It happens when an object like a rock or spear spawns inside one of the completely blocked den pipes, the game attempts to push the item out of the pipe every second (as with stunned / dead creatures) but it's not able to, and that makes a sound which the lizards can hear every single time (it also attracts them closer to the pipe), this can happen on any room with blocked den pipes.
