I had my son 6 weeks ago and had a 4th degree tear. I’m already feeling surprisingly great! Pushing unfortunately was rushed. DS’s heart rate dropped very low for 5 minutes and couldn’t wait and he was vacuumed out. I’m still happy about the birth! Labor was only 3 hours and I was able to do it without drugs! My midwife had me make low moaning noises and it definitely helped with pain management!

I love your videos! I'm British but giving birth in a foreign country where I don't speak the language so naturally I had many questions about pregnancy/labour etc. I always feel much more confident and calm after watching your videos. Thank you!

Thank you very much for this! Please can I ask if there is anything different to consider with a water birth, where I assume the midwife won't be able to look, guide and support with her hands in the same way?

You can work closely with the midwife by listening to her words - if she can see the baby's head x

I don’t remember feeling any urge to push. Could that have been from the epidural? My doctor held the skin and I only had superficial tears though. So that was good!

Yes! I had an epidural with my first, but not the second two and I didnt feel any push urge either with my first.

How do I ask the midwife to help me at the end not to tear? Last time I don't recall being helped to avoid this.. I dont want to seem as though I'm telling her what to do.. I'm hypnobirthing and so I'm worried she'll take a hands-off approach and not help me avoid that last tear...

What about birthing positions? Would being in a squat position or on all fours reduce the risk of a tear? Thank you :) x

I agree that would be useful to talk first about different positions as they reduce pressure on the perineal naturally too.

Yes being on all fours could reduce the chance of having damage, in my experience squatting or sitting on a birthing stool as the baby's head is born increases the chance of damage x

Yes! Perineal stretching excersize before birth and during labor helps tons!

Thank you for the info, just a quick question. I've had a 3rd Degree tear 3 years ago with my son. I have been advised an elective Csection yet I'm trying to find a way I can birth vaginally. Do you think if I have an epidural could that help me to control the pushing as I won't be dying to get the baby out and push when I shouldn't. I'd like to birth the baby slowly but know I may not be able to do so if I can feel the urges! What do you think?

That's a tricky question, having an epidural increases the chance of having an episiotomy (as you are more likely to birth by forceps) x

Is it possible to breathe your baby out rather than push? The consultant midwife I saw at my birth options clinic (following a previous 4DT) said that the midwife in charge would encourage me to breathe this baby out rather than push. I remember with my first that urge to push was incredible and I find it hard to believe that I can counteract all of it with breathing. If so what are your tips on ensuring the optimum breathing techniques?

I never tore with my little boy, I had a slight graze but no tear xx We shall see win no.2 due in August 👍🏼

I didn’t tear. Just superficial tears they said. My daughter is 13 so it’s hard to remember but I think that’s what they said.

I didnt have any tearing that needed stitches. My first birth with a midwife she did perineal stretching on me during labor. My first was 9lbs 14oz, second was 9lbs 2oz, 3rd was 8lbs 13oz. No stitches for any of those births ♡

I have had 3 vaginal births and no tearing with any of them.
