Thank you for your kind words. The first step before any treatment is to get the diagnosis correct. For this, you will need to see a dermatologist. If they feel you have a condition that may benefit from oral tranexamic acid for pigmentation, then it is worthwhile starting. There are numerous treatments for reducing pigmentation of the skin of which tranexamic acid is one. If pigmentation is prominent in the flexures, another possible differential diagnosis is acanthosis nigricans for which met firming may be helpful. Thank you for your kind words. Firstly, the diagnosis of melasma needs to be confirmed by a dermatologist. There are other causes for pigmentation on the face. These include maturational hyperpigmentation and also secondary top hair dyes. Hello, good to hear from you! Tranexamic acid is an option for anyone with melasma irrespective of the colour of their skin. It needs to be used foe about 3 to 6 months. It should not be used during pregnancy. However, it has no effect on fertility long term. Would also suggest the use of tinted sun screens too prevent the pigmentation from worsening. Hi Doc, if I may, with reference to a comment here, I would like to ask which of these combinations would be the most effective to treat LPP for an Indian: 1. Tranexamic Acid 500mg + Roaccutane 60mg 2. Tranexamic Acid 500mg + Glutathione L 500mg 3. Tranexamic Acid 500mg + Roaccutane 60mg + Glutathione L 500mg The 2nd one was suggested by the dermatologist first. Hi, I do not treat many patients with dark skin as I work in the UK. From all the lectures I have attended and the personal expetricne in having treated a few patients, a combination of drugs may work better. The dose of Isotretinoin (Roaccutane) used is much lower though, about 10 to 30mg. I do not use high dose isotretinoin as it is likely to cause more side effects. Most dermatologists who are considered experts in treating LPP in dark skin suggest tranexaminc acid with isotretinoin. Glutathione is an optional extra

I asked about Finasteride as well, but was told that they will prescribe it only if Minoxidil doesn't work). Do you think it's right? I am tempted to use an online pharmacy and ask them to prescribe Finasteride as well as I doubt that Minoxidil will work on its own, since it doesn't reduce DHT.

We generally try and use oral Minoxidil on its own as it is fairly effective. Most dermatologists would feel uncomfortable prescribing finasteride in women, so I can understand why your doctor is hesitant to do so. One option to control the hormonal component may be to use spironolactone. We use it quite a bit for acne I women and it can be used in combination with minoxidil for hair loss too. There have been no studies on oral tranexamic acid to improve the evenness of skin tone. The pigmentary problems usually tend to settle only when oral tranexamic acid is used for many months. There are a couple of other options for skin pigmentation. Sir this medicine can i use for contact dermatitis. My doctor is doing continuously q switch laser weekly twice .is it safe to use q switch laser every week.thank you

If it is the post inflammatory stage of contact eczema it may be helpful. Certain lasers are beneficial, however the lasers could also cause pigmentation in those with dark skin, so it is a bit tricky. A test patch is usually done before treatment is used. I am not sure how frequently lasers need to be sued as I do not do laser treatment myself. @@winifredfernandes9611

Hello Doctor! I'm a 19 years old female who has been diagnosed with post inflammatory hyperpigmentation all around my lips in big patches. I've taken Zimax antibiotics for 3 months and used azelaic acid 20% for 2 months but still no improvement. Can I take oral tranexamic acid for this?

Tranexamic acid has been reported to be helpful in post inflammatory pigmentation, so may be beneficial in that setting. There are other topical agents that could be helpful too. It is difficult to predict melasma. To prevent it recurring, it may be worthwhile using tinted sun screens so that the facial skin does not darken. It can be continued throughout and does not need to be changed for the periods. The dose is different when it is used for continuous periods. For melasma it is used all the time. @@wajeehamalik4611
