Thanks for the video. I have a similar CA issue when sleeping on my back, which doesn't occur when sleeping on my side at low pressure (6.4 APAP). I would sleep on my side only, except after about 4 hours I get pain in my shoulder and hip, which wakes me up. To avoid the pain, I need to sleep on my back, which then brings on clusters of CA, which leads to sleep apnea badness. I would like to see a follow-up video to see if you're able to correct this person's CA problem. I'd also like to learn how an ASV machine could be used to correct this type of CA problem. Thanks.

Thanks! Overall central apneas aren't nearly as common as obstructive apneas. For those that truly suffer from CSA they are a big problem though.

They have shipped my sleep@home kit. I was told they aren't as accurate as a in stay test. I had one of the machines that was recalled 😬 I used the nose pillows I did check into that inspire but I didn't have enough episodes. Thanks for all you put up for us man.

ASV requires higher pressures, which may cause aerophagia.

I think this is exactly my problem. Using the cpap on apap setting is causing me to have 25-40 ahi with 18-25 centrals per 5 obstructed. I also do have heart issues, I believe developed from untreated sleep apnea. I now have episodes of afib infrequently and experiencing arrhythmias daily.

Central apnea is a neurologic condition. It's often genetic and runs in families. It's a silent killer. And it is often misdiagnosed as SIDS in infants. CPAPS most often fail for neurologic Central apnea patients. They most often require pacemakers. CPAPs fail to stop blowing when there is no obstruction to signal the machine to rest cause the water to evaporate and blow dry air continuously. When they use CPAPs, the CPAP blows in and back out through their nose and throat, drying their throat as it did in my case when my throat became extremely dry and when my alarm went off for work I woke up and swallowed and my dry sticky throat glued shut. As I was getting near blacking out I thought to myself my kids are going to find me dead and not know why. Just as my muscles began to spasm in my body from lack of air, and I kept trying to massage my throat open it finally did but even though I could get a little air it still felt like it was too late and I fainted and woke up late for work so very thankful to still be alive!! When I told the technician on my next sleep study that I would refuse to use a CPAP because I had true Neurologic Central apnea and how the CPAP had failed and almost killed me she passively took great personal offense and without telling me or anyone, took it upon herself to throw parts of my study tapes in the trash. When I told the front desk when checking out that she shortened my study to 3 hours instead of the scheduled 6 they asked for the study tapes and she denied everything but they found some computer evidence missing and thankfully checked the trash and caught her in her lies. Waste of insurance money and sorely needed results. One neurologist told me no study because there's no treatment or cure for Central Apnea and sent me out. After my first study I was told they rewrote it as Obstructed because my insurance wouldn't cover Central and prescribed me the only CPAP I would ever use. Other neurologists were friendly about tests until it came back Central and their whole attitude changed. Some refuse to test me to find out how its progressing AS IT HAS AND IS, other nurologist said the medication for Central Apnea has too severe side effects and didn't want to go that route saying I was better off without treatment in view of the pros and cons, Rose Medical kept reositioning me trying to construct it as obstructed and ultimately misdiagnosed it as mixed when I coughed ONCE. Since then, neurologists say I need things like VPAP but that I have to be retested and prescribed a CPAP and use it and if it fails... which I won't ever risk again, then the insurance will be willing to move forward to VPAP/other. My great grandmother died at 42 in her sleep from Genetic Neurologic Central Apnea, her daughter, my dad's mother died in her sleep a few times but her insomniatic husband kept noticing and resuscitating her until he finally had to call an ambulance and only because he was like a Mack truck personality and the military man he insisted that the doctors go further, m so they gave her a pacemaker which saved her life for the rest of her life and she died at a ripe old age. I'm now in our fourth documented generation (my kids and granddaughter are now our 5th and 6th documented generation), and even though I was initially diagnosed with Central Neuologic Apnea I'm still having difficulty with doctors just being willing to give me a sleep study to find out where I'm at since the last time I was studied and was stopping breathing hundreds of times a minute. I also need a pacemaker. It takes hours for me to regain oxygen in the morning enough to be able to be mobile. I've asked about receiving oxygen and I was told that oxygen would kill me faster because the off-put of oxygen is carbon dioxide which is already too high in my body. I know I'm slowly dying. And ones who are supposed to be my helpers my rescuers the doctors are my killers. All I need is a pacemaker and a Real doctor. I think it's a shameful conflict of interest for any doctor to be able to benefit financially from the prescriptions he writes for CPAPs. Shame shame shame on those doctors!!!!!!!

