This guy speaks facts! My surgeon says the same thing and I had a patella tendon graft! Great recovery man.

Wish I used patellar tendon instead of hamstrings. My semitendinosus didn't regenerated and I had to quit playing football for that reason. My hammy is deformed and weaker and I'm 14 years post surgery. I've done my rehab but it still can't be the same because my semitendinosus is missing. Biggest trauma of my life.

First off, thank you Dr. Hurt for the information. I noticed you didn’t mention quad tendon autograft in the video but stated in one of your comments it’s your go to now. I have ACL reconstruction surgery next Thursdays and I’m stuck between PBTB or quad tendon. I am 34 and hike a lot for work and play soccer, lift weights, etc. I just want to make sure I pick the right one and my surgeon is leaning more to patellar. I know quad is fairly new with not as much research. Please let me know your thoughts. Thank you.

I watched a different video about each graph type and I think he probably didn't mention it because it's the least studied out of all of them. I chose the patellar graph but I hope your recovery is going well. I'm on week 2 of my recovery

Thank you so much. I am getting an acl reconstruction surgery next week and this video was the perfect thing I needed to ease my mind about it. Will definitely opt for the patellar tendon graft.

I did the hamstring autograft for my A.C.L. reconctruction, plus I had an A.L.L. to strengthen the knee. I am glad I didn't do a patellar tendon autograft.

I find patellar tendon graft is too tight which affects my knee bending

I hope you can see this comment? I tore my LCL and the doctor told me that hamstring is better for reconstruction since the LCL is long, but he can do either one. Which one would you recommend?

Hello doc, I would like to know if patellar tendonitis could be permanent? I mean the anterior knee pain, Is it temporary or persistent? In your practice, how many patients end up with persitent AKP? And if so, were they able to participate in sports? I wish to hear from you. Thanks

Great videos Doc! My question is what about using a Patellar bone tendon bone allograft? I would hope you’d get the benefits without the side effects of weakening your own patellar tendon by splitting it, thereby eliminating anterior knee pain with kneeling/squatting… Your thoughts?

Certainly an option but not very widespead. I've actually moved to using quadriceps autograft as my go-to for ACL's. Great results! That said, if you are going the allograft direction (which I do use in revisions or patients over 40) then soft tissue allograft does great with good surgical technique.

Not really. Just fills in with scar tissue.

It's not a frequently used graft for ACL surgery. I've never seen it used in fact. Tibialis anterior and tibialis posterior are much more frequent.

Never say "never"...you are technically correct in limited circumstances. However, in the vast majority (98%) it's really not. If you have ever scoped a knee where the ACL looks like a stick of dynamite was set off in the middle of it you would understand. There is simply no good way to sew that back together and the fluid in the knee makes it hard for it to heal (like gluing wood under water). A tear that is more of an avulsion (where ligament rips off the bone insertion) is your best bet for a repair. That's certainly not mainstream practice right now however.

Patellar tendon pain where the tendon was harvested is definitely a known occurence after this surgery. Could be just how your body responded or could be related to surgical technique (too large a graft taken for instance). I would definitely try a lot of PT with a patient like this but again I haven't seen you so give you any specific advice. I'm so sorry this occurred however.
