How I do it. Hey there everyone here’s another quick protocol video, as per the usual depending on your institution your image quantity and anatomical requirements may differ. But this is a more than basic protocol that you may be able to tailor to any place.

I work as an ultrasound trainee in an Australian public hospital and augmentation is included in our current scan protocol for deep vein thrombosis. We recently hired a dedicated vascular sonographer who doesn't perform augmentation so I have been looking into the research on this. I found a 2009 peer reviewed study performed by McQueen, Elliot and Keir that states that 'single-point augmentation has a low sensitivity in suspected lower-limb DVT, and that the majority of undetected DVTs are isolated to the calf veins'. The researchers found that an abnormal augmentation response was a poor predictor of lower-limb DVT given that diminished or reduced augmentation can be attributed to alternative diagnoses. They concluded that 'single-point augmentation does not add to the standard compression ultrasound examination for suspected DVT and should not be routinely performed'. I would love to delve further into this as my patients often find augmentation an uncomfortable part of the procedure. I would hate to be performing it unnecessarily.

so if a thrombus is found, do you completely avoid augmenting or you just skip augmentation at the location of the thrombus?

I augment at VERY end due to possibility of thrombus being distally ...IF thrombus seen ONLY spectral waveforms

So you always augment on your way down? What happens if you find a thrombus, do you still augment? I always do my compression first and then come back with color and pw Doppler. If I find thrombus anywhere, no augmentation done, I just demonstrate flow. I know the calf pump is more powerful than your hand augmentation but I still worry about dislodging a thrombus. lol
