Had rotor cuff surgery at age 70 and it failed 11 months ago so I just had reverse shoulder replacement seven days ago. Thank you for explaining what I’ll be looking forward to in the coming weeks. I couldn’t find no one else putting out a video like this except for you.

I am 75 years old and I had this surgery 8 weeks ago- after having a "Hemiarthoplasty" 3 years ago that left me with a stiff and "aching" shoulder-- this surgery has given me much more range of motion and NO PAIN--except for "stretched" muscle burn from exercise-- (use an ice pack after exercising). Physical Therapy will take you much faster through your rehab than this video shows-- I was out of the sling for all intents and purposes in 2 weeks while around the house and driving once again; and then doing stretching and motion exercises at 4 weeks; and now, at 8 weeks, am using 3 pound weights and 30# strength bands for resistance exercises. I think having experienced the failed previous surgery has allowed me mentally to do More and Faster this time around, knowing what to expect-- but all with the PT people and the doctor in agreement. I attend PT classes for guidance 2 times a week; but I do those exercises EVERY DAY at home. I expect to start practicing my golf game in 2 weeks, and playing once again 2 weeks after that. Don't think that what I do is any different than what someone else who wants "get better" can do-- you just have work at it, and while being careful not "over do" anything, do each exercise with full commitment.

I had a reverse total shoulder surgery on my right shoulder nearly 9 years ago. I did my own physical therapy. I had two other failed surgeries before it ,due to formal physical therapy..with a therapist who was too aggressive... both times . My shoulder dislocated, and separated both times .Not Fun at all....

Things you should know from my personal experience. I exploded my shoulder in a work accident. I had the old school repair done first, didn't work. Then they completely rebuilt the shoulder using cadaver skin. It failed, then they went in and did a scope. Cleaning everything out of the shoulder. Then with no option left, they did a complete reverse shoulder replacement. The only thing it did was get rid of the non stop, never ending pain. I'm extremely thankful for that. But i only have less than 25% usage of my dominant arm. If your shoulder is extremely damaged and has been through multi surgical procedures. Don't build, or fool yourself up into thinking. This surgical procedure will fix everything, cause it won't. My surgeon told me our best outcome. Would be ending the non stop, never ending pain. We did that so I'm forever grateful to my last surgeon for that. This isn't my opinion, this is exactly what i went through. In a two year and three month process. Nobody should have to go through that many surgical procedures. In that short amount of time. I wish i would have had the surgeon. That did the last two surgical procedures from the start. Instead of going through two surgeons before him. Maybe things could have turned out differently.

I'm in week 10 of recovery so it would be unfair to say go for it. I definitely no longer have inflammation (arthritis), but I anticipate a minimum of three more months of recovery -- lots of physical therapy and patience.

I very much appreciate straightforward teaching videos like this one. I'm doing research to decide if I will have RTSR surgery. I fell and pulled my right humerus out of its socket and down an inch a half or so. I began PT right away. To my amazement, my muscles have pulled my humerus humerus back up a half inch toward its correct placement. It will never go back to where it was, though. I'll wait another five months, have some new x-rays made, and talk again with the expert surgeon who would do the surgery if I decide I want that. It has been surprisingly difficult to get the information I need to understand the benefits and downsides of each choice. There are some unknowns, of course. Even so, I need to know as much as I can to choose the option that is most likely to be a good one for me. Thank you for this helpful video.

Your reverse shoulder replacement video was the most useful I have found. Your coverage of PRE surgery exercises was especially helpful because, although I had performed those exercises after prior surgeries years ago, I had wrongly "given up" because of my increased pain before this surgery. I also appreciate your captions of many of your most critical instructions so I can make screenshots that are easy to follow. For instance, "Most times in sling 100% of time first 72 hours " I need your specific instructions. Your video was complete and straightforward, and I can easily keep reviewing the things I might otherwise not do, like "icing several times a day...". My only suggestion from prior surgeries is that you suggest people buy one of the five-foot by 3-inch foam swimming pool poles and cut it into one-foot lengths to support your elbows rather than rolled-up towels. The foam is lighter, and easier to grab and hold than a folded towel.

After watching this video I can see the need to move slow with exercise. I like to golf so hoping this surgery allows me range of motion.

Great video. I am 74 and was in surgery a week after the diagnosis. (8 years post fauled rotator cuff surgery) Your 19:16 instructions varied some from my Dr. He just wants me to do the pendulum along with an assisted sawing motion and an assisted curls for the first month. Then, to the assisted broomstick close to my chest for 4 weeks, then the broomstick assisted raised straight arm in front the next 4 weeks. I have the pulley, but I forgot to ask Dr. if I should use it. Love the video. I do think it jumps around a bit.

