P.S. This has nothing to do with the death. These are just my observations based on the demeanour and words of the commanders. The real problem is that the SAF is run with a similar structure to the MNCs, where the commanders don't personally interact with the men on the ground. Much like how the CEO of a MNC doesn't personally interact with those at the bottom of the food chain. They get the perspectives of the mid-management, many of whom are seeking to climb the ladder and commit to impossible positions. Who bears the burden to meet these unrealistic targets, the ones at the bottom of the ladder. I was fortunate during national service in that my commanders spoke to us directly to understand our difficulties and it made life more manageable. Can't say the same about my work life, but that's a separate story. The point is, those at the top must, at least occasionally interact with the ones at the bottom and not simply rely on the word of middle management.

@@TM-hg4lv such situation shouldnt be "solved" with a black and white solution. If you said NS should be abolished then who will protect singapore in times of needs? Why will other countries bother helping us defend if we ourselves dont want to? Now if NS were to stay, innocent lives might be taken selfishly. The answer to this problem will be a change of training or an increase of safety in terms of personnel stationed at training sites to oversee safety.

Speaking as an SSPH combatant, the safety procedures were in place. It was up to the 3 personnel in the vehicle to abide by them. Aloysius was crushed between the ramming assembly as it was moving upwards and the rear wall of the internal SSPH cabin, as the gun barrel was being lowered. Typically, the Det Com controlling the vehicle was trained to check clear the path of the entire rear assembly before moving the barrel, as well as alerting the crew of the movement of the barrel. One of two things could have occured: 1. Det Com did not check clear the path of the ramming assembly 2. Det Com alerted the crew to his intention to move the barrel downwards, but it was not heeded by the victim, and Det Com proceeded to blindly move it anyway This is really human error, and is truly unfortunate. To be honest, there is nothing more that the higher ups can possibly do to make the procedure more "safe" already. Check, double check, triple check, that NO ONE was in the path of the assembly, but it still happened. Freak accidents are freak accidents. Condolences to a fellow country man.

Ya,I would have beaten anyone of these 3 jokers in a street fight, how many of our fellow Singaporean sons have died during training, we have to serve and risk our life while thos' high class' foreigners took our jobs and enjoying the security here at our expense, the reason why those are sitting there are because they are obedient, not deemed a threat to the government, to die during NS training for this current government is simply not worth it. I feel the sadness just like all my fellow Singaporeans, RIP Aloysis son of Singapore.

With the current army, Singapore will still get taken anyway. So what is the point of doing NS with the current system? The current system must be revamped. And how do I know Singapore will lose? The 3 to 1. Singapore has only one army, others countries have more. Basic art of war, we surround our enemy when we outnumber them. For 3 to 1 to work, we have to outnumber them 1st. Does it seems like we can outnumber them? Other countries probably trained their men in the opposite manner, which is 1 to 3, or more. It's a time tested method. Then, there's issue with experience, we are not exactly plague by war, riots etc. So please do the math, what are our odds of winning? And if anyone raise a debate about our officers' ability, well, with these REPEATED mistakes that resulted in deaths of our serviceman....... seriously? I don't even need to share my personal experience. And is SAF accountable to the us? Well, SAF is funded by the government. The government is funded by the taxes we pay. So, is SAF accountable to us?

I'm more interested to know a few things that were not mentioned in the press conference. 1 the noise level inside the tank (this will determine if the victim have actually shouted for help or not or can he be heard or not) 2 nobody have actually mentioned if the SOP have clearly dictate where the technicians should be standing (in sight of the commander or immediately behind the commander) when he's lowering the gun 3 besides the commander and the victim, there is another technician (28:00 mins) inside the tank. where is this technician? WHY have he not noticed that the victim is in the path of the gun as it's being lowered to standby mode?

I watched the video and I still can't tell how he was injured. Be specific. This incident had brought me back to my memories when My cousin fell from the parachutes when it fail to open during his commandos training. My Aunty nearly lost a Son but luckily he live. He is now nearly 50s n is a dialysis patient. Should SAF do better when providing support to those injured during training? Not just monetary compensations. RIP...... brave boy ......

And they are good at pushing responsibilities just like PAP their big daddy boss. They will only put up a show when an accident is already exposed to the public and TV cameras. Wayang wayang....

Safety is a individual responsibility as well as collective resolve in following the safety system. However, in army where there are safety talks organised , alot of soldiers play truant and not attend these talks because it is boring . And when accidents happen they blame leadership.

