"I made sure to work with Monk's real changes and not the Real Book ones." Why did the authors of the Real Book write the chords incorrectly? Was it intentional?

The standards were written with the language of the musicians at the time so every decade of standards reflects its style. The real book was written by guys in that era of the music meta so a lot of the changes reflect it and we got a lot of 2-5-1s out of it :)
