28 years in the medical field I can say, this is an outstanding video. During my research nothing has been as educational, easy to follow, & understand. I will be contacting my Local VA about this procedure. They had sent me a pamphlet on PAE to inform me of the process. Thank you very much for this.

I had this procedure with Dr. McWilliams a few months ago and couldn’t be happier with the results. A year earlier I had the Urolift procedure with a local urologist, but that procedure failed and was painful. My prostate was large, 100 cc, which is likely why Urolift failed. I actually learned about PAE watching this video. You will need someone to drive you home after the procedure has been completed and you have rested for a few hours, but compared to other procedures no catheter is needed and for me the recovery was easy. I have to say it is a good feeling to be able to turn back the clock 30 years and get a good night sleep. I highly recommend PAE and Dr. McWilliams.

@@williamosorio1849 I have Medicare so there was no cost. I looked what the doctor and hospital charged Medicare and it was more than $100,000 including CT-scans and consultations before the procedure.

Hi - very happy that this worked out for you. Did they do any scans or MRIs prior to the procedure ? I am considering it but live in Delaware so I need to get it done here and am not sure of the quality / skills here.

I had a TURP 4 months ago (11/29/21) and didn't investigate this PAE procedure because my Urologist said a TURP would solve my problem. I waited too long to do any doctor shopping for alternatives. Two years ago my prostate was 82cc's. I am 77 years old in good health. I was getting a lot of prostate infections and antibiotics were not working anymore. I took 50 days of antibiotics and the minute I stopped the pain came back. So I'll tell you how my TURP went. Surgery was easy. I don't remember anything but taking about 8 deep breaths of some gas and it was lights out. Took about 1.5 hours until I woke up. Stayed in hospital from noon one day until 6:30pm the following day. Catheter was removed in the morning. It was a HUGE rubber hose about .5 inches in diameter. No wonder my urethra is not happy. I had pain and frequent urination bad during day. I think they gave me narcotics twice. I'm thankful for my doctor keeping me overnight. I needed help. Doctor sent me home with no narcotics for pain. I think that is a NY thing after the oxycontin abuse flap. I live alone and was able to drive and take care of myself the next day. I have been back to see doctor 5 times since the surgery because I still have serious pain and frequency after 4 months. He says I have no infections so hang in there. Not much improvement since before surgery in my view. He has given me all kinds of medications and I have tried all the OTC pain pills I can find. None work so I have terrible nights and this has been really hard to endure. The good thing is he removed and sent 22cc-30cc of prostate tissue to pathologist and the results were NO CANCER found. I keep thinking he removed 26-33% (tissue shrinks between surgeon and pathologist) of my prostate with no infection or cancer. That is good thing. Now all I need is to endure until frequency and pain during urination are gone. I worry I have some other problem he hasn't found yet. I am peeing something that looks like skin every urination and that is what causes the burn/pain. I think I am still healing. Weird after so long but he assures me I will get better. Every man is different so that is why you need a doctor to advise you. My only suggestion is don't wait like I did. Find a doctor that does a procedure you like in advance and get ready for the inevitable. Your prostate will do nothing but keep growing. Eventually it may cause an issue so be ready and pray for no cancer. My PSA was 6.6 and I am convinced the bigger the prostate the higher the number. I had several prostate MRI's that said "very low probability of cancer". Never had a biopsy for fear of sepsis. Best wishes to all. Rick H Buffalo, NY 04/02/2022

@@nolanr1400 Hi Nolanr1400 I don't know if you are a doctor or patient but I will tell you the TURP is a really necessary procedure for a man like me. An 82cc prostate is really big and causes all kinds of problems. I have had a difficult and painful road because I now have Follicular Cystitis which is a bladder inflammation of unknown cause or cure. My doctor has me on a 6 medications (four prescription and 2 OTC), since last August, which has significantly improved my quality of life. I would say I am glad I went the TURP route because my prostate was just too big. Don't worry about this problem. You are not alone. Do what your doctor suggests if he has been practicing for awhile. They understand the problem because so many men have it. I am thankful I don't have cancer or infections anymore but it is hard having to urinate so frequently due to this bladder issue. Medications to fix that don't work for me so I am living with issue. Seems like it may be slowly improving. If I can help you with a phone call let me know. Best wishes Rick

I'm thinking of getting PAE but hesitant. My doctor says my prostrate is 215 cc and that is too large for him to do TURP so he recommended I go for PAE. I went for a second opinion and this doctor says he's done many this size, He is in his late 60s, 70s and has more experience than the younger doctors. If PAE doesn't work I'll go to him for the TURP. I hope I'm making the right decision. Does anyone have an opinion?

