Thanks for this awesome video! Would you mind doing a video over tracheostomy tubes (cuffed versus uncuffed, fenestrated, ones that we can/can't mechanically ventilate a patient with, passy muir valves/ speaking valves, and troubleshooting)? I am having a hard time grasping this subject.

I have really been enjoying your videos! I have a question! In your ICU, do you have those wonderful Stryker inTouch beds that play classical music, nature sounds, and Jazz music? I LOVE THOSE BEDS!!!!!!!

Hey Peter! We don't have those. At least not that we utilize to play music. That sounds awesome. Would be much better than Jerry Springer on the TV.

Hey joe, I’m an RT student and just want some clarification, if they can’t follow directions for a VC because they’re not neurologically there, can they still be extubated? Wouldn’t that affect their ability to protect their airway after extubation? Thank you!

Hi coach! Thanks for this video! Quick question, why do docs ask if they can lift their heads off of the pillow? Is it just to assess if they can follow commands? I was never asked that but I assess all the other parameters prior to extubation. Thanks!

Hi Mitch! Yes, it's a sign of alertness, ability to follow commands, aka mentation. I see it in some SBT protocols related to exclusion criteria.

Hey Joe! What would be a good device to measure Tidal Volume of the patient? Would i only use the ventilator? And where would i check it from the ventilator? Our Facility uses: RR² ÷ Minute Ventilation. Thanks in advance!

In regards to assessing RSBI, dividing the resting minute ventilation by the resting RR will yield an average spontaneous tidal volume and provide you the most accurate RSBI.

another great video! just one question, though. can you elaborate/explain a little more about +cuff leak? and how does that eliminate pulmonary edema? I'm unfamiliar with that technique.

Hey Alex. A positive cuff leak is anytime you have a cuff leak of greater than 15% when assessing prior to extubation. I'll have to rewatch the video regarding the pulmonary edema, because I can't think of a correlation between cuff leak and pulm edema. I'll get back with you.

I think I see what you're referring to. At 16:19 I state, "You have a positive cuff leak, so airway edema is not a problem." The difference here is airway edema vs pulmonary edema. Two different things. A positive cuff leak is a good indicator that your patient's upper airway is not swollen around the ett. A negative cuff leak,

Pretty poor predictor as ideal body weight/ Vt is not factored into the RSBI. There is a big difference between a 5 foot tall female and a 6 foot tall male with the same RR Vt and RSBI.