I could have written this myself. It's my story. Except for generational Central Apnea. No one understands. Not doctors, nor family. It's killing me, just not exactly slowly. Cognitive issues setting in. I'm 59 and unable to do much of anything anymore. Would be nice to have someone to talk to about it.

@@stargazer1359 I'm sorry you're going through this. I too suffer cognitive issues now, very close to your age. My hands and feet are cold, bluish and burning/asleep when I wake up. Thanks for sharing about genetic neurologic central apnea. I started on APAP 9 months ago, and migrated to BIpap 3 months ago. My OSCAR data shows a lot of central apnea and barely any OA. Your experience brought back a memory of the story I was told about my grandfather dying in his sleep. I don't know how old he was but he was still working full time. It seemed like a mysterious death. Now I'm wondering if he died during a central apnea episode.

So what's the deal with transitional central apneas when falling asleep and waking up? I get that in a crazy way and my O2 can fall to upper 70s if I'm not wearing a CPAP. Especially when lying on my back. It's almost impossible to get out of bed when this happens; it can be an hour or so before I actually hear my alarm. Additionally, I get 10-20 breathing related arousals (alpha intrusions) per hour while sleeping, followed by a RERA, hypopnea or even central apnea. That much I know after 5 lab studies and I still feel like they haven't got to the root of it yet. I swear, I'll get my Oscar data and past reports to you soon

I'm really curious if some of these artifacts that we see in the flow rate might not be reflections of sleep stage, possibly N3 or N4.

Are you familiar with the correlation between diagnosed Post Traumatic Stress Disorder (PTSD) and central apnic events? I was in a small 2013-14 study at the Department of Veterans Affairs (aka VA) that found there is a correlation between Combat Veterans with PTSD and sleep apnea. Wonder if your patient in Utah fits that criteria? Thank you for your channel.

I do know if the PTSD and OSA, but not PTSD and Central Apneas. He doesn't fit that criteria....however, a friend of mine does. Working on something with him right now to share.

I have done several consultations where people did have CSR and were on CPAP. Just didn't record them at the time. Switched to ASV and everything changed for the better.

Thnx. My sleep doc's aware of my CSR but doesn't feel it's bad enough yet to justify moving to ASV (couldn't find one on line anyway - apparently very hard to get), but continues to monitor me.

My Events are all over the place this month...I'm not doing anything different, but I'm getting some high numbers then back to low numbers....I'm getting some 10-12 events per hour nights then goes back to 2.9-3 events.... I don't get it. I need help. I just had to start with a new Doctor my long time Doctor left the medical group and disappeared so I'm in no mans land. :( Need help.

Great video Jason. I just ordered some filters and got 20% off using LEFTY20. Got rid of my rebuilt respironics 1 and have an Airsense 11 now. My AHI is almost nonexistent. My flow rate has looked perfect. My therapy is dialed in thanks to all that I've learned from this channel. Thanks for all you do. Oscar is my friend. My polysomographer was speechless when I called him and asked him to look at my data.

The end on my resmed 9 can bump my apena's just when taking off the mask. So yeah, don't just blindly follow simple numbers. Also decreasing the max pressure fixed my issue. 95% of my nights are well under 2.

I do that every two minutes all night long..cpap doesn’t help
