Hi Friends! In this video, I discuss Osteoarthritis - also called "wear and tear arthritis" - of the jaw. This is a chronic inflammatory process and is actually quite common. Osteoarthritis is NOT the same as Idiopathic Condylar Resorption (ICR). In future videos, I will be comparing ICR to Osteoarthritis. Please stay tuned for these upcoming videos and don't forget to SUBSCRIBE!
If you're unfamiliar with TMJ Disorders, common signs and symptoms include but are not limited to: headaches, neck pain, jaw pain, ear-related concerns, shoulder pain, specific tooth pain, jaw joint noises, and/or episodes of the jaw locking or catching.

I have OA in both joints. No pain. My pain is my facial muscles. My discs aren’t in place (one recaptured, one doesn’t, upon opening) but my lower jaw feels best if I jut it forwards a millimeter or so because that takes pressure off my condyles somehow. I love this video explaining all the facets of OA.

Hi I am so glad that I found you. I have seen many different specialists for many years and was never given a full explanation of my problem. I have learned more from your video than I have from anyone else. I have severe TMJ arthritis. I have had amniotic injections, steroid injections, PT, massage, acupuncture, laser heat therapy, mouth guards and Botox. I have had Botox done 6 times. The first two times it worked and lasted a year each time but the last 4 totally failed. They were no help at all. Do you have any idea why Botox would stop working? I am desperate for pain relief! Thank you

Vancouver WA is very far from NYC where I am. I haven't been able to find anyone who can help me with my destroyed degenerated TMJs. Is there anyone you know of who is at least half the doctor you are in NYC? I desperately need joint space. All I have is a splint which barely or doesn't distract my joint. Even when I go to the ER, they refuse to help me.

I have OA. The muscles inside my mouth are always red and rigid, and asymmetrical. Would an orthotic hypothetically/potentially help loosen up these constantly tight intraoral muscles? Massage barely does anything anymore, because I've degraded so much and am considering getting an orthotic to at least save one of my joints even the other is getting replaced.

There are no exercises for OA. Having said that, the pain is typically coming from the muscles so if we can calm the muscles down, you’ll feel better long term.

I tried a normal mouth guard but woken up fully clenched. I believe im in the beginning phases? Is the repositioning guard what you mean by having good orthotics? Also thank you, you decreased my stress level about this! You get overwhelmed by people saying theyve had pain for 10 years.

In terms of things to help slow my OA, I’ve tried #1, #3, #6. I’ve been looking for a place that does acupuncture and if that doesn’t help, I want to see if I can find a doctor who can inject steroids into the joints. (I did have several rounds of Botox but only on my masseters, pterygoids, and neck muscles - that won’t help my OA but the facial soreness I have all the time lessens with Botox)

Hello Dr. Priya Mistry, I want to ask you a question, and I hope you will answer and answer. A patient came to the hospital where I work suffering from TMJ ankylosis. The patient did radiotherapy in the neck as a result of having cancer in the lymph nodes in the neck without undergoing surgery more than a year ago. We did panorama rays, there is no Bone adhesion in the joint, but when I examined the muscles when I did the palpation test there is steffness in masseter and sternocleidomastoid ‏and muscle and other muscles of mastication and neck and muscles is very hard in palpation My question is is it useful to do sessions with the J5 TENS device For such a case and how many sessions the patient needs, I hope you will respond to me because the patient is suffering severely because he is unable to eat. He can open his mouth less than one finger.

Dear Dr. Priya, Thank you for these great videos. I am wearing an orthotic to keep my jaw in the right position as a therapy for TMJ. I have always suffered from Teeth clenching/grinding, but since I have the orthotic this problem has increased drammatically: I wake up many times at night noticing that I bite the splint so hard like a dog biting a bone. As a consequence, the jaw and temporal muscles ache even more than before I started the therapy. Is there an orthotic which could cure both malocclusion and bruxism? If not, do I need to live with bruxism making my TMJ orthotic therapy less effective? Thank you

Hi Dr. Mistry, would OA lead to inflammation around the jawline, specifically in front of the ear? I dont have any pain but I always wake up with swelling/inflammation around this area. Do you have any reasons I continue to get this inflammation? Is it just due to overwork of the masseter? As I do see this happen when I eat hard food as well. thanks!

OA alone won't lead to this. Inflammation is typically caused by muscle dysfunction.

I have experienced almost 2 yrs of TMD with no medical/dental relief. No one in my local area treats this issue. Why don't more MD's or Dentists specialize in TMD? OA is a medical issues. I believe TMD is then a medical problem.....not covered by insurance?
@theTMJdoc

There’s really no great training in medical or dental school for TmD. The one speciality program that’s recognized focuses more on stress relief and medications when I’ve found that TMD is often more structural. It’s definitely a medical issue - bones, joints, posture, alignment, etc. And I don’t see treatment covered by medical insurance often though in my opinion, it should be.

Hi dr , how r u Could extracting the lower molar tooth potentially worsen the issue of disc displacement in the jaw joint and jaw clicking? Because I have an old silver filling that broke, I went to the dentist and they placed a composite filling for me, but the pain still persists even after the filling. Additionally, the tooth is severely cracked and fractured, and I can taste metal in the tooth after it broke. I am very afraid that my jaw will be further affected after extracting my tooth.

Hi doctor. Is it normal for you to have cases where when you finish treatment with patients and while most symptoms are gone, clicking/popping sounds while eating/talking continue? Or still some slight resistance when opening wide? Will this possibly resolve as they continue wearing the orthotic even though treatment with you is over, or is it just something to deal with that won't go away?
