The thing he turns at the end is a rotary valve that adds in the little loop of tubing and puts the instrument in "A" instead of "Bb". (It seems you may not speak English natively. If you are used to a solfege system, the valve changes from "si bemol" to "la")

That is called a quick-change valve. It allows the airstream to be diverted into the extra loop and lowers the instrument a half step from Bb to A. Thank you very much for watching.
