Great video but I don't like how much he emphasizes punishment for those accountable. It takes great courage for a person who is accountable for a mistake to come forward and take responsibility. I'm not saying being accountable should be rewarded, but it shouldn't be punishable in every instance, especially in understaffed hospitals. Put any health professional inside a hospital with inadequate resources (including staff) and they're doomed to fail, so why punish them when failure was inevitable from the start?

Adam Milne wilful neglect is avoidable and indefensible, senior management, the NHS Trust, the CCG should be held accountable.

NHS Staff are still being silenced and seen as “problem staff” if they raise concerns. This is due to the poor management who are seen to encourage a duty of candour on one hand and slapping staff down with the other. So who do you go to when poor managers protect each other and allow bad practices to continue and punish those who stick their heads above the parapet? Cultures like this are systemic in the NHS and just contributes to decent staff leaving the service.
