This is on the UNI-ASSIST application form, not in the visa process. So my question is about the certificate, should I just translate a standard certificate or should I get a template from the embassy and have it certified by a doctor?

Thanks so much Promzy. I've applied for university of Gottingen but not through uni assist. An email was sent to me that, the admissions or rejection letter will be ready by mid or ending of July. Which month will be appropriate to apply for a visa if I gain admission. Thanks
