I have a videographer recording the procedures, then an editor editing the recorded videos, then I narrate over the edited videos on Friday and Saturday mornings. The process is much more involved and expensive than I originally thought it would be! Several videos camera, still cameras, computers, editing equipment and a separate film studio. interesting doing the osteotomy though the soft tissue. it makes sense. I'd imagine it probably leads to better results. Big flaps would cause reduced blood supply to cortical bone and possible supercrestal bone loss.

@anon511 not really. Doing an envelope flap is sometimes best. A soft tissue punch is more convenient. Its a case by case bases. Advantages of envelope flap is that you can level the bone before osteotomy, seeing the actual bone will teach you more about bone and jaw types. You can better access surgical depth. Place that implant subcrestal. Sometimes you might need to bury the implant with a little bone on top of the healing abutment. Using primary closure aids healing especially of you are doing PRF/PRP. Tissue punches are convenient and cleaner. If you have a set that has a drill stop that makes life easier too. Stoppers tend to move and wear out. I hope this helps. Both techniques are useful.

Plus, the healing abutment placed through the soft tissue develops an ideal emergence profile for the final abutment and crown while you are waiting 3 months for the implant to osseointegrate before taking the abutment/crown impression. Another plus is you do not have to reanesthetize the patient in 3 months and punch a "core" through the soft tissue to take the impression. I do not anesthetize the patient at all when I take the impression for the implant abutment and crown in 3 months, plus the "long" healing cap has developed an ideal emergence profile through the soft tissue during the 3 months of implant osseointegration. For many years I reflected flaps for implant placement, but have rarely done it the past 10+ years. Take your practice to the Top Tier. Very nice work .. I'm afraid these techniques don't always end up with successful results, There is absolutely nothing wrong with open flap procedure, you can visualise the bone directly, as the implants coronal or the collar part will be inclined lingually, following the S shape lingual aspect of the body of mandibular, going perpendicular, this can only achieved with CBCT safely, and more safe with surgical guide, quite taking a risk flapless free hand there... I love free hand but with open flap, and feel the bone with both sides... Great job..
