As an ICU RN, I love when certain pharmacists are on, because I know my patients will be getting the absolute best. If I’m ever in doubt (about anything really but especially electrolytes lol), I usually consult with pharm first before asking the provider. The MD/APRN/PA will usually just say defer to day shift (obviously NOT ideal - patients deserve the best at every hour of the day). Gotta love pharmacy ❤

So I did a year of nursing school in Australia (2012) where Electronic Medical Record hadn't caught on yet. Alllll paper charting. NOTHING warmed the heart like seeing pharmacy arrive to the floor, go through all the charts, and re-write all of the med orders in the patient charts in their purple pens. 1) You could suddenly read everything. 2) Suddenly everything was logical. We must protect pharmacy at all costs!

Ive had to wait until said pharmacist was off the clock to fill medications before. Now i stick with the same pharmacist. She knows me by name and knows what i am ok with and what im not ok with. I understand theres medications you just can't mix and medications that have a better option but this wasn't the case here. It was clearly personal judgement and i dont think they work there anymore. @@NajYlib-mc5fo noooo lol we don’t diagnose complex illnesses. I don’t mind letting the MDs have that scope. But also yes some pharmacist have specialties like diabetes, anticoagulants, HIV, etc etc where’s they manage their own patients *after* diagnosis. But our job is to make sure the medication is as close to a benefit for each individual patient as possible and sometimes drug regimes become complex when patients are complex so MDs need advice or a hand managing. We also are double checkers to make sure nothing is missed like a few weeks ago I had a patient who the ER doctor prescribed antibiotic called ciprofloxacin to a person on warfarin an anticoagulant . The antibiotic will cause the blood to become too thin when people take warfarin at the same time which can lead to stroke. Patient was smart and didn’t start the meds until after he spoke with us his “personal” pharmacist because he knew there was an interaction but didn’t know what to do. What we did was decrease the warfarin dose for the duration of his antibiotics treatment and checked INR (a special blood test) to make sure he was in goal. It sucks that ER doctor missed it and not all hospitals have dedicated ER pharmacists. But luckily this patient had a pharmacist who specializes in his condition who is often times easier to reach than their primary doctor. Luckily more pharmacy students are doing into speciality practice.

I live with a pharm student, and I realised pretty quickly how important it is that they're a separate job. There is so much he's gotta learn on how different things can interact, how you can optimise things, how to adjust meds to account for side effects or allergies and that can almost start the entire concoction over again in some cases. it's not reasonable to expect a doctor or nurse to learn all that, so having the mental load offloaded onto someone else can definitely cause some frustration when they point out why a plan won't work, but pointing out that there's a problem, or that it could be done better, is exactly why you do it. Also since both will have an understanding of the broader aspects, there's extra people to catch any mistakes, because humans make those
