This is my third retina operation in the past 10 or so years. The first one was a hole detected in my retina about ten years ago which was duly repaired with laser surgery. The second one happened in 2015 when I suddenly noticed that I was loosing vision in my left. I noticed a dark shadow started to appear from the bottom of my eye and gradually increased and by the time I was at the hospital I had lost complete vision in my eyesight. It had effected my macula as well. After the op I had to posture my head down ward for about 10 days. I regained virtually full and clear vision, the only thing was that due to the damage to the macula I had a slight distorted view from the left eye. But all in all I got used to it and did not even realise it as the right eye became the dominant eye. All was well until last week when I noticed a small shadow appearing at the bottom left of my left eye. I immediately went to the hospital eye department where it confirmed that I had a tear. The following I had an operation ether gas was inserted to repair the retina. Luckily this time I do not have to posture downwards but sit upright or lye on my left cheek. Quite sore at the moment. Just hop it doesn’t happen again.

I had retinal detachment and retinoschisis in my left eye. I now have a scleral buckle and silicone oil that will be removed after my cataract is fully developed. I am grateful to the tremendous doctor that re-attatched my retina. I am making a full recovery and feel very lucky that I live in this day and age.

Hi Dr. Wong. I would love to hear you talk about this in the context of children who are presenting asymptomatic. Our 14 year old son had several holes and retinal detachment in his left eye, and had an emergency scleral buckling surgery 12 hours later at UCLA for the left eye and laser treatments in the right for his strophic holes. 3 1/2 months later, he had the same situation in the right eye and less than 48 hours later he had a 4 1/2 hour scleral buckling surgery. We know he has lattice degeneration. He has never seen floaters, flashes or the dark curtain. He is completely asymptomatic which is terrifying. All the information I find talks about a traumatic head injury or being born premature, neither of which are relevant to our son. Thank you so much in advance for any information you can offer. This is an overwhelming new topic for us. His surgery was this past week and in November. He did not have a vitrectomy.

I have had 5 retina detachments, I have buckles on both eyes. I have developed advanced glaucoma in both eyes from all the trauma from the surgery's. Cataract surgery in both eyes & multiple laser and cryotherapy's. I think that's it, but I'm starting to lose track, I can actually see pretty well considering, I have been able to drive up until this week. I have a fantastic retina specialist, he saved my vision many times! I'm having optic nerve trouble now, my glaucoma is starting to win the battle just in the last few months. Stay positive if your having eye trouble, it's amazing the talent of these eye surgeons.

Hi! Thank you for this very informative video. I had my eye exam lately and my doctor said that i have lattice degeneration (left eye) that could lead to retinal detachment and he recommends a laser treatment. I have had floaters (both eyes) for a year now, they are not that many. More like 2 or 3 squiggly lines in my vision. I am also experiencing seeing like white bubble-like things or as if some thin shadow passes through my upper, left side of the eye. I was going to agree to a laser until I have read that most lattice degenerations are not treated but just constantly monitored and that laser scars may sometimes cause other healthy parts of my retina to have tears and eventually detach. I wanted to get a second opinion on this but we only have one retina specialist from where i am. Could you PLEASE enlighten me on this?
@RandallWongMD
@RandallWongMD 6 жыл бұрын
Sometimes lattice degeneration does need to be treated. I can't say specifically in your case because I can not examine you, but if your doctor recommends laser - I'd at least get a second opinion instead of ignoring his/her advice. Randy

Hello Dr. Wong. I have a concern. I have developed floaters in both eyes. I'm almost very certain they weren't there before. I noticed how they started showing up and getting more noticeable. On my right eye there is a floater that significantly stands out almost all if the time. It's a blurry patch when im reading or looking at something and definitely impairs my vision to an extent. Its bigger than the other floaters. Also in this same eye I feel discomfort as if something is in the back of my eye and very mild pain/stinging sensation . Rare but sometimes at night while I close my eyes I can see my vision flashing or if I go from a light to dark room quickly. Before all these symptoms both eyes were very irritated and my right eye was bloodshot as if a vessel popped or something. Im only 22 and I dont think this is normal. What do you think of this based on your experience. It has been going on for about a month maybe more. Seems to me the floaters are increasing. I have had two "optometrists" look at my eye but they didn't dilate my eyes. I felt like it was a very quick brief visit. I need to see a specialist.

