Hi ,one doubt ,do we need to verbalise all points in A-E assessment eg ., in breathing your breath is normal ,you are not using your accessory muscles etc., like that or simply take the vitals and mark it down and ask about ADLs question right?

Your briefing will be that questions have been already asked and scored, you need to add the score and address how will you support your patient regarding this.
