1-2 twice a month at the most), but would this mean that I would need to give up alcohol all together? Is it also allowed to take Ibuprofen and Acetaminophen when on this treatment? I deal with migraines almost every month, and usually take both painkillers together. Thank you kindly.

The doses of minoxidil used for hair loss are usually very low and it is unusual for it to cause heart problems or chest pain. It also generally does not interact with alcohol so an occasional drink is allowed as are painkillers. I'm a physician. This was way more helpful then I thought it would be, so thank you. I take minoxidil. took 1.25 bid for 3 months with improvement but felt like it more paused hair loss than improved density so with my doctor's permission increased to 2.5 bid. Now having the sensation that someone is poking their fingers constantly under my xiphoid and an increase on inotropic effect on heart beat but no tachycardia or pedal edema. I haven't heard any diminished heart sounds or rubs. I'm wondering if there is a difference in taking daily vs bid and have considered doing 3.75 daily.

Thanks for your kind words. As you mention, it may be useful to take 3.75mg a day as this may reduce any cardiovascular side effects. Taking it at night before bed is another useful strategy. At present, there is no evidence that taking it twice a day is more effective than once a day.

Hi doctor, I have been using topical minoxidil for over 10 years, and I intend to start treatment with oral minoxidil 2.5 as well. Do I expect fewer side effects due to the previous use of topical minoxidil? And is it worth continuing with topical minoxidil at the same time as using oral minoxadil? Thank you very much

Oral minoxidil is easier to use than topical minoxidil. Its less messy and does not irritate the skin. Side effects of the tablets depend on the individual but at 2.5mg it will hopefully be minimal. Using just one would be sufficient as they work in the same mechanism. Maybe have an overlap period of about 4 to 6 weeks to give the tablets a chance to kick in before stopping the topical minoxidil if you are worried about sudden cessation of the topical agent.

There is no particular regimen suggested in the literature. One option may be to gradually introduce the oral minoxidil and then once both are established, gradually reduce topical minoxidil and stop it.

Hi, most GPs will be able to prescribe KT for you. Sometimes they may want a dermatologist to assess the hair condition first. It may therefore be useful to have a consultation with the local dermatologist as they will want to examine the scalp and make sure there is no other condition co existing with the reduced hair density. I can prescribe it if you live in the UK bit as mentioned an assessment is needed first and that I best done by a local dermatologist. I practice in the Northwest, in Chester.

Oral minoxidil is relatively new and we do not have long term data on it yet. I suspect there will be some loss of efficacy with time as the androgen sensitive hairs will undergo some reduction due to the hormonal influence

I took a few days off from the drug then started it again at a half-dose and sometimes I still find myself out of breath. What do you recommend?

It is incredibly unusual for side effects in such low doses, particularly when there have been no symptoms for such a long time. It may be useful to see a physician to exclude other causes for the breathlessness.

Hi Dr Diwa, I just come across your KZbin channel and I’m blown away by how insightful and pleasant your presentations are I just wanted to ask one question if taking I’m taking 5 mg of oral minoxidil daily should I split the dose in two due to the fact that it leaves the system fairly quickly do you suggest taking 2.5mg in the morning and 2.5mg in the evening or would it be better to just take one 5 mg tablet daily?

With regards to dosage, there is no specific guidance on taking oral Minoxidil once or twice a day. Most studies suggest once a day dosage and from a practical point of view, I would prefer once a day dosage as we are less likely to forget if we have to take a medication only once rather than twice a day!

Dear Doctor, I have been diagnosed with FPHL grade 2 and I have been on oral finasteride 2.5mg (for 1 year) and oral Minoxidil (started with 0.0625 for 2 weeks, 1.25 for 2 weeks and 2.5mg for 9.5 months). I have seen increased hair shedding for about 8 months, and it continues. (At the same time as I started full dose Minoxidil ie. 2.5mg, I got A cell PRP also done). Will the hair shedding EVER reduce or stop? Will increasing or reducing the dose of Minodixil help to reduce hair shedding? Is the hair shedding seen with Minodixil use proportional to the dose? It depends on the cause of alopecia. Minoxidil seems to work for alopecia of all causes. Knowing the cause is helpful as other treatments can also be used. It is best to consult a dermatologist for it.

The medication needs to be sued at a certain dose for at least 4 months before we can examine its efficacy. The current evidence is based on studies of Minoxidil at a dose of up to 5mg a day. For hair loss, higher doses have not been studied, though it can be used at a dose of 10 to 40mg for hypertension. It may be useful to consult a dermatologist as other medications like finasteride could be considered if Minoxidil alone has not given enough benefit.