Dr. Wong: Great video. Two years ago I had a retinal tear and I was sent to a new retina specialist in a practice that previously only did procedures at the front of the eye like lens implants and LASIK. Two years later I have what is referred to as a 'stretched retina'. I see double flashes every few minutes and objects appear to be very distorted and much smaller in size. There are no straight lines in my left eye. I don't know if there is anything that can correct my eye. Do you have any recommendations?

Oh I am 25 I have to see a specialist for my eyes because I have retinopathy of Prematurity. I was born very premature, detached retinas at birth. They were lasered back in. Well I have really bad eye sight anyway. I'm extremely nearsighted. Can't make out faces. I have to walk close to the person to see their face. I have seen floaters and shadows in my vision and my vision looks like grainy. The specialist does so much testing I am surprised the lights have not blinded me! Last time he held my eye lids open as he was looking in my eyes. He told me my retinas are lasered in tightly, whoever did my laser surgery as a baby excessively lasered them so much scarring. And some myopic degeneration seen in both eyes.... I am not sure what that means

Mr Wong Thank you for your informative video regarding RRD I did my thesis on Mac off RRDs and can safely say that Mac off RRDs should be considered just as important as Mac On RRDs as time to surgery is just as crucial to preserve photoreceptor function and avoid photoreceptor death. If a Mac off RRD is treated within 24-72 hours then the visual outcomes are very favourable.

Hello Doctor Wong! Thanks for this very informative video. My wife went to an ophthalmologist today to get her right eye examined because she said she's been seeing a small orb/spark of light that last for less than second moving from her eye's peripheral to center. The doctor examined her eye for an extensive period but couldn't conclude if there was a tear or detachment. So he referred her to another eye surgeon. I'm hoping she has none of the tear or the detachment. Any chance that these symptoms could be temporary and could heal on their own? Thanks again.

I had congenital cataracts in both eyes, have had them removed at 16 years old, had YAG ladder to remove secondary cataracts in both eyes and had and injury to my right eye after that. Now my ophthalmologist says that I am at risk for retinal detachment due to the acrylic lens being smaller than the natural lense. Maybe I am being delusional, but I really hope that science can find a way to regrow the eyes through stem cells or something.

I suffered a burst of floaters and flashes 3 weeks ago, plus a peripheral shadow, but all 3 opthalmologists failed to find anything. Reading up I discovered that retinography didn't show my peripheral area, so I asked the 3rd ophthalmologist to use a binocular indirect viewer, allowing them to see my retina had puckered up around a haemorrhage caused by high blood pressure one stressed day at work. They can't photograph the area as no one in Spain has a BIO with camera, my peripheral shadow seems to improve some days, but the chief opthalmologist wants to give me a vitreoctomy and laser. What do you think, my vision is still pretty much perfect, should I go ahead?

I did, he recommened a vitrectomy. After the operation he apologised saying they didn't see what was wrong until after they had operated. I had a haemorage in the far periphery, and the blood had lifted the retina. It could have been fixed with gas and laser only, but most ophtamologists are failing to observe the peripheral area, and many cannot photograph that area. There was a paper published on this failing back in 2015, but nothing has changed. Now my vision is worse in the eye due to the vitrectomy, although the gas and lasr repaired the area of the retina, but I will never have 20/20 vision again as before, and of course will need a cataract operation in the near future due to the vitrectomy. I would ask all yoy ophtamologists to start checking the periphery and get ophtamoloscopes with cameras to record problems.

I'm so sorry for the delay in responding - it's hard to keep track of comments on KZbin....at least for me. Colobomas are congenital - a problem with embryonic development. There are several types - some completely benign - others can cause loss of vision. Retinal detachments certain compound the problem, but I can really elaborate with knowing what happened. I'm sorry, limited by the fact I don't have the ability to examine her. Randy

Hm, I've had some of these symptoms and promptly went to my eye doctor. So I'm unsure what else to do.

