D&D did a great job when they had material to adapt. So sure, they should partially be blamed for the show going off the rails, but part of the blame goes to GRRM for being a fat, lazy sack of shit who won't finish his books

Technically they did, as the books are titled differently. Game Of Thrones refers specifically to the television show.

But as you said they are different names - 'Game of Thrones', the show; and 'A Game of Thrones', the book - so game of thrones should only refer to the show.
