I’ve had diabetes for more than 7 years and no one told me about pre bolus lol. I knew about it from youtube, my blood sugar used to spike so high after breakfast... I just started prebolusing 2 months ago and omg it changed my life I’m so happy 🥰💕💕💕💕💕

Hi Andrea, I am a type 2 diabetic on insulin. I take Lantus in the morning and at night, I also take novolin with a sliding scale with meals and ozempic to help with my blood sugars. I recently got my blood sugars under control by watching your videos. Keep up the good work.

Andrea I am from Canada and have been a type 1 diabetic for about 25 years. I am 70 now and am really just now accepting the fact and starting to take it seriously. I changed over to two days ago from NPH to Lantis and bought into Freestyle Libre. I love the mo nitoring ability without the finger pricks. Your videos are just great to say the least and you are an inspiration for me. Thank you for what you are doing. This video is extremely timely as I have been thinking of ways to flatten the curve after eating. I lived in Morocco many years ago and still dream of the smells of the madinas in Fez and Marrakech. Now those are markets. Student price...........:)

Andrea, thank you for the super helpful - and engaging - post. I urge every type 1 diabetic to watch it... I've been type 1 for 20 years (OMG!) and was taught about pre-bolus... for 20 mins, during my 2-day 'Hey, you've got diabetes' interactive hospital tour, before being thrown back into the normal world again. About a year ago, I returned to pre-bolus, and, having taken the time to really listen to my body, it has made a massive difference. I've always had good control, but this really helps avoid hypers. Personally, I've found, along with stress, my pre bolus plans can be affected by temperature and exercise. So, like all aspects of managing this disease, nothing is ever simple... but then the alternative really isn't worth contemplating.

I’m 13 and I was diagnosed five years ago. I was taught to pre bolus in my diabetes training 15 minutes before I eat. We always did it for probably the first year and then slowly got lazier lol. Especially since I’ve been in charge of my own insulin since I got a pump two years ago I’ve been REALLY bad about not pre bolusing. I had a bad A1C at my last endo appointment and my endo really got on my case about remembering to pre bolus so I’ve been really trying to remember these last few months and I’ve noticed my numbers have been way better so I’ll really try to keep up with it! I’ve also been going to diabetes camp the last few summers and most of the kids there pre bolus and they encourage us to remember so I think more endos and diabetes educators are teaching newly diagnosed diabetics this!

@@MJ-gj6mj yeah! Soooooo cool I’m really lucky that I got to go for so many years. I didn’t get to go last year bc of COVID and it looks like this year will be the same, which is a bummer. I’m getting older though and hopefully in a few years I’ll get to be a counselor so I can help the new generation of T1 kids just like my awesome counselors did for me :)

I've bean T1D for about 5 months.... been so close to quiting, but I can't, if I want to be here for my kids..... thanks for doing this and coming on here with such a beautiful smile. I have been told about pre bolus as soon as I started injections, the harder part is to keep track of carbs and to control the urge to eat right then.😂I'm a carb loving human!

Hi, my 4 yr old grandson has dt1. They've taught me to pre-bolus when I babysit for him. So we usually do that. The hard part is when he refuses to eat what you have pre-bolused for. So then you're trying to find something else he can be tempted to eat... I've said many times to his mom that it'd be easy to bolus after, when you know what he's eaten. But your videos have helped me understand about pre-bolusing. I need to learn to do it a little earlier than I have been doing it. Ive been binge watching your videos and love them! Thanks! Thank you! Though this isn’t a new concept to me, I really struggle to do it. “Set an alarm”...boom! What a great idea. Your posts are always so encouraging. You bring me fresh ideas and confidence, thinking “I can do that”: go on a walk, do a bit of exercise to challenge that stubborn high, eat a more consistent meal to help me trouble shoot, and now, prebolus. I love when I get the notification that you’ve posted. A bright spot in my day.

I had never been introduced to ‘pre-bolusing’ in almost 15 years... since recently getting the Dexcom I just figured it out independently, as I started to notice these spikes and decided to try giving insulin earlier! Never even knew there was a term for it until now. Thanks for your video👍

Great video Andrea, well explained and very important. I’m a T1D for 35yrs, I learned about pre bolus when starting out on Freestyle a year back. I pre bolus 25% of my dose about 20 mins before, take the remainder (novorapid) as I start my meal. As you indicate it is important to vary the amount or timing based on the pre meal level and also the carb type, eg for pasta i pre bolus less or sometimes take no pre bolus, sometimes I take and a small post bolus 30-60 mins after the meal to catch the late spike that pasta can cause.

