@Lisa Marie Not exactly, the SFX crew had removed the powder charge from the rounds used in the revolver, and reinserted the projectiles so that it would appear to be loaded, but, they didn't remove the primers, which upon firing at some point caused a projectile to be pushed into the barrel, in the following scene a blank round was used, and the blast from the powder sent the projectile flying, unfortunately killing Lee. As an SFX techie myself, I'm a bit puzzled as to why nobody noticed that one of the dummy rounds was missing its projectile, it seems to me that either the weapons master didn't do his job properly, or too many people have shared the responsibility, something like this _can not be allowed_ to happen, obviously.
