You have such a clear way of explaining as well as showing exact diagrams of how to identify the type of pain, causes and possible treatment. I came across this video as I had previously subscribed when searching for sciatica causes/treatment. Up till now I thought my pain was caused by a problem with a disc in the L5 region but having watched this video, the pain in the buttock when sat down is exactly as described. it then shoots down the leg. At its worse, I felt pain in my left calf muscle but now I mostly feel pain on the sole of my left foot. I've also watched other videos from Dr. Ebraheim to better understand the causes of pain. I am not a professional but like to learn about what goes on with my body so that when seeking help I can adequately express my concerns but also respectfully challenge any assumptions professionals may make. I’m a little confused at 8:40. He says stretching the piriformis will reproduce symptoms by compressing the nerve. But aren’t you supposed to stretch the piriformis to reduce symptoms? Please clarify if you understand it differently - thanks!

@@madams989 stretching can help temporarily release tension, but it usually affects a weak periformis muscle. There's a test to know if you have a LONG VS SHORT periformis syndrome. After determining that, you either do specific stretches or a specific STRENGTHENING exercises to target the periformis. In most cases, the thing that actually works in the long term is to tighten the muscle, not stretch it as if it's already stretched, doing more stretching gives only a very short temporary relief then pain comes back. The best treatment for the periformis muscle is motion. Only one or few motions / exercises are enough to get instant relief, but they should be very specific and targeted. Also testing when having extreme pain is recommended to find the right ones ( glute bridges, side-lying clamshell, hip extension on all 4, rehabilitation bands, yoga hip openers exercises, hip thrust... ) Overall it's a game of flexibility & strengthening weak muscles. Hope this helps, it took me months to understand this and get over my 6 years worth of pain.

Thank you very much Dr. Ebraheim. This information is very helpful. I have long suspected that I am suffering from piriformis syndrome which continually aggravates my sciatic nerve. Stretching and staying mobile helps immensely. But when it does get aggravated, I usually can reliever it with ice and NSAIDS.

Try finding a goo pain management clinic or dr they will tell you right away. I get steroids shots injections every 6 months for it after a MRI, Some peoples get injections only once n get better pain free but not all. Talk to your regular dr and request to a pain management clinic. Hope it help a bit

I do not understand to the atomic level like my teacher but to the thousands and thousands of people I've seen, we resolved them all, with full participation of the patient which means daily practice of 3 x a day. I am not a guru, but I can take care of these sciatic problems, migraine, back aches, heartburn, cysts ...which most of the population suffers and sadly led to believe that only medication and surgery can solve. But the surgery can solve for how long? I haven't met a single person who had knee or back surgery that do not suffer later. With us it doesn't come back. But you need to do qi gong ( not yoga) and breathing exercises and stop Mc Donald's. For us anything that reoccurs in 9 months, 5 years, 15 years is not a cure. You either cure or don't. But of course we need maintenance everyday. We cure with 'chi', the invisible that the West can't see and can't believe. Our biggest guru was even successful with Parkinson's when the brain is already affected. Yes, we had 60 cases of success when he visited the US. We cure deaf mutes. The first response is to be able to count out loud 1 to 10, and then therapy follows. This sounds unbelievable to you. But that's the world we live in, with not a drop of medecine and old people don't have joint pain, stomach issues. We know how to prevent or heal with acupressure and the chi. If the patient has had only one surgery, we still can help. It also depends on the diet, living habits of the person. If we take medication at inception and cannot cure and continue with the wrong steps, of course the final choice is to suffer from surgery pain. Hopefully this can help someone. Please try it! If you do a video, please coin it the 'TT piriformis stretch' :) The stretches that give me instant and most relief, and can be done in bed: 1a. Lay with knees pointed up and feet close to butt about shoulder width apart. 1b. On the side of pain, rotate the knee inward so that the bottom of the foot is parallel with the body. 1c. Put the other leg, foot, or ankle over knee of the side of pain to hold it down for the stretch. It'll feel great and will stretch not just the piriformis but the entire butt and hip and lower back. 2a. Lay with knees pointed up and feet close to butt about shoulder width apart. 2b. On the side of pain, drop the knee so that it points forward and the foot is next to the hip. 2c. Take your hand and push upward on the foot. It'll feel great and will stretch not just the piriformis but the entire butt and hip and lower back. I've experience sciatic pain from my butt down to my toes possibly due to excessive sitting at my work and hill hiking. The figure 4 stretch sometimes work, but not always. My butt will spasm and tighten up throughout the day. What I became aware of is I sit crossed legs in a butterfly manner a lot, to take pressure off the pain, but it was making it worse. The stretches above puts the muscle in the opposite direction and the next day I was pain free.

Can you point to some videos? I tried to workout the two stretches. The exercise (2) seems to be - lying on the back - bring the feet of the affected side close to the hip, folding the leg so the knee points upward - bring the knee of the folded leg close to the chest, still lying flat. Does that sound right? I did not quite get what was it that is in exercise (1).

@@Abundantgrace-1 Hi Muthu, sorry I don't have any videos. I sort of figured this out experimenting with different positions that worked for me. Everyone is different. For excecise 2: Everything is right except for step 3. When the knee is bent, keeping the heel close to your hip or glute, rotate the knee inward (to the right if it's your left knee, to the left if it's your right knee). You can imagine for illustration purposes that the muscle is like a band from the side of the hip to back of your tailbone, and that movement will stretch it. For excerise 1: It's like a standard quad stretch, but instead of the foot touching the back of your glute, it's to your side hip, at a right angle instead of parallel. The knee of the affected side will point towards (left or right to) the other knee, which is pointing forward (if standing) or up (if laying down). The popular figure 4 stretch, stretches vertically, and the ones I describe stretches it horizontally, in an adductor manner. Progress slowly if you are still stiff in that area.