Hi rebbeca I had this same operation on the 16th January 2023 prior to this I had 2rotarty cuff tears same shoulder cartilage repair on my other shoulder I received 6 physio appointments then they said I could have no more because it looks bad on there feedback I am also on gabapentin which I’ve been prescribed for nerve pain in my spine due to 2x collapsed discs resulting in sciatica having pain going down my arm to the top of my hand if it wasn’t for garden gate hitting my shoulder full on I would not have had surgery i to wish I had not had done I know what you are experiencing and you have my deepest sympathy are you uk based I’m due to see my surgeon inappropriately 2 weeks have cut my gabapentin down from 3x600mlg to 3x150mlg cannot work out after 6 hours if it’s withdrawal from the surgery/or the medication I hope you recover the worst 4months of my life I am 68 years old and up to the operation I used to work 7 days a week doing timber restoration and exterior work painting/decorating i yours improves regards Paul

I am 52 and I just had a complete shoulder replacement in my left shoulder. Osteoarthritis bone on bone. Doctor said I have a shoulder of a 90 yr old! 😅 five weeks postop still out of work on STD, most likely will be in till first week of February, at least. Going to physical therapy three times a week but it’s only six weeks I believe I thought that was kind of a low amount but then they give me home courses.

Thank you for your comment. Be patient with yourself and follow the plan given to you by your surgeon and PT. It’s important to focus on quality of movement over quantity. Minimize substitutions of neck and wrist.

excellent. Mine was a quad break of the proximal humerus. I didn’t have much of a choice. I had some minor nerve damage but it’s corrected over time. Still have some numbness over the skin area of my affected shoulder. The time/gradual progress for PT was and is a mystery. My surgeon wanted aggressive rehab right out of the gate. I’m a medical doctor so I iced, did some mild passive ROM exercises. I’m past week 16 and can only lift my arm ninety degrees. It’s a different ball game when it’s traumatic vs degenerative. I can use light dumbbells and resistance bands. At this point It’s minimum 5 days a week for at least 40 -50 mins. 1-2 dats a week won’t get the job done. i’m expected to be skiing and playing basketball by December. That makes about 7-8 months.

I am 74 and four weeks out of left shoulder total reverse shoulder replacement. In the sling when doing nothing. The rest of the time I keep the elbow against my side. I had been exercising before surgery with Battleropes and TRX. Never had any problems with the ropes. TRX on some always kept elbow in to side. It the humerus was destroyed by arthritis it had to be replaced. I had good strength before surgery and it is helping tons. Today is your weeks but no lateral movement.

There is no reattachment of rotator cuff or subscapularis after reverse shoulder replacement. It has been debated in subscapularis but many surgeons feel that reattaching the subscapularis actually inhibits external rotation. But supraspinatis , and infraspinatis muscles are not reattached. And many of us have massive tears which make it impossible and this is why we are having the surgery to begin with. But the fact that they’re not reattached is the reason for the reverse replacement instead of a standard shoulder replacement.

I see the Dr in a couple days.. I think I will do Carpal Tunnel surgery first then the reversal I fell holding a glass sliding door wall. Never let it go. Yelled my leg my leg which turned purple from knee to butt.. and FAT I didn’t think about shoulder as I worked General Motors Truck plant assembly line 30 yrs Shoulders always ached but as time went on I am unable to raise to use arm So surgery here I come .. Hoping for the BEST

As I’ve had other shoulder done when I was 24 from bad car accident I threw a rope over shower rod and would pull arm up that way.. then I would lay in bed raise arm to headboard .. going to was hard but bring back down was very hard but I persisted and the arm they said would never work again .. works amazing they ran a ?? a coat hanger down my arm and whatever at shoulder .. crushed collar bone. Shoulder, arm and ribs I was young and wouldn’t give up.. but I’m afraid I might do less with my arm after surgery then now .. from some things I read.. WOW I usually don’t rant on but I feel you understand I love your routine for repair I’m so impressed thank you!

excellent and informative video. I'm having RTSA in about a week. One note however - If I could do the pre-surgery range of motion exercises you show I wouldn't need this surgery! For those with arthritis we often can't rotate out to the side much at all or get our arm much above shoulder height.

