Just a note, you should be consistent about whether or not you include 'Dr.' in the title of the video. My mom is a doctor, and she has noticed a pattern of male doctors being called by their title but not female doctors in the same context.

Yeah, I was disappointed to see that this video is the only one that doesn’t use “Dr.” I’ve heard female doctors and academics say the same thing-they’re often not given the title when their male colleagues are.

Ahhhhh....the vasovagal dance. I had personal experience once. My ophthalmologist was doing a SLT (trabeculoplasty) on me, and during the final phase... I told him "Jerry, I'm getting vasovagal." His response, "Oh come on, no you aren't!" I said, "Yes I am...I'm diaphoretic and nauseated!" Next thing I knew I was on the floor, with some surprised medical assistants and an even more surprised ophthalmologist!! I got the final word: "Jerry, this wouldn't have happened with Dr Glaucomflecken!" I received only a Jonathan-like nod.

Until I was in my mid twenties, I thought the spleen was a made-up joke organ created for popular culture to keep TV shows light-hearted. It has such a funny name that "ouch, I ruptured my spleen!" just didn't sound plausible to little kid me, so I ran with that assumption for another 20 years. English being my second language contributed to this. My native "Milz" sounds much more legit. Never doubted that one!

Haha I can imagine you can be skeptical about the spleen's existance. ONce you figure out Santa's not real, you start asking questions about other things they make you believe and test whether it holds up. What's the purpose of the spleen? "It's a very important for your immune system." What happens if your spleen is removed? "Not much." Riiiight

I think most of us in the 90s and 2000s thought that the spleen was a made-up joke organ, it was the punchline in so many cartoons. I would ask adults what it was and what it did and nobody could tell me.

😂 that’s the best giggle you just gave me😅 my grampa was a surgeon in ye olden days and he used to make us learn how to pronounce random long words. I have a couple of his med school books from the 1930’s. I once described a procedure he removed a cyst on my dads hand via slamming a wicked big textbook onto it to a doctor and she did not believe me. I started thinking “shoot was he putting me on about that?” (Did I basically invent the spleen?😂) But I found it in the textbook when I got home so Ha! It did exist lol. Damn I’m getting so old😂😂❤

That was awesome. I'm a family practice stock and I do fair amount of GYN. I also do a fair amount of ophthalmology due to eye injuries in a rural area. Not that I'm an ophthalmologist or that I treat the patients but can evaluate them to determine whether they're in danger or not. So being on both sides of the table on this one makes this all the more fun thank you.

Well now I have two data points and I am enjoying the heck outta this podcast! I think it's fascinating to hear that ping when doctors go through and start to really understand how hard it is to deal with the hellish confluence of problems that happens between administration and various types of insurance in the US (not to even mention policies or other issues), and then just how obscene both of those parts of health care can get to a point of seeming to collude to deny access to healthcare for patients when they are at really vulnerable and critical moments. I can attest as a now disabled former highly educated researcher and health advocate and policy nerd that it freaks me out how hard it can be to save myself at times, my family, my friends and the large community of people with my condition that have grown in my city and state. I've seen some improvements followed by catastrophic problems, and have seen some fights won after a lot of challenge and faced heavy losses. Almost everyone that I know though is either very educated and fortunate or is related to someone who is like that. We know that less than 1% of us are properly diagnosed, let alone cared for, and I think that's the haunting thought in hearing these stories always is... it shouldn't be this close to impossible to get life-saving treatment in the US, but if it is for us, what hope do the rest have? That's why I think these very human stories are so important for those who haven't faced it YET, and yes please keep picking on insurance companies and the rest of them because the only thing that will save everyone else from this (aside from the help of the great humor and the laughs) is for people to understand that they are broken and need to be fundamentally changed.

I have a rare genetic kidney disease called Cystinuria. I passed my first stones when I was 8 and needed surgery (they gave me lithotripsy because it was the latest thing in NZ and it badly damaged my kidney). That experience in hospital made me decide I wanted to be a nurse, I told Mum it was because "They do more than doctors". Sadly, I discovered in the first year of nursing training ten years later that my Essential Tremors made it too difficult to do the practical work when I was too stressed or anxious. 12 years after that, I finally got diagnosed with Cystinuria and I became a professional patient instead :D

There is a Fundus in the theater 🎭 at least in German the Fundus is where all the costumes and requisites are stored. Your props for your characters are in a Fundus.

I ruptured my spleen in a bicycle accident in the 80s. ER said I knocked the wind out of myself and sent me home. 24 hours of writhing and back to ER and spleen blotto Never the same since

That's a typical accident! Like on a bike - sudden stop - handle bar into stomach. Or skates, suddenly a bar blocking off the road - waist high. Or the 3 rd one : horse riding, horse stops and drops it's head, the rider glides to the neck, clinging onto the horse almost hanging on it's head, the horse jerks it's head up into the riders torso/ abdomen. The normal prozedure for non acute bleeding was : 11 days TOTAL BED REST WITH MIN. 2 TIMES BLOODWORK/ day , FULL MONITORING ( so that if a small injury that further ruptures and starts to bleed , the vital signs and blood work will give it away asap --> emergency OP , maybe "packing" the spleen in gauze to stop the bleeding (aka patching it up) and if it doesn't stop or get's worse - emergency removal as the last option. Seen an 11 y.o female rider who had a small rupture on her spleen exactly happening like I tried to describe it. She was ok after the 11 day watch. But NO EXCUSES TO STAND UP OR STRETCH FOR AT LEAST 7 DAYS, NO TOILET, SHOWER , NON LIEING FLAT, LEGS MORE IN A POSITION AS IF YOU HAD CRAMPS AKA PILLOW UNDER THE KNEES AND HEAD - PART OF THE BED AT A 30 - 45 DEGREE ANGLE, so the stomach area isn't flat or even stretched at all. After 7days, a toilet break might be allowed...to see if it's ok still after a 5 minute period in an upright position and more blood flow to the area....