My Diabetic Coach mentioned Pre-Bolousing about 3 months ago and it works great. Keeps me in range much longer. You are still very helpful I learned a couple of things from this video.

I roughly pre bolus 10-15mins before food. Sometimes it works, sometimes it doesn’t! Haha. 45 days of being T1D and counting 🤓

Was your daughter, a young adult, told she had Type 1 or Type 1.5 (LADA)? I was curious because first I was told it was Type 2..then Type 1 and they said for insurance purposes really. It wasn't until I switched Drs. and realized I was very slowly requiring more insulin that they explained there is a gray area, much less discussed that doesn't fit completely into either category and that is Type 1.5 Very similar, just a slower progression. Love your channel. I've been living with Type 1 for almost 11 years now and I neglected it for the past 5 and starting realizing that that's not okay and you helped me so much with taking accountability for it before things got bad (kidneys, etc) so thank you for helping me keep track. I've been doing so well for the past month! I feel so much better now.

Very well said. Do Most have an Endocrinologist in England. Here in the states they turned me over to one 30 years ago. Then we were assigned to a doctor that was both regular dr. and endocrinologists in Las Vegas NV. I had the guy for as many years , he pushed pre-blousing. It depends how lucky you are with insurance plans here in the states, and in the individual state. But keep up the good work for the young folks that have to struggle years. My wife said your good looking.

I just found your channel . My 11 yr old daughter just found out she's type 1. She went into dka that's how we found out . She's had it since March 2020. She's recovered good. I think you arw all brave who have type 1 it's so life changing

I just found your channel, & am really happy about it. I have had many doctors over my 45-year long journey with Type 1, & have gotten many different orders. Right now I take my shot right before I eat, I use the Libra.

Well done Andrea. Too many T1D's wait a fixed time instead of doing what you suggested...wait for the bend, in other words waiting to see when the insulin gets into the system which can be seen on CGM. of course if you are running low or high then that formula needs to be altered a bit up or down. But for the most part, watching the CGM patiently for the numbers to start coming down usually indicated the right time. It can vary drastically. Or even better, you can see the bend in the CGM graph. Thanks for the great video. I have been running mid 6's A1c for the past 4 years. My last one about a month ago was my best ever at 5.9%.

So helpful to hear someone explain it in a simple and clear way ❤️ I'm currently experimenting with pre-bolusing and though I haven't perfected it yet its definitely making a big difference! I also found that no doctor ever explained this to me, it's places like the Internet and platforms like yours that have helped me understand a lot of these kinds of issues. Thank you for making videos like this, it's really nice to hear an actual diabetic talk about some of the science of Type 1 😊😊 💕💕

I’m very grateful for this video because I never knew about it in the 14 years I’ve been diabetic. I was always told to bonus when I eat but was frustrated by spikes after eating. Thank you for your videos, super happy to have found your channel. Going to start experimenting with pre-bolusing

Hello, eight-teen years ago I was diagnosed type two, diabetes. I will give pre-bolusing a try. I use a Freestyle Libre, so I guess looking at my daily graph before I dose myself will aid. Never thought about that. Also been using a bolus calculator, seems to help. Thank you for posts such as these,

The Freestyle wasn't covered with my insurance sadly. However, I know it is atleast less out of pocket than the Dexcom would be.

Great video! My 6 year old is T1D and on MDI. Right now we dont prebolus because she is an unreliable eater. Once we get her on a pump (hopefully January), then we can prebolus for half of what we serve and follow up after with what she actually eats. Thanks so much for a your videos. They've been such a help for us. My daughter was just diagnosed a month and a half ago, and you videos have really given me more peace of mind. We even had her watch your dexcom insertion(?) Video before we put on her first "dexi" ❤

I definitely find pre-bolusing to help to alleviate the spike that comes after eating. I'm using 5-10min depending on the glycemic index of the food consumed. Thanks for the info & keep the vids coming!!

Andrea, thank you. This is a very helpful video and life changing information. I have been a diabetic for 34 years and have never been told by any Diabetic Doctor about prebolusing. I mean, why not? It can stop the sudden spikes in your blood sugars. Thank you again.

