There are several potential causes of right to left shunt in neonates, which refers to the abnormal flow of blood from the right side of the heart to the left side without passing through the lungs. Here are some common causes:
1- Congenital heart defects: Certain heart abnormalities present at birth, such as extreme form of Ebstein's anomaly, pulmonary atresia, tricuspid atresia and critical PS.
2- Persistent pulmonary hypertension of the newborn (PPHN): This is a condition where the normal decrease in pulmonary vascular resistance after birth does not occur, resulting in right to left shunting.