Can’t say enough about this procedure-life changing. I tried flomax for years, and then Rezum back in 2020. Nothing worked that well until PAE. Mostly sleeping through the night for the first time in years. Recovery during the first week is challenging, but well worth it.

I'm considering PAE for my enlarged prostate \ BPH. Did Flomax & Rezum help your urination symptoms caused by BPH? Was a foley catheter inserted for the PAE procedure? How is your urination now? Do you now take any medication for an enlarged prostate or for pain due to PAE procedure? Thank you

@@johng2116 Flomax didn’t to the trick plus it made me dizzy. The idea of being on meds like that for the remainder of my life was really unappealing. The recovery from Rezum was rough. Had a catheter for 3 days and then lots of pain and bleeding for about a week. Full recovery took a month. PBH symptoms were better for about a year, but then I was right back where I started. I only got 3 injections of steam, so perhaps the urologist wasn’t aggressive enough? PAE was totally different. By definition, no invasive procedure involving the urethra or the prostate. During the procedure, you are under twilight anesthesia and recovery, while challenging for a week, was way better than Rezum. My post procedure MRI shows my prostate shrunk by 50% and is now a normal size. Only complaint is that my ED is a little worse, but that may be more due to age more than the PAE.

I just had PAE this morning. So far for the first night is challenging. It hurts to urinate. There’s no way I can sleep. And I feel like I have to constantly pee all the time. So far tonight the feeling never goes away.and on top of that, I have diarrhea. So they tell me things should be better in 2 to 3 days. We’ll see what happens.

@@christianmani1730 It went well. Had it done May 23. I had some pain that evening in the gut area but it was only for about 12 hours. It's been 5 weeks now and my stream as improved. About 2 weeks ago, went to take a leak. I stood in my usual spot and low and behold I hit the BACK of the the toilet seat lid! LOL. This procedure has NOT affected my sex performace either. A buddy of mine is having it done soon too. I'm 70 years old and this procedure should continue to improve over the next 5 months. I'm happy to report that I've gotten off 2 medications that weren't working for my BPH. Hope this helps.

I emailed this doctor several months ago and I was very happy with his knowledge, advice, rapid response, and his politeness. We actually emailed each other several times over the course of an hour or so. So helpful. I will undergo PAE at the University of Miami Medical Complex in the next few months.

John, I've been having issues with bladder stones which doctor feels it's being caused by an enlarged prostate. I also have a diverticulum in my bladder which is possibly adding to my issue. I'm not wanting to do anything with my prostate because of all the bad reviews a read and still being only 52 years young. Did you have your procedure done and if you did how were your results? I'm also in miami by the way and thanks in advance for your response.

@@HS-cf4lb I am 62 and my doctor told me the younger I am the better I will tolerate this or any other procedure. Of course, the prostate continues to grow and symptoms continue to worsen--not to mention potential damage to bladder, etc. To answer your question, no, I have not had the procedure. I am planning to have it in July or August--delays due to covid and other events. By the way, my prostate is 102 CCs. I take Alfuzosin extended release and it does help, but I do know that this is not a cure. I also have an enlarged prostate lobe (very common). By the way, I actually live in Sunrise. I was 29 when I moved up here--when the last thing I thought about was a prostate. Remember, people are more likely to post about bad experiences as opposed to good ones. You only read about the planes that crash and not the millions of successful flights. Good luck and stay well..