Thankyou Andrea, for another encouraging video. Like you, no Doctor has ever suggested it to me. And I have never pre bolus . I will experiment with this for breakfast as often have nasty Spike in the mornings. Funny at night though, even if I might be around 10 before tea , after having done some exercise earlier, I can be crashing straight after my meal. Actually it is common for me to drop after evening meal, and just before bed start going to high, so have 1 or 2 units of humalog, haven't figured that one out? Keep up the good work, you are an inspiration for many many people. 👍❤

Many years ago when I was put on Novorapid I was explicitly told by a nurse that it was “so fast” I no longer needed to pre bolus (before then we’d beeb told we always had to inject 30 mins before!). Nearly 15 years later I had my first appointment with a new doctor and he took one look at my numbers and asked me to inject 15-20 mins before eating. So (fairly) simple for the amount of impact it has!! For me it also has a double whammy improvement effect too, as I think more about what I’m eating in terms of its timing relationship with the insulin/ it effecting my BG levels.

This concept is a game changer. There are a multitude of variables that cause it to differ from one person to the next, so a degree of experimentation is necessary in the beginning, but once you've established your "sweet spot" (I see what you did there 😉) it will likely become a game changer for (all of) you, too. Andrea, I personally applaud you for discussing this topic openly. I had the same experience you mentioned, no Dr ever mentioned the first word of it to me either. Don't tell anyone, but you're quickly becoming my diabetes hero, and for that I thank you. Be well, Friend.

Hi Andrea, I love the attitude of your videos they are very helpful to see there are other people doing good and experimenting with their diabetes control. I know you are doing great and you offer lots of great advice and with that I would like to recommend you to make 2 videos I feel will benefit diabetic patients in general specially those who tend be uncontrolled by the ADA standards, HbA1c above 7.5. 1. How you deal with the stress related to diabetes management. 50% of the game is in your head. 2. Watch the attached video about Dr. Bernstein diabetes treatment approach and give your opinion on the matter. Thats his intro, but you also see his approach to hyperglycemia and the diet. His methods are incredible I had full blood work and retina exam and based on the results they could not have guess I was a T1D. My HbA1c went from a horrible 7.8 to a 4.9 within non diabetic normal range. My eyes after 23 years of diabetes showed absolutely no traces of vascular abnormalities. Even my vision improved and I got a new prescription. I feel you are great influencer within the diabetic community and with any of these 2 videos you will have a positive effect on many diabetics. Here is a short video of his introduction and personal history. Quite incredible, granted in my case its only anecdotal evidence its just unbelievable the changes your body undergoes with not having a single BG above 180 mg/dl (10mmol) Thank you!! I live in Ukraine, our technologies and stuff are a little bit late from the rest of Europe, but my doctor did tell me to prebolus around 12 years ago. For those who does not have CGM, just use the glucometer at least when you intend to do a prebolus and wait few minutes more if numbers are high. Regarding CGM, I'm waiting for my first dexcom g6 this week, unfortunately officially it's not licensed in my country, and prices are ofcourse higher than in UK... Hopefully some day we will have access to all the good stuff in time with civilized countries. Stay safe and be healthy. I change my pre bolus a lot depending on a lot of things. I even change my pre bolus time depending on the site of my Omnipod. I always find it takes the insulin a bit longer when it’s on my arm. I also have gastroparesis which really affects my food absorption rates. My Omnipod & Dexcom Are the best things to have helped. I’ve always been a particularly brittle T1 but, now that I have the data, I’ve managed to get my HbA1c crown to 47. I wish I’d had someone doing videos like this when I was a younger & more naive T1. Keep up the good work Andrea.

I’ve had issues with my sight too & was recently registered as visually impaired. Luckily, my diabetes has made me quite the resilient problem solver so, I’m pretty good at making the best of a bad job. In turn, I always seem to be one of the first people the rest of my peer group go to for help and advice. Keep doing what you’re doing. Life is what you make it.

I kept getting spikes after eating, and i had heard to take my novolog 15 mins before eating, but i didnt think it would make that much of a difference to take it just when i was eating, My endo told me yesterday it makes a huge difference, and she is prescribing the Dexcom G6 for me now seeing as i dropped my A1C from 9.2 to 7.2 in 6 months after being increased on the Trulicity. I had previously dropped from 10.1 to 9.2 in 1 month after being put on the low dose of trulicity. I also have lost 2 lbs in 6 months, i hope to increase that.I am so happy to finally have my Dr say Great job! It feels really good to start getting some control, and im gonna be doing the pre-bolous today and see over the next few days how much that helps as well. Yesterday was a great day! I also have been eating better, The Trulicity has helped with my appetite, i'm not feeling as hungry all the time like i was. I am a brittle diabetic, my sugars tend to go real high all of a sudden especially later in the evening towards bedtime.So I am really hoping to get control of it using the pre-bolous. thank you for all your diabetes awareness videos and the dexcom videos as well. I am learning a lot.

