If I were to design an E-Mail client, hitting Reply-All would bring up a confirmation window that says "You chose Reply-All. Are you ABSOLUTELY SURE you want to do send this reply to EVERYONE?

The bank I used to work at, you could send up to $4,999,999.00 without any oversight, once you cleared your probation period of 3 months. Mistakes have been made.

I was supposed to apply a roaming plan to the president’s cell phone before a big trip. I went through the whole process but missed hitting “save” on the last page. She came back with a $1000 phone bill. My boss just laughed and told me to be careful next time!

I disagree with trying to negotiate with your current employer when you have a new job offer. The way I see it is if your current company sees the value in you and didnt give you the raise already (or at least have you on a road map to do so) then they dont care that much about you. If you are mentally already on the way out because you were looking for a new place then you will always be wondering "what if I took the other job" or looking for other jobs. Also when you do that to a company they know you are willing to blackmail them for the money. So they will put things in place to make sure they are ok should you leave (aka they make you dispensable). All of the way around its not a good thing to do. If you are looking and you get an offer then move on. If you are happy then stay and dont look for greener grass.

We once got a mail by someone from HR which was solely gossip, which she had sent to all 6000 employees... Whoopsie! I thinks she was solely reprimanded.

Not me, but a co-worker Missed a period in a payroll input that nearly paid the employee a 50,000.00 bonus and not 500.00

I am a support contractor for the government. Some time during the covid thing, I joined a Teams meeting where you are muted automatically when you join... out of habit I clicked the mic button again, leaving my mic open. I have my own office space and no one was home, so initially all anyone heard was me farting, burping, scratching my nose, whatever. My wife comes home and into my office, guns blazing, using ever cussword in the book... I mean blasting something... not only did a lot of people tell me "hot mic", they called me to make sure I was okay. Now, every time my wife walks in the room I check my mic status. Talk about embarrassing! I am pretty good about calling out my own mistakes, but that one took the cake!

A new employee made a $50k mistake, which was completely reversible, we made her feel better sharing various mistakes we had made including my $1.5M mistake which took almost a week to fix.

I sent out a lot of plans that said "wench" instead of "winch". We use saucy ladies to lift heavy things.

$20k research instrument? That's on the low end for some things. I have a friend who deals in 6- to 7-digit machines, and the users routinely still screw them up.

A manager in my group didn’t add a term date for a highly compensated salaried employee and they received pay biweekly for 6 months after they left. The employee called after it happened the first time and second and the manager still didn’t update the term date. They owed the company over 94K by the time it was fixed.

I feel like some of these mistakes need to include the impact it had on the company. For example, destroying a $20,000 equipment for a small company can be detrimental. However, large companies are shipping out hundreds of thousands dollars worth of equipment and $20,000 is just a write off.

On a project my coworker deleted what appeared to be a duplicate address book in the email system. A few minutes later people started calling as 500 mailboxes had been deleted!
