how do you play that low D on the bass at the end?

It's optional for those who have C extensions. If you really want, you could tune your E string to a D, but you'd have to transpose all those notes. Otherwise just play a regular open D.

Josh is indeed correct, but keep in mind with the c extension, you Might have to turn the Kapo before the end, to hit the note. Just an fyi
