In recent years, the discovery of cancer stem cells has revolutionized our understanding of this disease. It has long been known there is incredible heterogeneity in a tumor, and now it seems these dangerous cells that often resemble pluripotent stem cells sit at the top of the hierarchy. It now seems apparent this small subpopulation of cells is crucial to the self proliferation, self renewal, refraction to treatment, and most importantly metastasis that characterize cancer as a disease. Some studies have shown that 1 in 400,000 non-stem cancer cells may undergo EMT and tumorigeneses (metastasis), compared to 1 in 7 of these cancer stem cells. As stem cells, they are incredibly robust cells that are difficult to kill, since their survival is crucial to the survival of our species and are forcing us to rethink our understanding and treatment of cancer. It was shown in one study that this small population of cells are nearly immune to chemotherapeutic treatment and highly resistant to ionizing radiation. As the scientists applied chemotherapeutic treatment to tumors in vivo, the proportion of cancer stem cells that was originally about 2% of the cells in the tumor became over 20%, suggesting that the treatment was killing the "bulk" tumor cells that are incapable of metastasis and selecting for the survival of these far more dangerous cells. They are immunologically privileged, employ drug efflux pumps to remove chemotherapeutic agents, and have constituitively active DNA repair mechanisms (contrary to the believe repair mechanisms fail in all cancer cells) that protect from radiation induced damage. We measure drug efficacy in oncology almost solely upon the principle of tumor remission, or the shrinking of a tumor in size after treatment. Again, evidence is mounting that suggests we are selecting for the survival of these cells and initial remissions represent the death of cancer cells that are not stem cells and therefore largely incapable of metastasis. To all the future physicians, researchers, and oncologists out there reading this, you must be aware of these robust and dangerous cells, and I challenge you to find new ways to specifically target and destroy them.

Before I ask anything, your videos have helped me a lot with my essay about chemotherapy, so thank you for that! However, one youtube video isn't much of a source and I find it hard to find good scientific sources who go by the basics before getting specific. I see the link in the descriptions, but do you perhaps have a list with papers/chapters you've used?

Gene therapy is the way to go, but we're so far away still. Using these kinds of highly toxic compounds are really not the best way to treat, but it's all we've got for now.

I was wondering If a cell is replicating, does it (the cell) do its other jobs like producing proteins and hormones and receptors. Or does it just stop in all the fields ??? thank you very much for this video Armando

I think the cell pauses all of it's "normal" functions during mitosis. Interphase is the period where a cell continues with acquisition of nutrients, metabolism etc. The majority of a cell's life is spent in interphase.
