Great explanation as always, u make things easier to understand 🙂 though I had a query, I think before, irreversible pulpitis or pulpal necrosis won't there be first signs of reversible pulpitis ? If truma leads to pulp necrosis or irreversible pulpitis then can u plz explain me how ?

Its mostly based the on pain intensity and duration. With new classification of AAE there no acute or chronic irreversible pulpitits. Its symptomatic of asymptomatic

I am happy that you are watching my video from Libya :):). In reversible pulpitis since the pulp is vital, the treatment would be just removal of the cause. For example a caries in dentin can cause irritation to the pulp by sending toxin through the dentinal tubule. So if we remove the caries and restore the tooth the pulp will return to normal state since its just reversible inflammation as the pulp does not have any microorganisms in it. But in irreversible pulpitis the organisms and entered the pulp and pulp is irreversible damaged. In this case only root canal treatment will save the tooth. I hope this answers helps. Acute may need medication or emergency treatment. Chronic may have mild Symptoms. Overal the treatment is the same

There are many reasons. There may be leakage. Missed canal and multiple reason.. We cant say without seeing the xray. Ideally if the rct is done properly there should not be pain for any stimulants

Because saline is isotonic. In abscess there is inflammation at periapex and hypo may again irritate the tissue.

Sir I have a query? Sir in the case of pulp necrosis... Where the tooth has changed color to brown in course of time, but the history was trauma or he didn't remember of past event and just came into for the change in color In that case there won't be any apical periodontitis or any other sequel as it used to happen only when irritants are present.. Here we don't have any? So only endodontic treatment - rct will suffice the problem?? Can u tell about this sir If this is untreated what would be it's sequel?

Yes. Both thermal and electric tests only give nerve response that is why it has to be correlated with other findings

Both cold and heat test evaluate the nerve response and not the vitality. The the tissue is dead then the nerve will respond less giving no pain on electric stimulation. I hope you got the answer. Let me know if u need any help

@@Aspire32 Many textbooks state that EPT can only tell us if the pulp is vital or necrosed. It does not tell us about inflammation or extent of necrosis. But as you said EPT can tell us about reversible or irreversible pupitis. So does it tell us about inflammation or not?

@@Aspire32 so sir while doing pulpotomy we remove coronal portion of pulp and leave the redicular part then in that case what should we call whether it is reversible pulpitis or irreversible pulpitis .... as the bacteria have reached the coronal part...???

Irreversible pulpitis. But i. Permanent teeth the infection reaches both coronal and radicular pulp very fast, that why u have to remove complete pulp. In primary teeth since the progress is slow, we can remove coronal pulp and leave healthy radicular pulp.

Pulpotomy is also done in young permanent teeth where if the pulp is exposed because of truma, then even if there is slight contamination due to soil, the pulpotomy can be done. Here since its not caries but truma which lead to pulpal exposure, pulpotomy succeed

@@Aspire32 so sir in mature permanent teeth we never do pulpotomy?? And sir ..patient shows symptoms of irreversible pulpitis like spontaneous pain, pain at night e.t.c., only when the whole pulp is infected ?? And thankyou sir... For clearing my doubts and replying so fast.. U r the best sir
