I had a run-in with a crispy chicken sandwhich at Jack in the Box about a decade ago that was legitimately still frozen in the center. The thought alone that I might have ingested raw chicken made me sick. I'm also about 90% sure it was retaliation via food tampering because they didn't want me using more than one coupon. Being the only customer in the drive thru, I tried just going through the drive thru again, and they tried to tell me I couldn't. I got out of my car, and they finally took my coupon, but not without me questioning them as to why ut was any different taking an order on counter vs a brand new order in the drive thru.
