IF you can afford a tWIN JET, and all that goes along with it, it’s a VERY cheap thing to pay second pilot - preferably a type rated instructor - to be a cheap insurance plan for at least your loved ones. I bet Kobe would agree.

@@NateFanning lack of basic training to standard. Lack of recurrent training. Near total dependence on automated-assist flight. Single or multi crew, we ALWAYS hand flew till hand-intercept outbound tk. FMS/A-P Not engaged till 1. 1,000 agl 2. outbound track established on most appropriate NavAid.

As Juan Browne says, the private pilot's most important safety device is the credit card. Book a hotel room, or two, or three, for the night. You own a Cessna Citation, I'm sure you can afford it!

Sleep studies demonstrate that losing 2 hours of sleep is equivalent in impairment to being at .08 alcohol. Losing more sleep leads to greater impairment. This is why so many extreme incidents occur in the wee hours of the night shift. Humans with sleep deprivation often can't make good decisions

Fatigue can make you do the most stupid things. I recently did a double block flightlesson and started noticing this around the end of the flight. Reporting a wrong waypoint name, not flying the numbers, and the most dangerous one, trusting muscle memory, almost pulling the fuel valve instead of pulling the carb heat. Real eye opener.

@jingle1161 pulling carb ht: THE MOST basic rule of operation: 1. Think of control & action 2. Sight identity 3. Sight confirmation whilst initiating action 4. Confirm expected result/re-confirm control actioned. 5. Followup continued expectation. Here in Oz, we had indigenous trainer I flew/engineered on as we were national agents. THREE identical knobs in row. One was mixture, carb ht, cabin ht. MANY crashes due wrong knob - nil confirmation. Ditto fuel selector DESPITE ergonomic design - many bad crashes/fatals: fuel off enough to take you to 100-200 ft. Wing loading about as bad as American Yankee of Bede. Training basics: you really are at mercy of your primary flight instructor. I had two kinds: superlative ex-WW2/ ex- service, crop sprayer on the one hand. And Appallingly bad on other: not able to demonstrate accurately, unable to teach. Young "newbie-wet-flight-ticket: the worst with worst ego.

One of the very few videos I have seen where I can agree with all that was said. The NTSB investigation came to the correct conclusion. Perhaps the pilot and his passengers should have watched all of the game, and stayed overnight.

I have worked most of my life on man machine interfaces and am shocked at how bad the instrument panel design is on these aircraft. The consequence was deadly in this case. Anyone who thinks that identical rows of labelled buttons are a good idea should go far, and never return.

All the abbreviations as well. You not only have to know what they mean, but what they do. There is also very little consistency between aircraft - at least on a car the dashboard symbols are usually identical, things like the legends for the windscreen wipers and lights are intuitive and in an obvious place. Aircraft, no.

I don't know why black boxes aren't just straight-up FAA legal requirements for anything bigger than single engine bathtubs used for training, or indeed anything being flown commercially. Something like a Cessna jet absolutely should have them as standard.

Great analysis of the human factor components that may have been involved. Many I feel disregard the multitude of things that can possibly contribute to the eventual undesired situation. Probably because there are so many but in due time you can actually flip a coin and get tails 10 times in a row eventually statistically. That was a great analysis on the investigation side to note the possible autopilot miss use and the factors behind that with the different placement of the AP button on different aircraft and what happens when you XFR instead and how understandable that can be possible. 17 hours awake is generally referred to as bed time even if you don’t look tired, you are. Combine several small mistakes and with the sound of his takeoff clearance read back I would say fatigue played a huge role as was stated in the final report. We all start to make mistakes when we get tired. I am lucky and have the benefit to have 2 other pilots up there on takeoff and landing these days. We all keep each other in check.

And what a tragedy---documented to be competent in flight skills....but unfavorable conditions/ get-there-itis, mixed with fatigue, causing confusion and disorientation, when there's no room for error, proved too much. Terrible tragedy. Thanks for excellent review. Lesson: no such thing as 'night VFR' .. and add unfavorauble weather conditions ? This should have been two-pilot IFR. Full weather briefing and cockpit procedures review by both pilots. How sad !

I've just recently taken my first PPL lesson. My instructor had me close my eyes so I could understand first hand just how disorienting not being able to see the ground can be. he banked left, then leveled out, and it felt like we were still banked left. Eventually, I did roughly get more accurate, but not anything that I would ever rely on. Your inner ear and sense of balance is nearly worthless in the cockpit. On top of that, people tend to think that experience gives them the ability to skip some corners. On the ground, that may be. In the air, more and more I'm seeing "experienced pilots" cut corners and end up killing everyone onboard

"instant IFR": OZ-trained IFR driver default attitude drilled in from Day #1: from point of V2...U R "on guages" - REGARDLESS of ANY EXTERNAL visibility. Lots of (LOTS !) of my turbine/Jet IFR was black-black external condition. Even with PFD fail post departure early climb, single pilot opn, no drama. NOTING: as SP pressurised turbine flight, only average 650 (actual) air time PA. Hand-flew ILS to minina at least every 2nd sortie.