9:08: about conducting refresher. In reality, this is often very "rushed" and usually just a surface sweep, conducted by demonstration and hands-on. Most often, due to NSmen rushing to meet the training program timing, these are cut short and eventually become a "paperwork" to log that training was given. Take for instance, TSR (Training Safety Regulation) test and Technical Handling Test (THT) that is mandatory to "clear" as a paperwork to SAF and the public. A 2 hour refresher for someone who spend 351 days doing a regular job (acting, office, etc.) and then to handle/maintain weapons of war? How can a commander realistically study the entire TSR in a few hours, and then pass it given the number of questions and pages (>80% or 90% score needed to qualify a pass) Furthermore, ~90% of the questions/topics are irrelevant to your vocation or training. But this is hard on SAF simply because there is not enough time. No matter the money, time is a resource that cannot be traded. To try to make training meaningful, effective and safe yet not take up too much time of a Man's life. The nature of war is destructive and there just cannot be a way to train safely without compromising effectiveness. The question is: Where is the balance that we can accept and if so, can we still accept the effectiveness? Should the entire defense strategy of Singapore be re-evaluated? I for one, am pro-NS but given the dilution to training programs and conduct. Degradation of the quality of our professional soldiers (Regulars) and their lack of commitment, passion to the cause. I can't help but falter in my beliefs. Should a new form of national service be developed, where both gender may contribute? Where we do not train for war but nation building? E.g. public services, projects to improve the nation, life saving skills etc. Should Singaporeans demand for such a re-evaluation?

Accidents Happen When People Rush. TSR Relates to Safety in General. More Focus Need Be Emphasise On Vocational Safety. Chances are Most of TSR Would've Been Forgotten, Once the Test is Over. It Also Depends On Attitude cum Negligence. This Accident Shouldn't Occur if The 2 Step Procedures are Faithfully Followed: 1) Verbal Clearance and 2) Visual Check.

I have some questions. I hope CNA can ask them again when they have another conference. To CARMO Dr Lo: from the incident, how was the deceased treated and how long was he treated at the BCS? then to medical centre and finally to the hospital? I read from news source and found it took the SAF medical team 6+ hours to finally transfer him to the hospital. Why wasn't helicasvec activated prior at the BCS? Transporting the deceased within mountainous area using ambulance or safety rover? There is a directive to use heli for such urgent cases. Crushing injury to be transported using rover in mountainous terrain, doesn't this worsen his conditions? BCS and medicalcentre has no X-ray facilities to examine the crush injuries. So how long was he kept on transport, how long at BCS, how long at medical centre before deciding to push to hospital? To: CSSCOM COL Terry: "refresher training done. Was it done in a simulation environment or was it done in real primus? Simulation training and actual training has significant difference. Any form of grading for each technicians fo determine their skill level and capabilities? Skill level is something that differs from one person to another. So when u say qualified. "is The deceased trained to repair the primus to what extent? Should he be troubleshooting a complex hydraulic system? How qualified? Is he trained to check oil pressure? Remove hoses or test for switches faulty or check for any sensors fault? Please clarify his qualifications on repairing of primus. I suppose his level will be very basic. 3. To Chief of Army. (Cant see your name) The pictures you show the public is a clean and empty primus. During live firing exercise, the cabin will be filled with all the 4soldiers equipping, stores. Kindly show the public the actual picture of the vehicle cabin. There is nothing sensitive as its mainly the four soldiers rifles, field pack and other stores /supplies. Please show the actual picture. One additional question for COA. What was the barrel position when the gun commander called the deceased to check. Kindly provide degrees. Was the barrel stucked in a certain angle and hence the deceased was called in to check? Was the deceased wearing a helmet and headset communicating with the gun commander? If the deceased wasn't wearing a helmet + headset. I revert the question back to CSSCOM COL Terry. During all the refresher training, was wearing helmet and headset a must in your SOP? Do you train your techs to understand the gun commander roles and what they should do and expect during such situation when the barrel is stucked in a certain Angle? Should the tech disable the engine? Or should there be more techs to be sent for such repairs? Pairing senior regulars tech with nsf or nsmen. Do you think this is a much better solution? Final question to CDF. Stroke incident. Vehicle incident. Jungle incident and now crushing incident. What has been done so far? What has the army reviewed and changed? What are the steps in place currently to prevent the earlier 3 death incidents. Has all the training programs, directives, method been revised? Or it is still dated like 1991 or 1995 etc. Kindly clarify the changes army has implemented since the 3deaths. Also pls keep the public posted on what will be done on this 4th case. Also, since Air Force and Navy has been running their system well. Should you consider adopting their standards as they are dealing with more sophiscated weapons and equipments? Should the culture be change or perhaps either Chief of Navy or Air Force Should temporary assumed your position as CDF and try to instil their culture in Army? Perhaps that will improve things as culture and perspective are on a different level. Do contact me if you have answers or need more questions. I have tons of it. @@alexteo6371 every soldier that served the SAF before will know that not everyone has access to their TSR and training directives. The soldier that went through the training doesn't even know what was written on paper and what's wasn't. Even CARMO himself can't even explained the treatment process accurately. The TTSH surgeon did a much better job clearly but was withheld from saying more details. Clearly they didn't extract him out immediately and now saying he died on surgical complications. Easy to say that but when the process of lifesaving is delayed, given top surgeons that intervene would not have help either. 6+ hours delay is not acceptable. They better explained what treatment and investigations was done at BCS and their camp temporary medical centre before pushing to the hospital. The decision made is too late. If this happen in Singapore, the soldier will be evacuated to the nearest hospital at the shortest time possible. Last case of heat stroke death was due to delay in treatment and they have yet to reply on this. Improvement? I certainly think the all the service chiefs should step down. Investigations doesn't take months or years. They have the answers but refused to take responsibility.

