I can see that in any question if there is an answer option that contains words assess or analyse it will be 99% correct answer, in almost all areas which is a good tip.

Question 21 @ Minute 31. Why is the answer not “D”. Its says to update the project plan based on “Stakeholder Analysis” doesn’t it basically include A and advances to the next step ?

Good question. "You ARE IN PLANNING!" Did you catch that statement? You do not UPDATE plans in the planning stage. But beyond that ....... "Revised communication strategies" is an unknown at this point. There may not be a need to update the plan. Sometimes the team just needs to WORK the plan better so D is presumptuous even if you were NOT in planning.