Tali A. Yes, that's just some jell like substance stuck in your eye from the seperation of solid and liquid in your eye. It may be annoying, but not dangerous and common in most patients. however, if you experience too many floaters I recommend surgery and getting the floaters removed and replaced(not necessary)

TheLoneAssassin Animations yes I know the floaters are fine , my main concern was the fogged over/ blurred vision that obscures my vision and comes and goes along with the occasional flashes I experience. According to my doctor my retina is fine and she couldn't find a cause so I'm just taking eye drops in the meantime to see if keeping my eyes hydrated enough has any effect.

Thank you, Dr. Randall Wong for your explanation of retinal detachment. I am currently feeling that my peripheral vision is slightly compromised in my right eye. I have noticed the skin in the right corner of the eye seems to be out of place. It has shifted over to the left. My right corner, of my right eye is tinged with red. The corner is mostly bloodshot. I noticed all of this after sleeping with some form of night goggles over my eye, where I ended up resting my face on the pillow, and felt some discomfort in the same eye over night. I don't have floaters, nor the flashes described in retinal detachment. I have some degree of discomfort presently, mainly in the right corner of my right eye. It feels like there is a large degree of pressure there. I go to see my eye doctor tomorrow, and hope that he can help resolve this problem as soon as possible. Could this be a sign of a retinal tear? Seeing as the skin of the eye, particularly in the corner, seems out of place. enlarged, or perhaps swollen in some way. Initially, when I noticed the problem, it felt as though I had been punched along that side of my eye. An intense soreness. This hasn't gone away for about 4 days. I am going to see the doctor tomorrow, and would really appreciate it if you could give me some insight into these symptoms.

I have a question about what happened to my eye.first I had cataractsurgery on march 30 2021,and during the operation I heard that vitrectomy done because I think there is a vitreous tear and the operation lasted for more than an hour.after 1 month and 20 days I had visited my eye surgeon and said that I have to go for vitrectomy because I had retinal detachment and they put gas bubble.now almost 2 months already still I have a distorted vision as I see a person or object is small and the face of a person is like abnormal is this normal for now because on the 4th month post operation the doctor told me to remove the suture is there a possibility that if the suture remove can I regain normal vision?.hoping for your reply and thanks

Hi Dr. Wong, Thanks for informative video. My father had complications in cataract surgery a year before and diagnosis with Ratinal Detachment 6 months before for same eye. He did this twice in last six months. Is this normal? Thanks
@RandallWongMD
@RandallWongMD 5 жыл бұрын
1. Cause of retinal detachment is usually a retinal hole or retinal tear. Most "just happen" and are not a result of anything your dad may have done. Complications with previous eye surgery could possibly increase risk - depending upon the type of "complications." 2. May not be normal, but certainly not unusual. Randy

Thank you for such an informative video. I am a bit confused about the timing section .. around 6 mins you describe a situation whereby the macula is already detached and central vision is already lost as "An urgent situation but NOT an emergency" ... yet you describe the situation of "20/20 vision with the THREAT of losing your central vision as an emegency" ... I'm probably missing something here but how can already losing your central vision be urgent but not an emergency, yet the 'threat' of losing your central vision be an emergency ? why isn't the situation of already losing your central vision an emergency.

Thanks for the video, Dr. Wong. I'm a PA student, trying to understand management of retinal detachment when an ophthalmologist is on the way. Can you help me understand the following? "If immediate referral is not possible, the patient should be instructed to lie down with the face on the side of the detachment to the pillow (opposite the field defect) to minimise the detachment extending towards the macula." Ref: Kang, Luff: Management of retinal detachment: a guide for non-ophthalmologists (BMJ 2008) Maybe it's intuitive, but I've run it by a few classmates who were confused as well. Thanks!
@RandallWongMD
@RandallWongMD 6 жыл бұрын
I think the idea is to minimize spread/enlargement of the retinal detachment. The idea is to keep the head and body positioned to keep the subretinal fluid (the area detached) dependent. I find this: 1) almost impossible to know unless you are skilled at examination 2) determining area of retinal detachment by gross visual field may be difficult. My advice would be to get the patient to a retinal specialist before the area of non-seeing spreads and involves the macula (central vision).

Based upon only the comments you made, the prognosis is not great for regaining useful vision; the length of time and the "folded" retina are not great prognostic signs for getting your vision back. I could be wrong, but this is solely based upon what you told me. I am sorry. All the best, Randy
