well presented! of note - you mention few times abotu anaesthesia delivering spinal for labour. Important distinction it is epidural not spinal :)

Basically external rotation during cardinal movement happens when the shoulders about to approach the ischial spine, that is when the widest diameter of the shoulders would get compress by the spine otherwise. This is why we have to externally rotate?

Not sure what you mean by dividing the length of the cervix by two. If the cervix is fully effaced, the length would be 0. 0/2 times 100 is 0. Do you mean the length of how much got effaced/2?

For testing purposes, I think this depends on your current level (I'm still a student). First Aid for OBGYN states it's 4 cm, while uptodate lists 5 cm for multiparas and an indeterminate diameter higher than that for nulliparas.

Ryan Corn newly updated Kaplan notes also mention 6cm now. Numerous other sources mention 6cm too.

According to British guideline (Patient.info) active stage starts from 3-4 cm. So it is all about where you are sitting the exams.
