MEDICAL ANIMATION TRANSCRIPT: The shoulder joint is a highly mobile ball-and-socket joint made up of three bones: the humerus, or upper arm, the clavicle, or collar bone, and the scapula, or shoulder blade. The humeral head is the ball-like structure at the top of the humerus. The clavicle runs between the breastbone and the top of the scapula. The scapula is a flat, triangle-shaped bone that forms the back and top of the shoulder. The glenoid cavity, or socket, is a shallow depression in the part of the scapula closest to the arm. The ball of the humeral head fits into this socket. Articular cartilage is a tissue that covers the humeral head and lines the glenoid cavity. It allows the joint to glide smoothly. Four muscles start at the scapula and run toward the shoulder joint. These muscles are attached to the humerus with tendons, which are strong cords of tissue that connect muscles to bone. These tendons form the rotator cuff, which is a capsule that supports the bones of the shoulder joint and holds the humeral head within the glenoid cavity. The rotator cuff gives the shoulder joint stability, keeping the ball and socket in proper position. The rotator cuff also gives the shoulder joint its wide range of motion. Sometimes, the shoulder joint can become severely damaged from disease or injury. For example, arthritis can erode the articular cartilage in the joint, causing pain and less mobility. Injuries to the rotator cuff, such as a tear, can also cause pain and decrease shoulder joint mobility. Over time, a shoulder with a damaged rotator cuff may develop cuff tear arthropathy, a condition where the shoulder joint develops arthritis due to abnormal motion and instability from the tear. Severe disease or injury of the shoulder joint may need to be treated with a joint replacement. A standard shoulder replacement uses manufactured materials to create a new ball for the head of the humerus and a new socket for the scapula. However, when disease or injury of the shoulder joint is complicated by a severely weakened or injured rotator cuff, a reverse shoulder replacement may be necessary. A reverse shoulder replacement allows the deltoid muscle in the arm to take over movement of the shoulder joint from the weak and injured rotator cuff. In this repair, the position of the ball and the socket are reversed. The socket implant is attached to the top of the humerus, where the ball or head used to be. And, the ball implant is attached to the scapula, where the socket used to be. In a reverse total shoulder replacement, the procedure will begin with an incision over the shoulder joint. The surgeon will separate the head of the humerus from the shoulder joint …and remove it. The humerus will be hollowed out in preparation for placement of the socket implant. Next, the inside of the glenoid cavity will be reshaped to prepare it for placement of the ball implant. A metal base will be attached to the scapula with screws. The ball will be screwed onto the metal base. The socket implant is on a metal stem. It will be inserted into the humerus. Finally, the surgeon will put the new joint together by inserting the ball implant into the socket implant. At the end of the procedure, the incision will be closed with stitches.

It had been TEN YEARS of intense pain and misdiagnosis of my shoulder issues, -then a two and one-half year wait and several months-long delays due to discovered and now resolved medical issues (irregular heart rate, and apnea,) but I have finally been cleared and ok'd for TRSR next week!! I am so excited to finally get this done! THIS is going to be LIFE-CHANGING!

Typically they are just moved out of the way, if they’re torn and need repairing then they’ll address them but otherwise they clamp them out of the way of the joint

This is good to know as I had this surgery done on the 24th Nov after a fall on the 18th. On the left shoulder. I play guitar and sing as a way of life and a living. Hopefully I will make enough of a recovery in time to go back to this living but it will take time.Luckily I have small pension which just kicked in on my 65th Birthday on the 31st Oct. I this had happened earlier in my life I would have been toast financially.

I had a shoulder operation about 20 years ago which worked very well. It is now very painful and difficult to use, a scan shows that all the tendons are snapped and there is a rotator cup tear. I have tried for about six years to have it repaired. I also have had two left shoulder operations. The first was done by a trainee surgeon and monitored by an orthopaedic surgeon. I was told the joint just needed cleaning out. The trainee surgeon did a lot of damage. I was left with a shoulder that was very painful and difficult to move. I later found out my shoulder had been damaged and now had a 30mm rotator cuff tear and snapped tendons as well as a snapped bicep tendon. The surgeon who had been training the trainee surgeon refused to examine me after the operation and informed me there was nothing more they could do. I was forced to see a second surgeon. He tried to repair the tendons and rotator cup tear but was unable to repair them which left me with a shoulder I cannot use. It has taken me many years to find a surgeon who will help me. I have been told I need both shoulders replaced. My question is. Can torn tendons be repaired when I have these operations?

Hello Laura! I am slated for this surgery this coming May 2nd for my right shoulder. I had rotator cuff surgery on the same shoulder back in 2013. I have great range of motion, I do have some slight tearing, but the arthritis is effecting my quality of life. I love being active, weight lifting, exercising, kayaking, etc. Can you please tell me how the recovery went, how was the pain level the next day after surgery and going forward please. My Doctor and everyone else in the office says the recovery is not that bad. How are you now? How is your range of motion? Thank you in advance!

@tony2tiger1 Good morning. I'm very blessed because the operation brought me back to totally normal. I follow my doctor's orders regarding physical therapy & was extremely cautious to do only as advised. I have an extremely high pain threshold, so I was truly only a bit uncomfortable. The only thing I couldn't do was lay flat. I purchased a wedge & still use it when I'm uncomfortable. My only advice would be to BECAREFUL! Go easy on the physical activity. God forbid you fall & the shoulder is injured. The ability to repair is almost nil. I pray God will send an angel to watch over you & guide the hands of your surgeon

@@lauracamuso6950 thank you so very much for your reply, and well wishes. Can you send me a link to this wedge? It's a pillow wedge I presume. How many days in the sling? My surgeon "Dr.Gobezie" is telling me just a few days in the sling. I have great range of motion, maybe that's why? But yes, slow and steady.

In December 2023 I fell and Fractured (well shattered really) my left Proximal Humerus. In 2019 I had surgery on that same shoulder to repair a large rotator cuff tear. I have played the violin since I was 10, I'm now 62. Since the fracture 7 months ago I have not been able to play. I just had a CT scan of that shoulder and I may have to have the Shoulder Replacement Surgery. My PT seems to think I need it. After my surgeon reviews the CT scan I'll know more. My question is, will I be able to get the rotation that I need in order to play the violin again after having this surgery? I'm still a very active violinist (well, I was before the fracture) .

I was in a similar bind regarding musical instruments anf my left humerus being broken in a fall on very hard ground. A reverse shoulder replacement has been done in order to give me the best chance to recover sufficient flexibility and strength to play guitar again. I am right handed but the guitar demands almost equal dexterity in the left hand and arm when forming chords and scales to be picked out by the dominant right hand. Because I made clear my desire to continue playing guitar to my consultant the decision was made immediately replace the broken joint.

The inability to get a full 8 hour nights sleep at the moment is a right royal pain. This condition will take months to recover. I am on solpadeine, 1 gram 4 times daily, the max allowed and also ice packs 4 times daily to keep the swelling down and pain reduced. The tricky aspect is to avoid tensing of the neck anf fore-arm muscles as the body subconsciously tenses neighbouring muscle groups to minimise expected pain.