@@johngranato2673 again thanks for the reply , I'm also taking the alfusozin and it's helped , don't have any urgencies , not getting up at night to urinate, not having to go several times a day , it really feels like I'm back to normal to be honest with you. I drink a good amount of water , I also do limes with my water , cut back on all my carbs , dairies, sodas , basically i live the life of a diabetic with diabetes, I lost 35lbs in 8 months , I feel really good except for the whole bladder stones issue. Had 2 surgeries already for then my doctor to tell me I have a diverticulum, why he didn't see it with the first surgery , I don't know , but a year later and passing stones again I had my 2nd surgery which was this past March and here I am again passing stones. I went today to a different doctor at Baptist Hospital and sure enough the first thing out of his mouth was " it's your prostate " with out even reviewing what else I have , so hence my nervousness about having something down that I really know nothing about but what I read online. Your right , I should look for the positive information and go from there and see where it leads me. Anyway, Goodluck with your procedure and stay healthy and safe.

Hey John..Was curious if you had the procedure done yet? If so haw are you doing?I live in Lake Worth Fl and was curious as to which doctor you've chosen for the procedure? I'm currently wearing a catheter as unfortunately I have urine retention due to my 110cm enlarged prostate.Never thought this would be a problem.I'm 60 in very good shape 6' 180lbs workout 4x weekly..Any info would be greatly appreciated

This is just the best talk I have heard in recent years about BPH and other problems related to prostrate. The Doctor has indepth thorough knowledge on the subject and his presentation of the topic is precise and unique, that is what exactly we expect from a medical expert. My salutations tothe Doctor. Amazing, the advances of medical science and PAE is one of them. A procedure much easier for the patient than other options. I had mine done on Oct 20th. BUT, as Paul Harvey used to say “and here’s the rest of the story”. I’ll start with PAIN - for a day and one half after the procedure you will feel pain. Similar to a metal shaft being shoved from your rectum to your bladder. It hurts but after that time period it starts to diminish. Actually mine took 4 days to go away. FREQUENCY - if you think you had to go pee a lot before the procedure, wait until after. Easily twice or three times as often. BURNING - oh yes, yes, yes. FEELING - along with the frequency goes the constant feeling that you have to deficate but you don’t. Day 7 - pain is less but still there, had to call doctor for additional meds. But given the horrendous recovery of other procedures, DEFINITELY worth doing. THE TRUTH AND NOTHING BUT THE TRUTH. GOD Bless.

I’ve had it done and there have been no big benefits . I realize it works for most people though. My urologist who recommended it, is very surprised at my lack of results. Life goes on.

I have 3 stents put in, and the dye is no joke if you have weak kidney or kidney failure. Talk over with your doctor first, this is great news for enlarged prostate.

I had this procedure twice. Six months of success and then unfortunately the veins find a way to grow back around the blockage. My prostate is virtually the same size now as it was before the operations. I hope others have greater success.

At "The Ohio State University", PAE is done in the Interventional Radiology Dept. It is a six hour procedure lying flat on your back and awake. A Foley Catheter and urine collection bag is used. Local anesthesia. Six hours of radiation, Wow! A very long time to be awake and stressed out. Very scary I think. Can't handle that.

@@lowrypierce8054 I’m having this done December 8th - 3-3 1/2 hour procedure at most - leave with foley Cath to prevent irritation and discomfort when urinating - basic prostate reduction is 40-50% - leave same day and don’t get knocked out - my IR guy is great and very knowledgeable 😃

Great presentation, clear and precise. Question... Is all blood supply to prostate blocked, if so will this affect ejaculations, especially those who have vasectomy ? Will the amount of semen reduce as prostate minimizes?

Question: Dr. what happens to the injected beads? Do they dissolve over time? Would you recommend this procedure over TURP, if you needed the procedure? Are there any long term known side effects? My prostate is 153cc. My wife, my daughter and I have all have had in hospital surgical procedures done at UCLA and we have all been extremely happy with both doctors and hospital. I appreciate you video and your kind response to my questions above.

The doctor will not answer based upon a KZbin video. Call the office and speak with them about making an appointment so you can discuss everything with the interventionalist radiologist.

I had this procedure yesterday it was very successful but next day I see a little blood coming out between the folies and the head of the pens but the doctor said it is normal so I am praying for a better outcome.