My doctor told me to prebolus especially with diners but I keep forgetting....and it is also hard because diners are never the same and hard to calculate...But now you inspired me to try again. I just started on CGM witch is hard work but I can see the benefits flowing in allready. This prebolus will help even more. Stay save all.

I hear you! It can be really hard to pre-bolus when you don't know exactly what you're going to eat. In that case I would do a very conservative pre-bolus (the minimum you know you'll need) and then take the rest when the food comes. Hopefully this helps! Plus I think you'll find it much easier with your CGM!! All the very very best to you!!

I'm 20 years old and have had diabetes for 17 years, I had used insulin pens/shots for that entire time up until a week ago when I got the Tandem pump. Watching this video is funny to me because it's all I've ever done! Before getting the Tandem pump (using injections) I had an A1C of 6.4 with 70-88 percent in range, which was largely due to getting my shots around 15-20 minutes before I ate. I'm excited to see how the pump will change how I approach things, but right now I'm still looking at it as an attached insulin pen.

Thanks for this video Andrea thank you so much for this video, I never even knew there was a term for this. Watching your videos I have come to realize that my doctors are not very informative.. so thanks again for these detailed videos. One thing I find that I struggle with is when I get low blood sugar, I tend to really freak out when I get low BS, and so I also don't know how to handle the whole do I take insulin before I eat to bring up my BS? I find that even if I just eat a little bit, sometimes my BS just shoots up, others it doesn't and almost always I get a headache. How do you personally handle this situation?

When I was in the hospital during my diagnosis they pushed prebolusing. After I got out, I didn’t have any doctors discuss it with me at all. I got back into it a few months back and the huge difference it made was crazy. I was weary of it when I was on MDI because I didn’t want to “limit” myself on the amount of food I could have. Now that I have the pump, I don’t so much have that problem. Wild how the simplest things haven’t been normalized by doctors.

Hi Andrea, just subscribed to your channel and was wondering what this Pre-Bolus thing was. On watching your video I realised that I was taught this from day one way back in 1976 when I was first diagnosed. In the days of porcine (pig) Actrapid insulin, at the time the impurities in insulin were thought to slow its absorption down so it was recommended you take your insulin 30 minutes before eating. With the development of synthetic human insulins in the 1980's the absorption time reduced but to this day I will try and take insulin 15 minutes before meal time to help reduce the glucose spike.

Hi Andrea. I’m curious if you have tried ultra rapid acting insulin. I used Lyumjev for the first time today. Took a dose at the start of my lunch with no early pre-bolus. I barely had a spike reaching only about 125 before going back below 100. It was amazing. You may want to try it.

You know what - I never have! The fastest I've tried is NovoRapid (Novolog in the US I believe) - I'm so up for trying a faster acting one though - oh that would be amazing!!! Especially what you've described above - I'm going to try to source some - thank you so so much!!!

I treat my highs with a bolus and my lows with a bit less than 1/3 of a glass filled with flavored milk. I treat it straight away. The 2 litters of flavored milk last me more than a fortnight. I take Lantus before bedtime and 3 or 4 injections of Fiasp during the day... Usually I have the flavored milk if my sugars are low before bedtime. Cheers I love your videos...

I have to pre-bolus for every meal. If I wake up at 8AM and take a pre-bolus, I'm not usually ready for breakfast until 75-90 minutes. I target breaking 100 as "the time." It will continue down a little but usually starts up around 85-90. The same is true with most food depending on my starting level. If I'm 200, I've waited hours before I'm ready. The more I pre-bolus, the lower my A1C. It is easy with my Tandem X2

I’m just over a month into using Libre so am seeing those spiky graphs and realising I had no idea how much my sugars went up and down between tests! Going to try this out now 😁 thanks for sharing!

She's Diabetic my experiment was blown up by my v frustrating recurrent cannula issues... sharp spike upwards about 12hours after changing cannula 😠 been having problems for ages, finally diagnosed it due to seeing Libre graphs... gonna try steel cannulas soon

Excelente video and very helpful because i strugled with this..and i would add to thisto make the control too much easier in addition to this is eating very slow carbs vegetales...avoid all high GI foods, you will see truly flat line.

