On the one hand, it does sound more like Deborah was just coughing rather than choking, but on the other hand, surely Ray could tell something was wrong when Deborah didn’t answer him? Isn’t that kind of an indication that someone can’t talk?

True, but I think we’re being too hard on people for this. During an emergency situation only those who have been trained know how to react. If Ray had been choking Debra probably would have just rolled her eyes or asked what the hell is wrong with you.