This is An Area Worth Implementing, As Safety has Been Compromised. TSR Must Be Ingrained & Drilled into Training Proper for All Soldiers. Due to Medico-Legal Reasons, Full Explanation Not Granted, However The Public Demands a Proper Explain. There were Crush Injuries / Trauma to Multiple Organs and Post Surgical Complications. All National Servicemen and Members of Public Deserve a Proper Answer cum Reassurance. In Addition to All Commanders, Soldiers and Buddies Have An Onus to Watch Out for One Another. Let this Be a Final Warning. Coroners' Court of Inquiry is Always There, Awaiting. Wrong Doers Be Forewarned Over Acts of Negligence. 痛定思痛。

Time Taken to Set an Infusion Drip Until Evacuated to BCS. 3) Still Have to Find the Exact MGR Location of the BCS, Whereby Helicopter Can then Be Summoned. This is Complicated By Nightfall Due to Visibility. 4) How to Radio the Helicopter (Perhaps Out of Radio Range) and Let them Know the Exact Location of BCS. And the Exact Location, MGR of the Nearest Hospital. 5) Delays, Can Occur During Transfer to Regional Hospital for Crush Injuries. Can't Really Speculate What Actually Happened. Need the Final Report by COI.

@@alexteo6371 all I can say 6hours+ to send one crushed injury patient to the hospital is something COI better not missed. I been to NZ and all I can say is that the nearest hospital has heli services. All it cost is abit of money. 30mins he's in the right hands.

This is the 8th death in a shot span of one and a half year, all the committee of inquiries (COI) didn't improve safety at all. This is clearly a crushing hazard and some form of infrared safety switches should be in place, there should be a safety lock out procedure commonly practice in the industry so that nobody mess around with the machine while someone is repairing it but in this case the gun commander can still have full control of the gun while poor Aloysius working behind not knowing his life is at the mercy of a push of the button by the commander. After so many death and our SAF is still too arrogant to admit that there is a systemic failure in safety and they need help from external safety professionals. Our soldiers are not kills in war but by the stupidity of the leaders, If they don't change their mind set this will not be the last.

What is the point of having this press conference when there is nothing to announce? So many motherhood statements. Training tempo is not the issue; SOPS are. Obviously, with so many deaths, those in command are incompetent - they should not be in office until they actually do better. How many deaths and accidents are needed to train YOU to do better?

Based on the press conference, the fault likely lies on the other 2 persons. Either the commander wrongfully activated the lowering procedure or the one next to Aloysius Pang had confirmed the clearance for lowering without due encertainty as there are only the 3 of them present inside the vehicle. Most certainly one of them will be court marshalled to put this incident to rest.

