Great question! You're correct. Without a known baseline you truly don't know if the bicarb is at baseline or attempting to compensate. The H&P can provide clues along with the presence of chronic physical findings (digital clubbing, barrel chest, etc.) can provide distinguishing insight, but other than that a prior abg is needed to know for certain.