Isn’t this what MDI patients do on a daily basis? What is the difference as long as you inject 10-15 mins before meals with the knowledge of how many units to inject compared to how much CHO you are consuming? When I was diagnosed with T1 I was told to inject a certain number of units and to titrate up by a unit every 4 days if I needed to. I found these numbers a little arbitrary. I was offered to take a course for T1s which gave me the actual info I needed. First 10 units are free but beyond that give 1 unit of insulin for every 10g of carbs plus how to give a correction dose if needed.

I pre bolus my sons breakfast by 30 mins and it really works 99% of the time. I find other meals tricky to do though, as it's more impractical when you're serving up the meal.

Andrea, that is such great video. Only new to your channel. love all your tips and tricks! anyway, I have had T1 diabetes for 24 years and pre bolus was never mentioned. I just received a new 780 Medtronic pump. Its great and my sugars have been in range for the first time in about 20 years! HbA1C was 10.4 and now 7.1. I never saw a 7.anything in my life! I do seem to be going through a lot more insulin and as I am not very mobile after surgery I fear the weight going on as I am heavy already. Any ideas or advice? Also.. how do you know how much to pre bolus. Most days I am not sure what I am having for dinner until I make it or it is put in front of me. I always worry about this in case i give too much or do you just wing it.. Thank so much.

Thank you so much for your kind words Majella! And congratulations on your fantastic HbA1C! You should be SO proud of yourself!!! I completely understand the concern of going through Insulin more rapidly and fearing weight gain, but also keep in mind that this does not always have to be the case - being that your body is in range - everything will start to function more healthily, give yourself some time to enjoy this time in range and see if your hunger/energy levels contribute to a more healthy weight that you feel comfortable with, maybe! Of course I'm not a dietitian, and maybe meeting with one would be of help to you! However, I personally always try to prioritize vegetables on my plate and make sure that those are the "star" of my meals. This seems to keep the nutrition and vitamin density high and the caloric density low, but I can't stress enough how personal this is and how you need to find what works best for you. With regards to pre-bolusing for dinner, I would go ahead and dose the absolute minimum (very conservative) that you would need in a pre-bolus fashion and then do a second follow up dose when you know what you're going to have for sure to flush that out the full dosage that you need for the meal. I really hope this helps and I wish you a very swift recovery from your surgery! Keep the faith and keep up the absolutely AMAZING WORK!!!

Usually before a big meal, and 1 or 2 units does not do anything. I was once told to do it if ti was over 200; but the insulin does not get in your system, usually I will eat if I pre-bolus when the sugar starts moving more the ten points depending on what I am eating and how much insulin is needed to subtract the sugar out of me.

I really have enjoyed your channel. When people are diagnosed do they typically have the option to be on an insulin pump or using regular insulin pins/injections first? Do people typically have the option to have a glucose meter attached to them first? Also i do find it amazing to how pre-bolusing was not the first thing you were told- it makes total sense. I don't have it, but I am trying to learn all i can about type 1 because a friend in school has it.

Oh wow - what a WONDERFUL friend you are to be doing such research and asking such intelligent questions. I have no doubt your friend is extremely appreciative of this and clearly you are a person capable of wonderful empathy! Thank you, basically! For being so caring! In terms of diagnosis choices like pump or pens, it massively depends on your coverage, the country you're in, etc. When I was diagnosed, though I had excellent coverage I was not offered a pump, only pens. Actually needles and bottles of insulin at first. And in terms of the glucose meter, it was the same - I only started CGM (continuous glucose monitoring) recently (in the last few years). The technology is constantly and rapidly evolving, so some of the above is simply down to the fact that the tech wasn't available when I was first diagnosed. I wonder what it's like for people diagnosed today, and if the tech is more widely available from the off. Would be fascinating to know! Anyway - thank you so so much for your comment and for being so caring to your friend. You inspire me friend!!