This is an excellent video. Unfortunately PAE doesn't work for everyone... I had it done about two years ago and the results were... zero effect! The radiologist asserted that the procedure was a technical success and I had no complications, unless you might think that it having on effect is a 'complication'. ☹

How damaging is the dye to the kidneys? Also why hasn’t this become the gold standard?

Its not gold standard because it's not permanent. Arteries re- anastomose in many people rendering the procedure useless. Also long term effects haven't been thoroughly studied yet. But it works exceptionally well in many people

First you need to get information about your prostate. If it is larger than 80 grams, REZUM and Urolift won’t work. Also, If you have a median lobe that would limit the effectiveness of Urolift, and possibly REZUM. My prostate was 100 grams and I had a median lobe; therefore Urolift never worked for me. My urologist told me it was normal to void 200 to 250 ml and the feeling of not voiding completely had to be dealt with. Thankfully, I found out about PAE and got the problem cured. I did consult with my urologist before selecting PAE. He recommended REZUM, but said the insurance wouldn’t pay for the procedure if he reported a size greater than 80g. He also told me incontinence was one potential side effect. I’m so glad I chose PAE. Good luck.

Just a couple of questions. 1) what happens to the prostate after the procedure? Does it disappear or still alive? 2) I'm allergic to iodine, found out during a contras procedure. Are there options for the contras?

Isnt all this caused from a vericocele that is sending blood the wrong direction..so this would be the same fix for a vericaceel,???

Have had the Urolift done twice. Seems the little pins work initially but the prostate keeps growing and spreading around them only lasting about a year. So, maybe this thing this time?

Turp hasnt worked for you? My doc wants me to do turp.It's the only procedure he offers and has told me because of the size of my prostate (11) it's likely the only option that will work

@@bocapete88 I have had HOLEP and TURP and now need to have it again apparently some times this is the case prostate keeps enlarging at a fairly rapid rate, I was told the same many of these new procedures are not suitable for very large prostate

I asked about this over a year ago. Was looking for a doctor who did this in the Chicago area. You gave me a name and I was going to look into it. Unfortunately an elderly parent became seriously ill and required a lot of attention. They have recently passed so I do have more time to address this. PAE usually uses tiny PVA Beads which are therefore plastic and non-magnetic so MRI which as the name implies uses Magnetic Resonance will not interact with them. MRI can "see" soft-tissue in the body because its Water Molecules resonate.

Live here in Alabama , who is ordering Doctor for this procedure , I was told that it has to come from a urologist first and they will try Green light Lazer ,etc. First ,in my case Im 66 with several different health problems going on mainly back and arthritis Problems that I think this PAE I would be more suited for !

I don't believe the information you have received is accurate. Call any office or hospital that has an interventionalist radiologist and speak with them.

If you block off ALL the blood flow to the prostate (or any other organ or tissue), wouldn't that kill it? How does the prostate survive with no blood flow?

I understand it is impossible to block all blood flow to the prostate, only reduce the blood flow significantly to shrink the prostate. I had PAE at UCLA 8 months ago and couldn't be happier with the results.

Can this process work for persons who have had external beam radiation for prostate cancer with a psa of 0.2 but still dribble while peeing. I guess from radiation scars. Was put on flowmax and finestride but

@@ebenezergarbrah5255 I'm not a doctor and I don't play one on TV; however it sounds like you don't need to shrink your prostate, but have issues with scarring. The purpose with PAE is to shrink the size of the prostate, and its efficiency increases with prostate size. Mine was 100 cc so I was not a good candidate for alternative procedures, except for roto rooter ;)

@@arjenpunter3108 Yes. I’m on Medicare so there was no cost. I looked at the bills. Without insurance it would have cost north of $100,000 with everything included. How large is your prostate?

I am actually a believer in natural treatment. My father suffered from BPH and had several surgeries unfortunately, no cure. It was all expensive procedures which exposed his health to risk at 68 years of age. *Read reply to know how he found help*….

I'm almost 75, have non-cancerous BPH, and right now I'm on these two meds (Doxazosin Mesylate 2mg & Finasteride 2mg), but their side-effects have pretty much wiped out my ability to get and maintain an erection ☹️, therefore my QL has diminished. So on my next appointment with my VA Urologist, I'll ask him about this type of procedure to see what he has to say about it. Great presentation btw.