I’m two weeks postop tomorrow still going through basic PT here at my home and start physical therapy on December 2. Looking forward to regaining function following left reverse shoulder replacement had two prior rotator cuff attempts and they lasted a good long time, but my rotator cuff was completely torn so although there was not a lot of pain prior to my surgery, I couldn’t lift my left arm above 45° to gain as much over head function as possible. Prior to my massive rotator cuff tear I began having severe neck and shoulder blade spasms. I also have problems with c3-4 and c7-t1 so I thought that was the reason. Now I am wondering if it has to do with my shoulder. My infraspinatus and Supraspinatus are both completely torn and retracted. I’m wondering could this be the cause of my spasms? Also there will be no rotator cuff attachment after a reverse replacement! So doing rotator cuff exercises are not needed!

Thank you for a video on reversed total replacement this is the 1st 1 ive found could you do a video on rehab on LONG TERM ROM STRENGTH like 1-2 yrs out i had a 1 with a humourus elbow replacement almost 2yrs ago ( i had a crushed shoulder broken arm ribs ×4yrs ago and had intial shoulder replacement and arm kept breaking and eventually went compound) i need a rountine to devolpe strength post 2yrs on arm im grateful they didnt amputate tht was the other option 😮 so if you could do a video on this niche issue i would be gratefull thank you

Best ways to avoid scar tissue? I had a TKR two years ago, and working through the scar tissue was tough. I did have a good overall recovery with my knee. I’m hoping to stay ahead of the game with development of scar tissue on my upcoming (reverse) shoulder replacement. Thanks.

Great question! Gentle regular movement of your shoulder will reduce scar tissue formation. The surgical techniques down today minimize the amount of scarring as well. You should be in great shape following the care guidelines from your surgeon and physical therapist.

Wish I had never had this surgery. Left me with nerve damage in my arm and hand. My hand is now deformed and I drop things. The feeling in my arm and hand is horrible. Have to take gabapentin and it does not much good but does help me to be able to rest at night. When I woke up from surgery I told the nurses something was wrong with my hand. They did nothing. I had a total knee replacement, rotator cuff and carpal tunnel surgery and they were perfect. Same surgeon. But I pray I never have to have another surgery because this shoulder surgery done me in!!

Sorry to hear you had such a poor outcome Rebecca and wound up with nerve damage in your hand. This is definitely not the norm and more than 95% of patients have a successful outcome to this procedure.

I had damage to all 3 nerves in arm and have a numb hand and can't make a fist. Barely touch top of my head. Seeing a chiropractor next week. Am thinking I have calcific tendonitis. No pain, thankfully.

I was 55; horrible osteoarthritis. Bone on bone with a rotator cuff tear. The Doctor did a rotator cuff repair with a hemi replacement. I am in so much pain 2 1/2 years after surgery. I’ll be 58 years old this year ; I’ve had two opinions, one doctor will do a reversal; the original doctor won’t because of my age.

Sorry to hear about your situation. I would try an get your function right now. 58 is young and having function right now should be your top priority. Please seek out skilled physical therapy care as well.

So I don’t think I have many options 🤷🏼♀️ Also, I’ve had foot fusion( twice) both hips replaced, back fusion, neck fusion, right major rotator repair! Yes, I AM A MESS!😭But I’m tough!💪🏼

Hi Dr. Waldron I am 13 weeks out from shoulder replacement surgery and I still have trouble lifting my arm past 90 degrees infront of me. I have a torn rotatorcuff and my surgeon and I agreed to wait 6 weeks to see if things improve,but if they dont would a reverse shoulder replacement help someone like me that is very active?

Great video Dr. Waldron, I'm 79 and had my reverse shoulder surgery about 7 weeks ago I'm worried because I can only get 20 - 30 degrees of motion with the cane push(rotator). I've been in therapy for 3 weeks(3 times a week) and do the major exercises 1-3 times a day, but still have a lot of soreness. The therapist says that my shoulder is very stiff and will take a lot of work. Should I go easier until the pain goes away? - Andy

Hello Andy, thank you! Definitely follow the instructions of your PT. A stiff shoulder it’s a good idea to respect the pain and do your exercises 3-5x a day in more gentle bouts than going more aggressive 1-3x a day. Be patient with your body and remember to do the same exercises on your non operative shoulder so you don’t get neck pain.

