Rh incompatibility in pregnancy nursing NCLEX maternity review: management, pathophysiology, Rhogam shot, and hemolytic disease of the newborn.
What is Rh incompatibility? It occurs when a father who is Rh-positive and a mother who is Rh-negative have a baby that is Rh-positive. The baby's Rh-positive blood will leak into mom’s circulation (most likely during delivery), and this will cause the mother's to build antibodies.
The baby of the first pregnancy is not typically harmed. However, the baby during the second pregnancy, who is Rh-positive, will be affected because those antibodies created during the first pregnancy are in mom's circulation. These antibodies will attack the baby's red blood cells.
This will lead to hemolytic disease of the newborn and cause: anemia, jaundice, heart failure, swelling/edema, splenomegaly along with hepatomegaly.
The key is to keep mom from being sensitized, which is done through giving her a medication called Rhogam. This medication is given at 28 weeks and again at 72 hours after delivery if the baby is Rh-positive.

If mother becomes sensitized with antibodies, and she cannot take rhoGAM. Is every pregnancy from now on at risk for complications: anemia, CHF, jaundice, etc? Or just the second pregnancy is affected and subsequent pregnancies with RH+ baby are fine?

I’m not in nursing school but I am pregnant and I’m RH negative and wanted to look deeper into what that meant, what it does, and just be informed 😇 loved this video and now I understand why I got the Rhogam shot. Just 7 more weeks before I see my baby 👶🏻 🌸

I am RH- pregnant with my first child, his father of course. is RH+. tomorrow I'll be getting a shot to hopefully prevent me from becoming sensitized to RH+ blood, here's hoping. I was so scared about what this could mean for my baby and any future babies I am to have, but knowing exactly what's going on in my body and baby's body makes me feel confident to at least (somewhat) know what I need to talk to my doctor about. Thank you for these informative videos.

I am so happy that this information is available now. It was not when my friends child was born 25 years ago and things went sideways because of the blood incompatibility. Fortunately there was time for a blood transfusion that saved the baby. Thanks so much.

This is very crucial information for me. I have the extremely rare AB negative blood type. I am 33 and been thinking about motherhood more and more, I had no idea about this. So now I'll be making sure I get with a man of a negative blood type lol also I teared up when you were talking about what would happen to the baby with positive blood type ......

thats a grear video, in my case im the baby rh negative and my dad has the same blood type, my mom is rh positive tho, so it was a huge problem on my delivery.. i think it could be great if you do a video with those examples, many people dont know about this subjests yet. i know actully lots of pple that dosent even know their blod types

Serious question - can someone explain to me the logic behind putting antibodies into your bloodstream while pregnant, as a means to prevent antibodies from getting into your bloodstream the next time you’re pregnant?

I have Rh- blood and have had 4 babies who were all born Rh+. I only got the Rhogam shot after delivery. I always asked about the one at 28 weeks but they said I didn't need it. They did do an Indirect Coombs test to see if I needed it, but said I was fine. My babies were/are all fine. I'm wondering if this varies by country. (PS: I had all my babies in Japan).

The problem with the Rhogam shots is that they carry severe and deadly adverse events - Intravascular hemolysis (IVH) leading to death has been reported in patients treated for immune thrombocytopenic purpura (ITP) with Rho(D) immune globulin. IVH can lead to clinically compromising anemia and multi-system organ failure, including acute respiratory distress syndrome (ARDS), acute renal insufficiency, renal failure, and disseminated intravascular coagulation (DIC). Alert patients and closely monitor for the signs and symptoms of IVH in a health care setting for at least 8 hours after administration for ITP. Perform a dipstick urinalysis at baseline, 2 hours, 4 hours after administration, and prior to the end of the monitoring period. If signs and/or symptoms of IVH are present or suspected, post-treatment laboratory tests should be performed. Hemolytic reaction includes hypotension, nausea, chills, headache, and a decrease in haptoglobin and hemoglobin. Disseminated intravascular coagulation, Intravascular hemolysis, hemolysis resulting in death, clinically compromising anemia, extravascular hemolysis, hemolytic reaction, etc.
