Wow, I'm currently waiting for an MRI for suspected subscap tear due to lift off test. This guy in the video is EXACTLY me, I stretched it out and the pain is basically gone. Today I started with light weights with no issues or pain. Thanks Doc, you saved me mentally as I didn't want another shoulder surgery. I could also preform a successful lift off within the first hour of stretching. Still weak, but I can do it pain free.

I have had chronic pain in my left upper arm, neck, and upper back for over four years. The only real mobility problem I have with my arm is doing the lift-off test. If I try to attempt it, I can lift my hand but I will get a deep spraining like pain in the upper back tricep into my shoulder. I've had my son assist me with the belly test and I definitely have weakness resisting him pulling my hand away. I have more strength with the bear hug test, but with enough effort my son can get my arm to give. No pain with those two tests, just the lift off test. I have the same pain if i do the lift off test with my right arm. I've had physical therapists test my mobility and strength as you are doing in the beginning of the video and i have no issues. Do you have any advice for what I am experiencing? If I had a tear or tendonitis wouldn't I get pain with overhead arm activities?

WHy do all these tests when and mr or ultra sound can give you the answere. All of this is speculative, that is my biggest concern with physical therapy. To opinion based.

What would a painful lift off test potentially indicate? Specifically pain deep in the anterior shoulder.

A subscapularis tear

@@OrthoEvalPal internal rotation excercises is good idea to solve this problem/tear/pain ? Something else? Maybe using ball on the back is good idea too, to loosen tight for example upper traps and places between scapula? Thx for help

Not relevant to this but doing some research- Subscapualris tear or trigger points can jeopardise and worsen symptoms of frozen shoulder.... Why is subscap often related to frozen shoulder; is it because the patient would avoid ER and abduction and stick to internal rotation??

Did the patient do a lot of overarm throwing?

Hey doc. I had a bankart repair (GLAD lesion) and after the surgery I test positive for the subscapularis (internal rotation/lift off with the arm behind the back), a sharp pain, like 9/10. I didn't had any problem with this before surgery. It's been 2 years now and no improvement. I also got a winged scapula after surgery. Is it possible that the surgery failed and the ancors are hitting the subscapularis? Or is it a mobility issue caused by the winged scapula?
