Rukia isn't think she has liability because "She can't save anyone", it is more like at that very moment her response isn't "I need to do this to save Sir Kaien's heart", Rukia's body frozen at that moment and in her eyes, she didn't do what Kaien thanks her for, she didn't realize by doing this it saved Kaien's heart, she thinks she did that in order to save her own life, although the result is the same, she believed during the process her mind put her own life above Kaien's issue, which is why she feels guilty and think she "killed" Kaien.

Rukia was in Kaien's unit. Because Ukitake is often sick, he's basically the vice-captain. I think she blames herself for letting her captain fight alone, which led to his death. And fact that Kaien was able to take control of his body at last moment and fall into her sword makes it even worse. She'd rather die with her comrades, I suppose.

Kaien does look like the dude with the 69 face tat. But he was the one Rukia, Byakuya, and the 13th captain was reminded of when they saw Ichigo, because they have the same face and similar personality.

I can’t tell you the specifics because I don’t want to ruin the surprise regarding the details but without giving too much away I can say this, that there is a heartbreaking tragedy in his past that shook him to his very core that changed him. But as to what that trauma in his past is I’m afraid I cannot say because the explanation behind it is just too good when details are revealed so you’ll just have to keep watching until you find out for yourself. But I can promise you that it will be worth the wait, especially given that you’re one of my favorite reactors to Anime.

You can see how Kaien and Miyako resemble Ichi and Orihime. Thats why Rukia treats them as younger siblings.

As far as the Japanese are concerned, honor is like the biggest thing through their history. It's equal in fervor to American patriotism. Historically, dishonor in Japan led to committing ritual unaliving of yourself. While I get your sentiment of fuck honor, Kaien having help in that fight would be considered a high moral and emotional wound that he wouldn't recover from. Ukitake is very honorable, and he knows that if Kaien won with assistance, it would be as if Kaien had lost his life. Kaien was alive when the Hollow fused with him, so while it was complicated and not a straight forward situation. They couldn't separate him ever. He had assisted suicide through Rukia. She is prone to blaming herself. As far as Ukitake, he was sick prior to him entering the Academy. It's a miracle he survived. Byakuya is the head of a noble house. They play by the rules. As far as the "doesn't concern you" situation, it actually doesn't as far as Soul Society is concerned. She committed a crime, which makes it a matter of that family. He's giving Renji the textbook answer. Only the noble house in question really has any standing to contest the situation.

Byakuya is the head of a Noble family. Rukia was close to her Captain and Lieutenant despite not being a Seated Officer (There are 20 seats including the Captain and Lieutenant). Worth noting as there are hundreds of members in a squad. Though the Captains are usually as strong as the whole squad combined. Captain Ukitake is a good man and probably one of the strongest Soul Reapers if his health wasn't so poor.

You can say she blames herself at this moment because she just want to find an excuse to die. By doing so she allows herself to accept her fate and have a peaceful end.

Kaien is supposed to look like a black haired Ichigo. This is why they repeatedly referenced that he reminded Rukia of someone. The guy with the 69 tattoo has the same look. Seems to simply be a case of Kubo reusing the same design. Probably the worst of his design, but I think every other character is top notch.