Had this done June 2020, best thing I ever did. No more Arthritis pain and my doctor said I'm a freak because I can now raise my arm almost as high as my good shoulder. Only thing I can't reach back or put on a belt but who cares? I don't lift any weights but I can still golf and actually hitting it further than before

Isn’t one of the post-op precautions for rTSA is avoiding extension past neutral as well as IR and adduction which is pretty much hands behind the back?

Great question Warren. Yes you will be careful with motions behind your back the first 6 weeks. After 6 weeks of you have normal healing most patients begin work on these motions gently. Of course follow your physician and physical therapist’s instructions for your case.

I had Balloon Surgery at NYU LANGONE .. after eight weeks in a sling and seven weeks of physical therapy three times a week and MRI now shows I am worse now than I was before the operation… apparently my only hope is reverse shoulder replacement which I’m having done April 26 by Dr. Jonathan Levy…I am 67 but extremely active ..racquetball and weightlifting…Just curious to know if I can return to the sports and if so how long and any limitations

Having multiple surgeries or procedures done can change or influence your potential return to sports. This will need to be a discussion you have with your surgeon and physical therapist. In general most RTSA can return to sports at 6 months to one year at a graduated progression and racquetball would be fine as long as you don’t run into the wall.

The initial surgeon told me they were two options depending on how much arthritis was in the shoulder..#1 if there wasn’t too much arthritis he would be able to do the balloon surgery..#2..If the arthritis was too much there would have to be a reverse shoulder replacement done and he does not do that operation.. after the operation he told me he was able to do the balloon surgery…. now he’s telling me the reason why the operation failed was because there was too much arthritis..Sorry to vent ..again thank you for your time and guidance

@@charliebutera1198 Can empathize somewhat - had a failed "partial" shoulder replacement 3 years ago-- was told it might take a "couple of years" to get rid of the pain-- after a 2 1/2 years, no improvement-- so I inquired of another Dr. who recommended a RTSA -- operation was 8 weeks ago-- and the results "night and day" better-- I am pain free and well on the road to playing golf in another 6 weeks, if not sooner. By the way, a youngster like you should fare much better-- I am 75!

I had a total right shoulder reverse replacement Fe 2nd this year and am wondering if you have a video for this surgery? Some of the exercises you have do not work with my surgery. I have no rotator cuff. At 76 I cannot afford PT for months on end and want to get as good a recovery as I am able. Can you help?

Thank you for watching Ginger. I would recommend getting a few visits of PT to get pointed in the right direction and then use the posted video if your PT agrees. You have already spent a lot of money on the surgery, a few visits of PT would be well worth the expense. I understand that everyone has different situations and I hope this advice helps you.

I hade frozen shoulder by posterior dislocation before 4 months How long does it take for my shoulders to be normal by performing above video exercises

The pillow is supportive for the shoulder and keeps the optimal level of tension on the cuff while allowing for good joint positioning for healing.

Generally within the first three weeks depending on your surgeon and how you are recovering. You will be in a sling the first 4 weeks and will need to ice and might start PT the first week to help reduce pain and get instructions on how to care for your wound.

Hi Jeff, and thx for this awesome video. I have a reverse shoulder coming a week from today. I stopped doing the floor based, dumbbell locked to hip dumbbell against gravity work a few months ago. Listening to u I realize that I should have kept it up - instead I've been power walking up a steep hill and strongly swinging my arms - will this help me in recovering, or just the wrong motion? Also, is one week doing the moves in this video going to be helpful, or have I waited too long? Thx again, really appreciate the PT wisdom!

What if there is pain doing normal activities - lifting coffee cup - book - at top of bicep and crunch noises after six months post 2ed reverse (1st failed). I am a left leg amputee which complicates this?

Hi Alf, you will need a physical therapist to help give you more optimal movement strategies. Crunching and pain especially after a revision, your body is substituting and will need guidance to alter these movements and get support from muscles that alleviate pain and allow increased function.

That will limit your ability to lift your hand off your back when reaching behind. But luckily you have other shoulder muscles that internally rotate like the Lat muscle Teres Major.

I have had 12 shoulder surgeries on left... 10 was a total reverse shoulder replacement. I have had a open wound since 2016 when it was done... I was told that I can only get 15_20 yrs out of this surgery. I'm 43 now do I was 37 when It got done. Is this true there a expiry date on this type of surgery

12 surgeries is a lot on one shoulder, sorry to hear you went through so many procedures. It’s hard to predict how long a joint will last and there are just general guidelines of 15-20 years. Do the best you can with self care to keep it healthy and mobile. Maybe have yearly check-in appointments with your local PT to update exercises and get advice on appropriate activities.