That will be the Finasteride. Doxazosin up to 8mg daily is the max dose for BPH. Having used it as 8mgXL the slow release and 4mg (immediate-release) morning and early evening the 4 + 4 regime seems to be more effective at bladder voiding and reducing nighttime peeing. So try increasing the Doxazosin providing it doesn't cause dizziness since Doxazosin as an Alpha blocker is also commonly used as a treatment for Hypertension (high blood pressure)

Is ejaculation affected after this procedure? if there is no blood flow in the prostate, I imagine men who undergo this procedure become strile. I'd like to know more about the cons of this procedure.

I'm thinking of getting PAE but hesitant. My doctor says my prostrate is 215 cc and that is too large for him to do TURP so he recommended I go for PAE. I went for a second opinion and this doctor says he's done many this size, He is in his late 60s, 70s and has more experience than the younger doctors. If PAE doesn't work I'll go to him for the TURP. I hope I'm making the right decision. Does anyone have an opinion?

I had this done early October 2021 in Sydney Australia. My prostate was 240cc so very large. Had a catheter in for 5 weeks which was most uncomfortable. But the radiologist said that should be the minimum time. Well I’m most impressed with the outcome a good strong flow after many years of weak flow. Unfortunately the worst side affect for me was my back passage had become irritated due to my enlarged prostate. Have had near six weeks of bad toilet runs. I hope it settles down soon as it’s been exhausting. I hope it shrinks by 50% or so even then it will be large but manageable. I will have a MRI late December see what is the shrinkage. Keep you informed…Anyone else on here have any side affects similar to mine?

I didn't have your issues but my prostate was 1/3 the size of yours although I had a median lobe. How are you doing now?

@@ulindmark Hi Ulf , I’m very much better now. I had to see a bowel specialist who put me on antibiotics for three weeks. It was reduced blood flow to rectum. It is now much better and can lead a normal life. He also told me other men have faced the same or worse. Have yet to have MRI to see how much it has reduced by…hopefully half the size. The flow is much better and reduced visits to toilet. Did you have good results? Thanks for your reply Andrew

I had this done 6 years ago and now pee like a horse. No incontinence or impotence. Whatever you do, don’t let a urologist near you...they’re butchers.

No. No incontinence or impotence. Vastly superior proceedure. Stay away from all urologists...they’re butchers. PAE are carried out by interventional radiologists.

@@Jcccc23 I live in Australia. Contact the people shown at the beginning of the video and they’ll point you to your closest practitioner who does PAE. You’ll have to talk to them about your ED.

If you spoke with a urologist, it is important to note the he/she wouldn't benefit financially if you had the procedure. My urologist stated he didn't know anything about PAE.

My urologist recommended PAE and a Dr. who performs the procedure at University of Miami. By the way, if a doctor won't or can't answer your questions, it's time to find one who will. As a patient, you are the customer, you hired the doctor, the doctor works for you! Good luck.

I had this done in 2020. Big mistake. It didn’t help a single BPH symptom of mine and now my prostate is bigger than it was before the PAE! Also had to pay $1,800 out of pocket. Money down the drain. Be warned - works for some guys, but definitely not all.

At "The Ohio State University", PAE is done in the Interventional Radiology Dept. It is a six hour procedure lying flat on your back and awake. A Foley Catheter and urine collection bag is used. Local anesthesia. Six hours of radiation, Wow! A very long time to be awake and stressed out. Very scary I think. Can't handle that.

My prostate is huge ~120 grams- emergency ER failure to pee last week. Have a catheter. Arranged for HOLEP in a week. If I had known of this procedure before, I could have been saved from this. Why don’t doctors inform? I never heard of this procedure until now. But it’s too late for me. Cannot afford to wait a couple of months for it to work while still wearing a catheter. Ugh.

@@williamosorio1849 I tried all the herbs which helped for a while until the prostate became so large it shut off urine flow. I had HoLEP so now can urinate at will. It’s the best procedure for a very large prostate.
