Prometric Skills Demo for Hand and Nail Care

According to Prometric guidelines for the nurse aide state exam, this is a step-by-step demonstration video for hand and nail care. This skill is one of the 21 testable skills on the Texas State CNA Exam. Remember to include indirect care with this skill. Prometric requires the candidate to (a) place the fingers or full hand into a basin of water to soak before removing residue from under nails and (b) to rest the resident’s hand on a barrier such as a dry towel once the hand is removed from a basin while providing nail and
hand care.

Hello Tanya, Sorry for the long delay. I just got back from vacation. Some school use two hampers (one for wet linen and one for dry linen). There are other schools that only use one hamper. The state requires one hamper to be in the testing room. I use two hampers at my site and so does all of the sites in Houston, TX.

Hello Angela, That is a great question. Prometirc wants us to have clean hands before we touch the patient. I have the choice of washing my hands before touching the curtain and the call light or After giving the patient the call light and closing the curtain. The rules are, clean hands before touching the PATIENT and to END THE SKILL with clean hands. Does this make sense

Hello Helena, You bring up some great questions. We are given a certain amount of time to complete 5 skills. Prometric has told us what will be graded for each skill. Therefore due to time restraints, we should do what is on the skills checklist. I may do differently in a real environment. This is just testing.

Hello Noorita, In Texas the candidate will not be asked to remove his or her mask due to COVID 19. Things my change later in the year, BUT, as of 2-27-2021 the oral care skills are not being tested. A bath is tested because this skill is performed on a mannikin. The mannikin does not have to wear a mask. I hope this is clear.

Hello The Sketch Artist, Prometric does not give a time for soaking the hand/nails. I teach my students to place the hand in the water for about one minute and continue with the skill.

Hello Caitlyn, Prometric has ALWAYS tested on the Hand and Fingernail Skill. Did you instructor say why Prometric is not testing on this skill? To my knowledge, Prometric is not testing on three (3) skills that have to do with the mouth. The reason for not testing on the three skills is because the client will have to remove his or her face mask. Right now, during the COVID 19 epidemic, ALL STUDENTS are required to KEEP their face masks on during the entire time of the test. The three skills are feeding, brushing the teeth, and denture care.

Hello Maria, If you are doing a skill and forget something, don't panic. Tell the evaluator that you need to make a correction. Make sure the patient is covered as necessary, have the call light in reach, remove your gloves if you have them on, and wash your hands. NOW with clean hands go and get the supplies needed and be sure to get a new pair of gloves if needed. Return back to the patient with clean hands (this is part of indirect care) and proceed with the skill. I hope this is clear.

Hello Nancy, I am not sure what state you are in, but, I can check if Prometric tests in your state if I know. The skills I teach are good for any state that uses Prometric to test their Nurse Aides.

Hi Dr. Lee I have a question, what should I do if the patient has long nails and they need to be cut? As a nursing assistant Should I cut them off or I have to notify the nurse in charge?

First of all when in doubt ALWAYS check with the nurse in charge. Certified nurse aides are allowed to cut patient's nails. If the patient is alert and oriented I will ask the patient if he or she wants their nails cut. The patient has the right to have long or short nails. It is a personal preference. I have seen some people in the community with long fingernails that are curling inward. I can't go and cut the person fingernails. So, we have to respect patient's rights. I hope this helps.

Hello Michelle, Thank you for the question. If you want to pass the exam, you MUST put water in the basin and actually wash, rinse, and dry the hands. You can not make believe for this skill. The evaluator will not tell you to place water in the basin. You must know this is expected. Good luck and be safe.

Hello Sylvie, Prometric will not ask a person to do any of the oral care skills due to the Covid epidemic in the United States. Therefore, Dentures, Brushing the teeth, and feeding will not be tested at this time.

Hello Raquel, It will be up to you if you use 4 sides or not. The important thing here is to change the position of the wash cloth when it comes off the body before reusing.

Hello Miyat, Michigan and Texas are the same with regards to the skills checklist. I would advise you to check on the face mask policy for Michigan. In Texas, everyone testing MUST keep their face mask on. Therefore, the oral skills are not tested during COVID-19 (mouth care, denture care, and feeding) in TEXAS.

Hello Chinedu, Thanks for visiting my channel. 1. You can use as many towels as you would like. The ones you don't use, even though they are clean MUST be placed in the dirty linen container because the towels were in the patient area. I teach my students to bring a certain number of towels to the patient room so that the students testing after the person will have towels to test with. 2. Prometric has 22 skills. I have made videos for 19 of the skills. I didn't make a video for the following three skills: mouth care, denture care, and feeding. I didn't do these 3 because Prometric Will NOT test the student on these during the Covid 19 Pandemic. I don't want my students to concentrate on something that is not going to be on the test right now. Prometric requires the student to keep their mask on at all times during testing. Therefore the student can't test on denture care, mouth care, or feeding. 3. The blood pressure skill is not tested under Prometric.

Why are you holding the basin with paper towel after pouring away the water used in the nail care. You held the basin with your hand to the sink, you also held the supplies lotion and and soap with your hand to drop them at the sink. But when want to return them back to drawer or dresser. Hello Tayyaba, I would have 3 large towels and 2 washcloths. 3 large towels = barrier for the table, support the wrist, and to dry the hand 2 wash cloths = soap for washing, rinsing the hand A student can always have more but this is the basic.