Even this is the preliminary investigation conference, please at least give a clear dictation of the accident happening!! Where was he crushed, on the cabin floor or hung up? where were the other 2 staff positions? Why wasnt he aware of the lowering of the gun barrel? Who activated the gun barrel lowering? Stop harming all the poor Singaporean boys in army for the country. Pitiful cold body underwent 3 surgery and post modem. RIP AP~~

There is nothing wrong with the gun barrel and cabin design. It was the safety procedure that have not been complied in doing the repair job. It could also be the repair and servicing procedure that is questionable. There is also a very high possibility that his injuries treatment have been delayed at the site of the accident and all the way to the hospital which is the most common cause of eventual death.

The following is a forwarded message: When I was serving the country as a soldier, I served in Control of Personnel Centre. During my stint, I received awards for productivity and process improvements signed by the then Chief of Army and also Dr Yeo Ning Hong. Now I am serving as a Practising Management Consultant certified by a Board closely related to the government. My job concerns the design of management and operations systems for companies. The principles of business management & designing operating procedures for these tanks are the same. When I am looking at the design of the Self-Propelled Howitzer, I would tell you that the design is inherently dangerous and no amount of SOP design / redesign is going to change the fact that this is a dangerous vehicle. 1) In the press conference, one officer mentioned that CFC Pang could not get away in time. That is nonsense, that space should not be occupied at any point of time. Given that all the potential movement of the turret would crush anyone who is there, that space should be blocked off and only to be accessed (if ever) during repairs. 2) I remember some officer mentioned that "usually most people would not have problem". That is one sure way of pointing the blame at CFC Pang. That is nonsense. I was not trained to be operating the Howitzer gun, so let me just exercise my commonsense a bit. Did the tank even take into account of the recoil effect when the turret is firing? To take into account of the recoil effect all the space behind the turret cannot be used at all and access blocked off saved for maintenance purposes. 3) One officer even mentioned that there is a procedure to sound an alarm or informing when the turret is moving. This is again nonsense, if CFC Pang is repairing the turret, he must be the one who can decide when the machine can be used. A few of my peers who are in workplace safety has mentioned this practice known as LOTO (Lock out tag out) system, in which no officer of any rank should be able to override to operate the machine when there is a soldier repairing the machine. The design of the turret is from ground up, wrong, and let's not mince words. When the design of the system is wrong, no amount of SOP nonsense would be able to compensate for an inherently dangerous design. All management consultants would be schooled in Deming Principle - when there is an operational problem, 94% of the time you can find fault with the systems and only 6% of the time the error is human. Just basing on prima facie analysis of the design of the system, I am sorry the fault is with the design, so the fault is with the designer of the system and if the system is not locally designed the fault is with the person approving the purchase of this system. Do not let SAF explain their way out of this. I am very infuriated by the press conference they have conducted. While they expressed their condolences, they did not take full responsibility for this. All the other accidents I can explain away with logic and probability (like allergy to smoke bombs and stuff). This is totally avoidable and I want to see Singapore Armed Forces take full responsibility in this. There is no way I can logically explain this away.

A standard procedure was mentioned that after verbal command, area must be checked clear before lowering the barrel. But in actual situation was this being followed? Was the whole 10s of lowering being kept visual on? What instructions were told to Aloysius Pang before lowering the barrel? What were were the Commander and the other technician doing during the incident? Too many gaps in the whole explanation. Lack of info!! There must be more details to this! A continuous mentioning of reduction of training tempo does not play a big part in this tragedy. It doesn't mean that reducing the number of times would prevent such incident. It's whether if the proper procedure was being taken place seriously and communications were conveyed clearly. This applies to any other training exercises. This is human lives at stake. The truth must be told. Accountability must be taken.

I was serving at that time. Based on my experience safety measures are in place on paper but commanders pressure men to breach them to hit the timings and KPI. One commander warrant officer literally jumped from the tonner and encouraged us to do the same. Just one of the few examples.

It is really very sad and chinese new year is just around the corner!!! Any family would be heart broken by the death of their child/son/brother/cousin/friend. But truthfully, these news will just disappear after his military funeral and the government will act like nothing ever happen. Plus, the army safety standards will be very high for now but after a few months that standard will drop and chances are, another death will sadly happen. It is preventable but unavoidable.

Isn’t there any killswitch or mechanism to halt the gun barrel from operating during servicing it? Shouldn’t the operator or specialist to ascertain the gun to be in non operating mode while performing maintenance??? Shouldn’t there be any safe distance to demarcate between the gun barrel and service crew within the chamber????

It was mentioned that SOPs were in place, trainings and competencies were done so what really went wrong?Human factors and/or system factors are causes of errors, mindef has to address these in their finalised investigation report to the public.