@@ShesDiabetic You are sweet! And, that is very interesting about your experience! In researching at first it "seemed" to me as an outsider that a pump was an "easier" choice. But from watching your video about your pump experience and other people's videos about pen vs pump i can see there is not an "easier" choice as both can have issues. Although I can "guess" from a doctor's perspective it's better to start out with learning the basics of using pens since there are more factors to consider when using a pump - and in case of pump failure you would need to use your pens anyhow on an occasion. I'm from the US and it is crazy how a pump ( well if I remember correctly from some videos I've seen ) a pump can be more expensive and it depends on what your coverage is. I mean I know in the US we have a huge problem with health care, but people with diabetes should have whatever they needed covered no matter their coverage without paying extra. Like if someone wants a pump it shouldn't be seen as a "luxury" to insurance companies. All seems crazy to me, but hopefully, the healthcare in the US will change one day. Some of the tech for diabetes today seems so amazing and how many things connect with apps. And,in life if you care about a friend you learn about ways to help them if needed:). She is a newer friend i met this year, so later on if I know her more I'll ask her questions :). (Everything I would like to do is in the creative field, but I always liked learning about medical things.) Sometimes you need to be careful with people, because not everyone is very open. At first I was nervous about pre-bolusing. Once I got used to it I was even able to stop spikes after snaking when I did need to bring my sugar up a little bit.

What do you do in unknown situations? When eating at a friend’s house or at a restaurant it is often unclear when food will be delivered and how large the portions of carbohydrates will be before they are served. Unfortunately, the default may be to wait to bolus until the meal is served. However, I thought I understood you to say in another video you gave yourself a small bolus ahead of time in these situations - less than what you expect to need for the meal but something to get the process going. Did I understand you correctly? Can you be more specific (perhaps in another video)?

I do it for more than 30 minutes if im out of range and it realy helps control things a bit more( i bolus for the meal and the bit that's out of range). If my blood sugar is in range i will pre bolus but i'll do it for under 25 minutes, sometimes i'll do it for under 15 minutes I use novorapid and i take 1 unite for 10 grams of carbs

Hi, I’m new! I’ve had T1D for 40+ years now. I’ve been on a pump for about 20 years now. I just got the Tandem t;slim and Dexcom G5 here in Australia. Now I’ve remembered that back in the good ole days of MDI that of course I pre-bolused. But somehow, since my first pump, it was never mentioned again. I picked it up recently from a Facebook group, asked my DE and she said that she had just assumed that I was pre-bolusing. I have to say that I am truly shocked by the information from a CGM. I always have a good HbA1C of around 6.2 - 6.8, but after seeing the patterns from the CGM I am totally shocked at how I manage to keep it this low.

Welcome Di! And thank you so much for sharing your experience! I know what you mean about pre-bolusing and there not being this mentioned when going onto the pump - actually that was similar for me! It's like the training on and technology of the pump overshadows the basics that are basics and so helpful for a reason! Happy to hear you're gaining insight with your CGM!! Good for you!!!

Hi, I think the best indicator in general whether is R, lispro, apidra or aspart insulin is determined by getting a pre meal BG level, administer the insulin for the meal and check your BG after 15 minutes and wait until you see a 5 mg/dl about 0.3 mmol/L drop from the pre meal level. If after 15 minutes you don't see a drop repeat 5 minutes later. That indicates the start of action of insulin. Carbs are absorbed so quickly that at peak of insulin for rapid acting insulin about 45 minutes there is considerable serum insulin to at least try to minic non diabetic phase 1 insulin response. Assuming good carb counting this should limit spikes to normal non diabetic 140/160 mg/dl shortly after a meal. For me just like her, my time is 22 mins with lispro insulin. R might take 45 mins + and I have no experience with aspart insulin or apidra.

@@guskokalis9535 It is a your biological time for insulin absorption. If you take the insulin too late the BG will spike really high and then come down making you feel sick, now likewise too early your BG will drop until the carbs get digested. Rapid insulin vary slightly like humalog is a bit stronger than novolog but about 5 mins slower. The only way to figure this out is to check your BG before a meal follow the insulin suggested action time and check your glucose until you see a drop of 5mg/dl. That is the time insulin takes to start acting. By the time carbs digest about 15 mins after consumption insulin will be at its peak and will start absorbing glucose immediately. Your BG will not raise extremely high. My peaks after lunch are about 10 mg/dl with a low carb diet. About 0.5mmol/L carbs are more unpredictable so if you let say eat 50g of carbs your BG on average will raise my BG nearly 300 points.