Thats an interesting question! Just looked at the literature and I could not find any article that shows any interaction between the two in hair growth. They are used together as anti blood pressure agents so there is no reason to believe that they will counteract each others actions. Let me know if you find any research contrary to what I have mentioned.

Yes, thats certainly possible. Its hard to know what will happen after stopping finasteride as there may be hair loss again in some. Topical finasteride may be a useful compromise long term.

Hi, I am not sure where you live in the UK. Usually, there are other dermatologists who may be able to prescribe the medication in your city or town. It is always helpful to get treatment from someone whom you have seen face to face. This is because a full assessment may be prudent before prescribing the medication. Other scalp conditions should not be missed as minoxidil alone is unlike to help if there are associated skin conditions. I practice in the Nuffield in Chester and in the Spire in Wrexham, so you can contact these hospitals for an appointment. However, ideally a face to face appointment would be very helpful for both you and for myself to assess the scalp condition.

I've also been taking dutasteride and oral minoxidil for close to 9 months now and seeing great results myself. My hair is getting thicker and thicker. I actually have to undergo surgery, in which the doctor told me to stop taking dutasteride and oral minoxidil for 1 week before operation. If I do as the doctor instructed, will I lose the hair that I've grown right away and does it have to take a few months without taking those pills for that to happen?

It usually takes a few months for any hairs to fall, so if the surgeon had suggested stopping them, it would be best to follow his advice.

It is unusual to develop side effects at low doses especially after a few months. Sometimes, stopping treatment to observe if the side effects settle is one way of knowing if the medication caused the problem.

Yes, just as topical minoxidil can cause temporary hair loss initially, oral minoxidil can do the same. It stabilises as it is used for longer periods.

Minoxidil will help, but it needs to be used lifelong so it will need careful consideration in an 18 year old. It may have leas side effects compared to other Ora medications for this condition.

Usually, any hairs gained by oral minoxidil will be lost when it is stopped. It will help thicken those with normal hair but the added hairs may fall when oral minoxidil is stopped.

Just switched from topical minoxidil (after many years) to oral minoxidil (1.25mg) due to losing ground. I've also been on Finasteride for many years. Although my hair fall had increased, which is why I made this change, in the last 30 days since using oral Minoxidil, I'm noticing ALOT of hair fall. I was hoping it was a temporary shed but because it started prior to me using the product, I'm not sure. Any input is appreciated, thanks.

There is usually a period of temporary hair fall for ups to 90 days as the telogen or resting hairs are shed. One option may be to increase the dose of Minoxidil as men usually need about 2.5 to 5mg a day. Serial images of the hair parting will also help to be a bit more objective.

@@Zakkary Unfortunately the same. Still plenty of hair fall (hopefully just temporary shedding) but I do see some hair with bulbs, which is not typical. My derm stated that I have a combination of not only Androgenic Alopecia, but also Telogen Effluvium and Lichen Planopilaris so the typical remedies may not work for me as much as others who are only suffering from male pattern hair loss. I do start microneedling in 2 weeks so hoping that helps stimulate some growth. I haven't experienced any sides with the oral Minoxidil so thinking to up my dosage to 2.5mg is probably next step. I checked my bloodwork to ensure that my FIN/DUT combo isn't negatively affecting my testosterone and that has been confirmed (positive).

Prof Sinclair from Australia first reported the combination of oral minoxidil with spironolactone. It seems to be a very useful combination as one counteracts the other. I have discussed this in a recent talk which I enclose below. Spironolactone can harm the baby, so contraception is a must whilst on this treatment. Sir I have been using 5% minoxidil and finasteridine topical solution for six months besides taking finasteride orally for this period on alternate days..I hv seen good resilts... Niw my Dr has suggested me to go for minoxidil 2.5 once daily for 15 days followed by 2.5 twice daily for 15 days and then again shifting to 2.5 daily for rest of life... For men, we can start at 2.5mg a day (1 or 1.25mg for women). For long term conditions like hair thinning, it is better to start at these low doses and work our way up. Each dose will take at least 3 to 4 months to work, so can titre up the dose after 3 to 4 months, increasing uo to 5mg in men.

Dear Dr. I started with 2.5mg treatment and increased it to 5mg and developed a persistent mucus and burning on my throat and cough. My doctor said it would probably not be caused by Minoxidil but I am not convinced. I stopped using oral min and started asthma inhalers and started feeling better. Is coughing a known side effect on oral min?

