Someone else pointed out that the one Sammy has is an older version of Randy. If you look at his mouth it's less stiff on the lower jaw where recent Randy has a much more firm form to his mouth comparatively. So I tend to believe that person that the current Randy is a new and improved and this is an earlier version that Sammy has for the skit.

I think it's easy to see how they pulled off this trick. Randy's puppeteer did the work and then quickly gave the first puppet to Sammy. Then the rest is pretty obvious.