Pre-bolusing is so essential it should always be taught right after diagnosis. It just makes sense. Subcutaneously injected or infused insulin has to first make it to the capillaries, then it has to travel all the way to the liver (to suppress its glucose release), and also has to reach insulin sensitive tissue. Contrast this with natural endogenous insulin, which is secreted by beta cells into the portal vein. It does not have to reach any capillaries. Instead, it reaches the liver _immediately_ , and the other tissues only seconds later. Of course injected insulin cannot keep up! In light of that, it is rather shocking that pre-bolusing isn't being taught. I've been able to handle large, complicated meals (like rice with a whole lot of chicken and rather fatty sauce) with a combination of pre-bolusing and dual wave bolusing, and the resulting peaks were below 150 mg/dL. Overall, partially thanks to pre-bolusing, a BG above 150 mg/dL is very rare in my case. The other big _big_ BIG factor is the finely tuned basal rate. Also, thanks for mentioning the sensor. Pre-bolusing _can_ lead to a low if done improperly (perhaps that's why it is not being taught). Without a sensor, using it is more of a risk. Oh, and my pre-bolus time is 15 minutes (shorter if I'm low).

I replaced pre bolus with FIASP insuline what is an ultra fast working insuline. Pre bolussen with my chaotic live was not always possible, and the Fiasp has also a better effect for me with the correction bolus. I have to change later this year to a new pump (big change that this is tandem t:slim X2 with control IQ. Seems that with this type of insuline are much more collusion alarms then with the other insulin, that together with the fixed active time of insuline of 5 hours in that pomp means possible that I have to change again. I really hope not because the fiasp was really a game changer for me.

@@ShesDiabetic Thank you dear, I appreciate it. For my 11 year old son base on what his doctor said is 150 or less. But sometimes when he eats his food he goes around 190 or 200 and after 2:30 hours he starts going less than a 150. I want to know if this is typical rection for majority of TD1 or not. For instance before food he is 120 , 8 unit insulin and after four 2 or 2:30 hours 180_200!!!. Is this common reaction! I hope u are doing better now. My prayers are with u all the time. Thank u. I noticed you mentioned pasta and doing the injection earlier for that meal. However, wouldn't pasta actually absorb more slowly and maybe require a later dose vs earlier? I realize this is different for everyone, but timing really is important. I actually prefer own injections for this reason and also the location of the injection. I would love to hear more tips on this! Peer support on these topics sometimes really is more helpful than Dr. appointments (sorry health professionals, it's nothing personal :)

Great question! Like you say, we're all different, but for pasta, which would be a very fast-acting carb heavy meal for me personally, I need to get a good amount of insulin on board prior to any meal that has a heavy carb content. This way when the fast-acting carbs hit, my insulin and body have already prepared for it with that pre-bolus, and thus the hit of fast acting glucose isn't as extreme and I've got the insulin to match it, rather than spiking and needing to battle it down. I hope this helps answer your question! Like you say - we're all so different so you just have to find what works for you - but this is my way of dealing with that kind of meal 💙🥰

I don't know if you know about keeping cars real low and not eating things like rice and pasta and potatoes. It may be different for type 1 diabetes but I find no to little spikes watching what I eat. Whatever you do, do not follow the American Diabetes Associations recommendations for daily carb intake. Your are probably a head of me on this . Thanks for your videos.

So my question is, food takes approximately 4-5 hours to bring down. If u bolus early then won’t your insulin run out and won’t you still have food in your system to bring down..? Hope that kinda makes sense

This raises a giant red flag in my mind. Something is amiss. Talk to your endocrinologist about post meal target range (ie - 2 hours post @ 180), the amount of time your fast-acting insulin remains active in your body, as well as your carb load per meal and the associated required insulin dosages. Since KZbin is not the place to get medical advice I won't offer any, but I will suggest that something is off base and needs to be corrected by your diabetes management team. Sooner rather than later. Good luck to you.

ES - again - you've said it perfectly, thank you! Food does take a while to digest, but insulin is active in your body for quite a while too, so there should be a relative matching to that time frame - however, ES has is spot on - talk to your doc to make sure all of this is considered within your dosages. If I’m going low quickly I wouldn’t pre bolus and take my insulin with the meal, but if i was only trending slightly low I might tighten up the pre bolus time, say 5 mins rather than 10. I hope this helps! Please know everyone is different!

If you're on injections you need a needle that has these markings on it (.2/.25 units, etc), or if you're on a pump you can 'dial up' the increment you wish. I hope this makes sense! If you don't have a pen that allows you to do this, be sure to speak to your doctor about getting one! I hope this helps friend!
