I learned something from your video. You used is when it should have been are, when you spoke of comas. Subject and verb must agree.

Brilliant videos! Could you tell me please when to use coma before whose? I’m referring this patient (?) whose symptoms are suggestive of diabetes? Is there a coma there (?)?

probably. If the adjective clause does not identify which patient you are talking about. If the listener already know which patient, then a comma is necessary.

It depends if the listener or reader knows which "man" you are talking about.