@@davidmotter5140 certainly also a possibility. Particularly an uneven flap retraction would be a possibility. Still the flight paths, conditions and lack of maday are similar.

The Citation CJ4 is one of the default aircraft in MSFS 2020 and it's an absolute horrible thing to fly. It just does what it wants, it's very unstable at low altitude and to top it all there's no autothrottle. The flight displays are an assault on your senses. There's some setting called "FLC" which isn't an autothrottle but sounds like it should be. I'm afraid this sort of accident comes as no surprise.

No matter your intelligence levels or experience, never underestimate the frailty of the human body. The unfortunate occurence might have been avoided, if the pilot's situation awareness was heightened as it should always be, as flying groggy is flying impaired. A heightened awareness of the early cues might have led to instant corrections and probably saved all the lives onboard as well as the aircraft.

I just don’t understand rich people sometimes. A basketball game? This requires a personal jet flight in atrocious weather as if it isn’t already dangerous enough? You think to yourself, well he must have good business judgement to make that sort of bank but exercises very poor judgement in other areas of his life? That’s bad enough when it’s just your life on the line, but to take out those passengers who trusted your skill and judgement? That’s another matter.

Keep in mind that passengers trust the pilot to make the right decision to fly safely. The passengers likely had no idea what was going on. It doesn't really matter why they flew to their destination in the first place.

Why if their Destination is South west & their Departing RW 24R are they assigned an Initial heading of 330 Nowth West?

Traffic from BKL would be routed away from the big Cleveland airport, heading it out away from the class B airspace. So they would probably be sent first north, then east, before turning southwest.

Diligent doesn't necessarily mean intellectual surplus. While avionics is still garbage and could be much clearer, it's not impressive to go into such a primal mode as to completely ignore the digital artificial horizon in zero visibility conditions. Or perhaps in this case completely blindly assume that the autopilot knows what it's doing if he pressed the wrong button to start it without any kind of verification. One could further argue that an "autopilot transfer" is bad design especially when placed where AP is on other models. It's not like one side of the plane can be on autopilot while the other side is manually flown.

And what about him BEING MORTAL as everyone?? Without a body to examine after the crash nobody can be sure that he wasn't experiencing early symptoms of a deadly stroke, or a sudden heart attack wich could explain the minor mistakes made before the crash. People die suddenly anywhere, in any circumstances. Death just doesn't care.

No mystery. VFR flying at night in bad weather is a sure method to die. As for the so-called experience of the pilot based on the number of hours flown, it's a sad joke. I doubt he had ANY flight at night in bad conditions. Nevertheless, having modern instruments for piloting and navigation in the cockpit, yet still crashing without any mechanical malfunction, is a disgrace. Rookies make mistakes, especially when they are used to flying by autopilot. That is not flying, that is laziness.

That's it! Too much bullshit. Standardize, standardize, standardize! Stop finding new ways to arrange controls. I drove my wife's car yesterday. It took me 3 hrs to figure out how get the Calypso music off the radio. This is bullshit. What if you fly different planes? Who's in charge here? Hey! While we're talking about stupid; someone pointed out that the rudder pedals are...what's the word?...he meant backwards. I can't think of the work right now. Remember the wooden carts with the stick and rope for steering? It's before my time but I know what it is. You have a stick across the front with the front wheels attached. You push on the right side to cause the stick to face more left. No toe in on turns or any of that. Center pivot (probably a nail) and off you go. In a plane, if you push on the right rudder pedal, the plane will not go left. It will go right. So every kid with a wooden cart has to unlearn his reflexive instinct. I was the one who ran all over the grass next ot the taxiway at Republic Aviation in the '60s. I had a wooden cart. We switched the cables in the crop duster so we could make sense out of the steering going around trees and powerline towers. What idiot set the pedals up like that??? (notice: everything is bullshit)

Flying over the Great Lakes on a moonless night as an amateur pilot is beyond dangerous, proving that money doesn't mean you're smart. You've got enough money to buy a jet but not enough intellect to know your credit card is your best friend on certain nights. And imagine simply hiring a second pilot for a night flight as well, all cheaper than death. Unnecessary waste!!