Can you please talk to someone, or declare outright, that the vasovagal response IS NOT VOLUNTARY?? I have had nurses, and even doctors, tell me to "just don't hold your breath" or "Don't worry. You'll be fine." These comments are given to me even though I am breathing regularly and well, and when I am perfectly calm. But I do faint when I am in great pain, and I DON'T WANT TO. I am a veteran. 5 years of active duty in the Navy. That's where I found out I will faint at the sight of my own blood. Not anyone else's blood! I can grab an open wound and save someone's life, and I'm perfectly fine. But if I see my own blood, and I know it's my own blood, I will faint. It's embarrassing as hell. So I'm here to tell you, the vasovagal response is NOT VOLUNTARY.

The vasovagal discussions were 😂 I myself go vasovagal very readily - have to drink water, lie down and not look when they're taking blood for tests - and did feel queasy and faint at the mention of pulling on the cervix and trapped eyeball muscles 🤪🤮

I have always said it is important to advocate for yourself and have someone with you to advocate for you when dealing with the medical system. Sadly, the Canadian system is also broken.

Another similarity between Opthalmology and Gynecology: people put off their exams far longer than they should. Edit: Also, sudden eye floaters. If you're not pregnant, you call your opthalmologist for them, if you are pregnant, you call your gynecologist.

@@andrewhegstrom2187 at least when I was pregnant, the provider wanted to know if there were any sudden visual changes, including floaters, bright spots, or partial vision loss, because any of those can be symptoms of preeclampsia.

@Mathnerd that is a big sign of symptomatic pre-eclampsia, along with the other symptoms. From an ophthalmic standpoint sudden floaters with maybe a flashbulb effect could be a sign of retinal detachment but usually floaters are no big deal. A "veil" coming across your vision, holes in your vision (scotomas) or rapid onset loss of peripheral clarity are usually the most urgent ones. From my memory of what they teach us of eyes in nursing. Which is surprisingly not much.

Ophthalmology doesn't make me nervous weirdly, I like hearing about eyes! It's gynecology things that make me very nervous. I think my eyes are just normal to me and eye appointments are normal to me bc I'm nearsighted and have dry eyes so I'm used to dealing with my eyes a lot. Eye fluids like tears don't gross me out bc my eyes water all the time lol.

I had two splenic artery aneurysms and had to have a transfemoral coil embolization. Developed post embolization syndrome. Hospitalized for 3 days for post op pain control as a result. Was the most thing sinful experience of my life. Don’t even remember being in the ER for 8 hours. I puked in the parking lot going in. The pain couldn’t be controlled with IV dilauded, IV Tylenol, and oral hydrocodone, administered alternating every 2 to 4 hours. 100/100 do not recommend. Wish they had just removed the entire thing but they said where I have Lupus, MCTD, autoimmune hepatitis, and now I also have developed lymphocytic colitis, that I should probably be able to fight infection with more than my shotty immune system lol.

I have done the 10/10 pain where one just flips in and out of reality. Ruptured spleen is about a very good 8. Could only just walk very very slowly and the pain in the ambulance was fekking high when it hit some tiny bumps.

Lady G is a boss. Make her dean of a med school! Why didn’t I have any training on how to talk to patients??!! Why did I spend so much time on Kreb’s cycle and random minute chemical cascades but no training on dealing with difficult patients, deescalation, how to break bad news, etc? I’m an attending and STILL have no training on those things. I did my FM residency training in Appalachia, and everyone asks what church you go to there. I was NOT expecting that question to follow me to California, but it does. They are not happy to hear “I don’t.” Why is this an appropriate question to ask your doctor?!?!

It's not normal or appropriate. Maybe they are just trying to make conversation like talking about the weather. Maybe you can just ask them if they have one they can recommend to you, when they ask what church you go to. Then ask them how much it costs to get in. Lol

The reason O2 was lost at 6 months is because ALL commercial companies expect ALL sick kids to go on Medicaid ESPDT. As per the provider being out of network the aut for the hospital covers the physician. A biller, that wants to rat out the whole system.

Hi just to say anything involving eyes freaks me out especially mine, I came across a youtube video where this guy was getting a mould of his eyeball taken it was a little cup thing being filled with something that looked like a rubber solution, to take an impression, I turned it off at that point I think it was in south America is that a real thing that gets done

Part of the problem is that the hospitals and insurance companies make the patient responsible to pay but give zero transparency or control for the patient. My gyn office put in the wrong code. Because their clerk didn't know how to do their jobs, i was on the hook for COVERED PREVENTIVE HEALTH. Insurance says the code is wrong. Me: Okay, whats the code? Ins: can't tell ya Me: okay what does the code need to be? Ins: can't tell ya, you need to call your doctor. Then have the exact same conversation with the incompetent clerk that couldn't do their job right the first time. Will they be fired? No, their bosses have no way of even knowing that the clerk is screwing their patients over.