I dont think oral minoxidil is more likely to produce hypertrichosis than topical. As long as we keep below the threshold dose of 4mg for males and 2mg for females, the chances of hypertrichosis is diminished.

The use of medications is usually each physicians choice and there is no right or wrong. Personally, I use oral minoxidil and tend to avoid oral finasteride in women. However, there are studies which show that finasteride has been successfully used in women too.

From my reading, I do not think there is a direct correlation between the topical and systemic doses. The dose will need to be titrated according to the effect on the hair. Increasing as mentioned in the video would be the best option.

Feeling tired is an unusual side effect for Minoxidil and I have not come across it before. I guess any side effect can occur with any medication. Usually the body tends to adjust to any medication after a few days so it may be worthwhile continuing the medication for a few more days. Another option is to reduce the dose and take it on alternate days rather than everyday and then gradually titrate the dose upwards?

@@mansooresfandieyar5860 no. But honestly I had to stop it after 1 week -10 days of using it. I was feeling so bad, tired, lazy, low energy, terrible puffy eyes, and even low sex drive. Once u stopped it, it took me 3-4 days to get back to normal and feel better. I strongly believe that oral minoxidil has a lot of side effects and that it’s not only vasodilators. I strongly believe it has a hormonal effect. I used the topical for years with no side effects at all except the hair dryness. Once I used the oral at very small dose, I felt so bad.

Initial hair loss can occur with both oral and topical minoxidil. It usually stabilises after 6 to 8 weeks so continuation of the treatment would be the best option. If there is no response within 3 to 6 months, it is unlikely to work.

Oral Minoxidil may be a better option if there are side effects with topical minoxidil. It can be used for the next 4 to 5 months and will then need to be stopped before trying for a family.. Sometimes changing there the topical minoxidil solution to minoxidil foam may also reduce the skin sensitivity and is worthwhile trying.

From the data we have till now, Oral minoxidil is more effective than topical minoxidil. So some of the hair gain can be maintained when oral is changed to topical minoxidil but there probably will be some hair loss, which tends to be different in each person.

@@ArisAris-ku7np Yes, it is a safe drug. The only factor ro consider is that it needs to be continued long term and due to your age, this may be many years yet!

Hello Dr, my daughters are 13 and 11 years old and they have female pattern baldness. Can oral minoxidil be given to them ? If not pls suggest some other treatment . Pls help. Thanks

It is very unusual to give oral minoxidil for such young children. It may be useful to assess the reasons for hair loss first. Usually nutritional factors are important in young girls. Here is a video that explains the causes of hair loss. kzbin.info/www/bejne/f4G7g6uCbNdnjcU

We are unable to find any reason Dr. Their hair is getting thinner day by day, giving all the supplements like iron, vitamin d, using hair serums but nothing is helping. Can you suggest some line of treatment at this age ? Thanks

@@user-wh5ro6xb9c A specific diagnosis will need to be made. Perhaps a skin biopsy could be considered by the dermatologists? Without knowing the cause it is difficult to treat the underlying condition.

Anyone above the age of 18 can take it. However, it needs to be taken long term, so that is something that will need to be considered.

Most guys don't mind more general body hair growth, I suspect. For women it's a worry.

That is quite a rare side effect and occurs in those with pre existing heart problems or those on high doses of the medication

Hey doctor, I am a male and have been taking 2.5mg since October and have been having awesome results. Also I sometimes use 3.75mg. But noticed recently that I’m getting tired on oral minoxidil and whenever I sleep and wake up, my head feels spinning for 5 seconds. Curious if this warrants a complete stoppage or should I cut down the dose to 1.25mg? Thanks

Tiredness is not a common side effect of the medication. the spinning sensation off the head may be postural hypotension or reduced blood pressure on getting up. Have you tried taking the medication in the evening rather than the morning? A slight reduction in dosage to 1.25 or 2.5mg may be helpful if Minoxidil is the cause of the symptoms.

@@ITZME11592 Usually most would have tried topical Minoxidil, so this is s step up and I ask them to wean off topical minoxidil once the oral minoxidil is established. Some may need oral finasteride too but this is decided on a case to case basis.

Thank you for your comment and request. As I have licensing privileges only in the UK, I am not allowed to treat those outside this country Im afraid. My medical indemnity covers only those in the UK. I would suggest that you get the medication from India or other countries where pharmacies will be willing to prescribe it for you. It can then be monitored by your own doctor in Russia as the side effects from this medication seem minimal and can be overseen by any doctor.
