The recapitulation is mandatory by liturgic use, it's not written.

Yes. This is indicated by the "Organum Quadruplūm", which signifies that 3 voices were added to an initial Monophonic chant.

...skilled performance - the only historical performance note I'd add is that the polyphony would have been performed by soloists, while the chant sections would have been done by the whole choir...

...there are lots of studies on the performance of organum, but a good place to start (in my totally biased opinion) is Penelope Turner's discussion here: www.musicandpractice.org/volume-1/notre-dame-organum-duplum-modern-edition/ She makes reference to the major studies that are most available, and compares various editions as well...

...it also is sort of common sense, in that the choir would have been a mix of people who could read the newly devised notation and those who could not - those who could would have most likely been the most accomplished singers, while the standard chant would have been something nearly everyone involved would have grown up with - just my $0.02...
